Dictionary of Transaction Error Messages

About This Guide The Dictionary of Transaction Error Messages contains a composite list of all transaction error messages that can be returned from the Emdeon data center across all payers and transactions. It is designed as a reference tool in which you can look up the meaning of any transaction error message you receive in a response.

In the response, an error message prints as a two-letter prefix (such as CL, HT, or RH) followed by a three- or four-digit number and a line or so of text.

The error messages in this guide are listed numerically by the number. The two-letter prefix is omitted.

Occasionally you will see a message at the end of your response. Such messages are used to convey pertinent information about Emdeon products, such as program changes, new product releases, or additional transactions or payers that are available to you. Call your Emdeon sales representative or Emdeon Customer Support if you would like more information.

001 No Response From Host

The payer did not respond to the transaction. Retry the transaction. If the problem persists, call Emdeon Customer Service.

0001 Account Suspended. Call Customer Service

Prefix CL: Your Emdeon customer account is not active. Call Emdeon Customer Support.

0001 Insufficient Data for Request

Prefix PC: Max attempted to perform an auto MCO transaction, but a default provider ID was not set up for the payer that was automatically queried.

0002 Incomplete Customer Information - Please Call Customer Service

Prefix CL: Your customer account information is not complete. Call Emdeon Customer Support.

0002 Incomplete Customer Information - Please Call Customer Service

Prefix HT: Your user ID and password has not been set up. Call Emdeon Customer Support.

0003 Insufficient Information To Complete a Search-Please Call Customer Service

The Medicaid number submitted contains unacceptable characters, or it is the wrong length.

0011 Invalid Card Control Number

0012 Invalid Member Number

The member number submitted contains unacceptable characters, or it is the wrong length.

0014 Invalid Social Security Number

The Social Security number submitted contains unacceptable characters, such as letters, or it is the wrong length.

0015 Invalid Date of Birth

The date of birth submitted contains unacceptable characters, such as letters, or it was in the wrong format.

0016 Invalid Gender

The gender code or description submitted contains unacceptable characters, or it is the wrong length.

0017 Invalid Date of Service

The date of service submitted contains unacceptable characters, such as letters, or it is in the wrong format.

0018 Invalid Beginning Date of Service

The beginning date of service submitted contains unacceptable characters, such as letters, or it is in the wrong format.

0019 Invalid Ending Date of Service

The beginning date of service submitted contains unacceptable characters, such as letters, or it is in the wrong format.

0020 Beginning Date of Service Must Be Before Ending Date of Service

The beginning date of service submitted is chronologically after the ending date submitted.

0021 Date of Service Older Than One Year From Today

The date of service submitted is more than one year in the past, based on the submission date.

0023 Date of Service Older Than nnn Days From Today

The date of service submitted is more days in the past than the number of days noted in the message, based on the submission date.

0024 Future Date of Service Not Allowed

The date of service submitted is in the future, based on the submission date.

0025 Range for Dates of Service Greater than nnn Days

The begin and end dates of service submitted span more than the number of days noted in the error message.

0026 Duplicate Recipient

More than one record was found using the data submitted. Retry the transaction, using a different search type.

0027 Duplicate Name

More than one member was found using the name submitted. Retry the transaction, using a different search type.

0030 Provider Not Authorized

The inquiring provider is not authorized to access the payer’s database.

0031 Provider Not On File

Prefix HT: The inquiring provider is not set up in Emdeon’s database for this payer. Prefix RH: The inquiring provider is not in the payer’s provider files.

0032 Recipient Not On File

The recipient’s record was not found using the search data submitted.

0033 Recipient Not Eligible

The recipient’s record was found in the payer’s data files but the recipient is not eligible on the requested date(s) of service.

0034 System Not Available

The Emdeon data center did not receive a response from the payer. Try again later. If the problem persists, call Emdeon Customer Support.

0035 Host Timed Out Waiting For Response From Carrier

The Emdeon data center did not receive a response from the payer within the predefined response window. Try again later. If the problem persists, call Emdeon Customer Support.

0037 Invalid Transaction ID

Call Emdeon Customer Support.

0038 Invalid Transaction Type

Call Emdeon Customer Support.

0045 Future Date Of Service Beyond Current Month Not Allowed

The date of service submitted is beyond the end of the current month, based on the submission date.

0046 Provider Not Eligible

Applies to Missouri Medicaid: The inquiring provider is not eligible to submit claims on the current date (submission date). Call the Division of Medical Services Provider Relations Unit to verify your enrollment status.

0047 Required Request Parms Missing

Call Emdeon Customer Support

0048 Name Not On File

The name/date of birth combination submitted did not find the patient’s record in the payer’s database. Retry the transaction, using a different search type.

0049 Social Security Number Not On File

The Social Security number/name or Social Security number/date of birth combination submitted did not find the patient’s record in the payer’s database. Retry the transaction, using a different search type.

