By SubwayDuring rush hour, take the Flatbush Avenue Line (#2 or #5 Lexington Avenue) trains to the Winthrop Street station. [Take any Brooklyn-bound train (#2, 3, 4, or 5) to Nevins Street in Brooklyn, changing there for a #2 or #5 marked “Flatbush Avenue”. Note that the # 5 runs only during rush hours.] Exit at Nostrand and Parkside Avenues. Cross Nostrand Avenue and walk one block on Parkside Avenue until it ends at New York Avenue. Turn right onto New York Avenue. Cross New York Avenue and walk east on Lenox Road until the entrance at 395 Lenox Road.

PROGRAM DESCRIPTIONBased on patient and physician feedback and literature reports, we believe that the transition from Chronic Kidney Disease (CKD) to dialysis can be eased by enlight-ened management of both CKD and Dialysis patients.
TARGET AUDIENCEThis educational activity is designed to improve knowl-edge, competence and performance of US Nephrolo-gists, Dialysis Care Providers (Administrators, Physician Assistants (PA’s), Nurse Practitioners