0050 Medicaid DCN Not On File

The Medicaid DCN submitted was not found in the payer’s database.

0051 Medicaid X-Ref DCN Not On File

The appropriate Medicaid DCN Cross-References were not found in the payer’s database.

0054 Function Not Available

A temporary condition has arisen on the host database which is not allowing transactions to process. Wait briefly, then retry the transaction. If the problem persists, call Emdeon Customer Support.

0055 Host Processing Error

A temporary condition has arisen at the payer’s mainframe which is not allowing transactions to process. Wait approximately 30 minutes and resubmit your request.

0057 For More Info Resubmit Per Instructions

Applies to Missouri Medicaid: A portion of the service dates submitted has non-restricted coverage, but the other portion has restricted coverage. Either the beginning date of service or the ending date of service of the response will be different from the dates you entered in the request. Run a separate transaction for the portion of your service dates not covered in the response.

0058 Claim Not Found

No claims were found for the provider, subscriber, and date of service submitted.

0067 Invalid/Missing Patient ID

Your request did not include a patient identifier, it is the wrong length, or it contains unacceptable characters.

0068 Invalid/Missing First And/Or Last Name

The first and/or last name submitted contains unacceptable characters or it is the wrong length.

0071 Multiple Eligibility

Call Emdeon Customer Support.

0073 Invalid Provider Number

The provider number submitted contains unacceptable characters, or it is the wrong length.

0079 Invalid Provider ID

The provider number submitted contains unacceptable characters, or it is the wrong length.

0082 Incorrect Request Or Version Format

The payer’s database is currently unavailable. Try again later.

0099 Invalid AHCCCS ID

Applies to AHCCCS only: The AHCCCS ID submitted contains unacceptable characters, or it is the wrong length.

0100 Future Beginning Date of Service Not Allowed

The beginning date of service submitted is in the future, based on the submission date.

0101 Future Ending Date of Service Beyond 30 Days Not Allowed

The ending date of service submitted is more than 30 days in the future, based on the submission date.

0102 The HAWI ID Entered Is A Secondary HAWI ID. Correct HAWI ID: xxxxxxx

Applies to Hawaii Medicaid Eligibility only. The Hawaii Medicaid identification number submitted is for a secondary record. The correct ID is noted in the message.

0103 Invalid Recipient ID Card

The card number submitted contains unacceptable characters or it is the wrong length.

0104 Duplicate Response

More than one record was found in the payer’s database using the data submitted. Retry the transaction, using a different search type.

0108 Invalid Recipient ID

The recipient ID submitted contains unacceptable characters, or it is the wrong length.

0109 Future Ending Date Of Service Not Allowed

The date of service submitted is in the future, based on the submission date.

0110 Invalid Member ID

The member ID submitted contains unacceptable characters, or it is the wrong length.

0142 Invalid Request Issued

Applies to BCBS Alabama Benefits only: The request was sent for a non-PMD physician.

0147 Provider Number Not Present

The provider number submitted was not found.

0150 Patient Not Found

The patient’s record was not found using the search data submitted.

0150 Patient Not Found Matching This Criteria

The patient’s record was not found using the search data submitted.

0152 System Error

There was a data length error or a maximum packet error. Call Emdeon Customer Support.

For Address Verification, Payment Predictor, Medicaid Predictor, and Charity Care transactions, can indicate that the entity submitting the transaction is not authorized to send this transaction to the payer.

0156 Recipient Eligibility Pending For Services On DOS

The patient is recognized by the payer as a covered individual, but the status cannot be determined at this time.

0158 Search Type Must Be 01, 02, 03, Or 04

Call Emdeon Customer Support.

0159 Member Not Eligible For Dates Of Service

The member’s record was found in the payer’s data files but the member is not eligible on the requested date(s) of service.

0160 Member Not On File

The member’s record was not found using the search data submitted.

0161 Unable To Determine Group ID. Please Call Customer Service

Call Emdeon Customer Service.

0162 No Subscriber On File For This Member

The subscriber was found, but the dependent patient was not found using the data submitted.

0163 Unknown Error

The Emdeon data center received an error from the payer that it did not recognize. Call Emdeon Customer Support.

0173 Search Criteria Format Invalid

Applies to BCBS of Arizona Security Registration only: The security registration failed when it reached BCBS of Arizona due to an improper format. Retry the transaction; make sure you enter the correct provider information. If you receive this message again, call Emdeon Customer Support.

0205 Invalid Access Number

Your request did not include an access number, it is the wrong length, or it contains unacceptable characters.

0211 Invalid Referring Provider Number

Your request did not include a referring provider number, it is the wrong length, or it contains unacceptable characters.

0214 Invalid Service Units

Your request did not include a number of service unit, or the number of service units entry contains unacceptable characters.

0231 Authorization/Access Restrictions

The entity submitting the request is not authorized to trade information with this payer.

The provider identifier submitted does not match the provider identifier on file for the member’s primary care physician.

0238 Service Dates Not Within Provider Plan Enrollment

The dates of service submitted are not within the provider's enrollment in this plan.

0239 Inquired Benefit Inconsistent with Provider Type

The provider does not supply the benefit or service submitted.

0240 Inappropriate Product/Service ID Qualifier

The product or service ID qualifier submitted is not consistent with the product or service ID submitted.

0241 Inappropriate Product/Service ID

The product or service identifier submitted is not suitable for this member or provider.

0242 Inappropriate Date

The date of service submitted is not suitable for this member.

0244 Date of Birth Follows Date(s) of Service

The date of birth submitted is chronologically after the date(s) of service.

0247 Patient Not Found

The patient was not found using the data submitted.

0249 Inconsistent with Patient's Age

The benefit or service submitted is not suitable for the patient's age.

0250 Inconsistent with Patient's Gender

The benefit or service submitted is not suitable for the patient's gender.

0251 Patient Birth Date Does Not Match That for the Patient on the Database

The patient was found, but the date of birth submitted does not match the date of birth on file.

0252 Invalid/Missing Subscriber/Insured ID

Your request did not include a member identifier, it is the wrong length, or it contains unacceptable characters.

0253 Invalid/Missing Subscriber/Insured Name

Your request did not include a member name, it is the wrong length, or it contained unacceptable characters.

0255 Duplicate Subscriber/Insured ID Number

More than one member was found using the subscriber or insured identifier submitted. Retry the transaction, using a different search type.

0257 Subscriber/Insured Not in Group/Plan Identified

The group/plan number does not match the group/plan number on file.

0258 Invalid Participant Identification

The provider identifier or participant identifier submitted does not identify a provider or payer.

0259 No Response Received - Transaction Terminated

The Emdeon data center did not receive a response from the payer. Try again later. If the problem persists, call Emdeon Customer Support.

0268 Invalid Member's Plan Code

The plan code submitted contains unacceptable characters, or it is the wrong length.

0272 Group Number Not Found

Applies to the Sloans Lake Managed Care transaction: The group number that you entered is not on file with Sloans Lake Managed Care. Try the transaction again and use a group name search.

0274 Subscriber Number Not Found

The subscriber number was not found in the payer’s data files.

0276 Member Number Not Found

The member’s record was not found in the payer’s using the member identification number you entered. Retry the transaction, using a different search type.

0278 Benefit Package Not Found For Group

The benefit package for the identified group was not found in the payer’s files.

0300 Recipient Number Not Numeric

Applies to Illinois Medicaid Supplemental transactions only. The recipient ID that was sent with the request contained characters other than numbers. Call Emdeon Customer Support.

0301 No Data Found For Inquiry

Applies to Illinois Medicaid Supplemental transactions only. There is no claim in the Illinois Department of Public Aid (IDPA) files matching the input information by which you searched.

0303 Transaction Timed Out At IDPA

Applies to Illinois Medicaid Supplemental transactions only. Consultec (who facilitates access to the Illinois Medicaid data files) did not receive a response from the Illinois Medicaid data files within a predetermined period of time. Wait a few minutes, then retry the transaction. If the problem persists, call Emdeon Customer Support.

0304 IDPA System Not Available

Applies to Illinois Medicaid Supplemental transactions only. The Illinois system has indicated that it is temporarily unavailable. Files may be closed for maintenance. Wait briefly, then retry the transaction. If the problem persists, call Emdeon Customer Support.

0309 Date Of Service Greater Than 8 Years From Today

Applies to Illinois Medicaid Supplemental transactions only. Your requested beginning or ending date of service extended into the past further than eight years from the current day.

0310 Invalid Document Control Number

Applies to Illinois Medicaid Supplemental transactions only. The Document Control Number (DCN) you entered is not valid. It must contain at least 13 digits.

0311 Member Not Found With This Name

The member was not found using the name submitted. Retry the transaction, using a different search type.

0314 Member Not Found With This ID#

The member was not found using the identifier submitted. Retry the transaction, using a different search type.

0315 Member Not Found With This DOB For Name

The member’s record was found using the name submitted, but the date of birth on file did not match the date of birth submitted. Verify the data and try again.

0316 Multiple Members Found For This Name And DOB

More than one member record was found using the name and date of birth submitted. Retry the transaction, using a different search type.

0317 Member Name Not Found For This Social Security Number

The member’s name submitted was found in the payer’s database, but it did not match the Social Security number in that record. Retry the transaction, using a different search type.

0318 Multiple Members Found For This Social Security Number And Name

More than one member was found using the Social Security number and name submitted. Retry the transaction, using a different search type.

0319 Member DOB Not Found For This Social Security Number

The member’s record was found in the payer’s database for the Social Security number submitted, but the date of birth you submitted did not match the date of birth in that record.

0320 Multiple Members Found For This Social Security Number And DOB

More than one member was found using the Social Security number and date of birth submitted. Retry the transaction, using a different search type.

0325 Member Files Not Available

The payer’s files were not available. Wait a short period of time, then retry the transaction. If the problem persists, call Emdeon Customer Support.

0330 Provider ID Cancelled

Your provider ID has been cancelled. Call Emdeon Customer Support.

Applies to BCBS of Alabama Benefits only: The Provider ID number submitted was not on file, or it was added today.

0356 Password Not Matched

The payer does not recognize the provider’s password. Call Emdeon Customer Support.

0375 Security Error

Applies to BCBS of Arizona Eligibility only: Your security registration transaction has not yet taken effect. It takes BCBS of Arizona up to 24 hours to process a registration transaction. If you have questions, call BCBS of Arizona Provider Assistance at 602-864-4320 or 800-232-2345 ext. 4320.

0392 Invalid DCN Begin Date

Applies to Illinois Medicaid Supplemental transactions only. The DCN beginning date range that you entered was in the wrong format (for example, it was only five numbers) or you entered numbers that did not represent a real date.

0393 Invalid DCN End Date

Applies to Illinois Medicaid Supplemental transactions only. The DCN ending date range that you entered was in the wrong format (for example, it was only five numbers) or you entered numbers that did not represent a real date.

0395 Beginning Document Control Number Must Be Numeric

Applies to Illinois Medicaid Supplemental transactions only. The Document Control Number contained invalid characters in inappropriate sequence. Please check the DCN carefully and retry the transaction.

0396 Invalid Document Code

Applies to Illinois Medicaid Supplemental transactions only. The two-character Document Code you entered was invalid. Please check your data and retry the transaction.

0397 Document Code Required With DCN Date Range

Applies to Illinois Medicaid Supplemental transactions only. You must enter a Document Code when also searching by DCN begin/end dates.

0399 Continue Information Not Requested Correctly. Please Call Customer Service

Applies to Illinois Medicaid Supplemental transactions only. There were more claims on IDPA’s files than can be transmitted in a single response, and the Emdeon host did not request the subsequent claims from IDPA correctly. Notify Emdeon Customer Support if you receive this message.

0410 Future DCN Begin Date Not Allowed

Applies to Illinois Medicaid Supplemental transactions only. The DCN beginning date range input was greater than today’s date.

0411 Future DCN End Date Not Allowed

Applies to Illinois Medicaid Supplemental transactions only. The DCN ending date range input was greater than today’s date.

0415 Invalid Response, Please Call Customer Service

The response sent to the payer was not in the correct format. Call Emdeon Customer Support.

0431 Invalid Relationship Code

You entered an unacceptable relationship code.

0433 Invalid Claim Type

Call Emdeon Customer Support.

0436 Internal Database Error. Please Contact Customer Service

Call Emdeon Customer Service.

0447 Invalid Card Issue Date

The card issue date submitted contains unacceptable characters, or it was in the wrong format.

0448 Name/SSN Not On File

The member’s record was not found in the payer’s database using the Social Security number and name submitted. Retry the transaction, using a different search type.

0449 Name/DOB Not On File

The member’s record was not found in the payer’s database using the name and

date of birth you entered. Retry the transaction, using a different search type.

0459 Inquiry Type Must Be (N)IPS Or (P)harmacy

Applies to Illinois Medicaid Supplemental transactions only. You entered an invalid Inquiry Type on a NIPS/Pharmacy Claim History inquiry. You must enter either N for a NIPS Claim or P for a Pharmacy Claim.

0476 Unidentified System Error

Call Emdeon Customer Support.

0491 Password Missing or Invalid

0493 No Match Found

No files were found matching the search criteria you entered.

0499 Invalid/Missing Date of Birth

0542 Provider Not Valid For Logon ID

The provider ID submitted is not on file with at the Emdeon data center as a valid provider for this payer.

0547 Subscriber or Insured Not Found

The subscriber’s or insured’s record was not found in the payer’s database.

0553 House Number Missing Or Invalid

Your request did not include a house number or address first line, the data is the wrong length, or it contains unacceptable characters.

0554 Street Name Missing Or Invalid

Your request did not include a street name, the data is the wrong length, or it contains unacceptable characters.

0555 City Name Missing Or Invalid

Your request did not include a city name, it is the wrong length, or it contains unacceptable characters.

0556 State Code Missing or Invalid

Your request did not include a state code, it is the wrong length, or it contains unacceptable characters.

0557 ZIP Code Missing or Invalid

Your request did not include a ZIP code, it is the wrong length, or it contains unacceptable characters.
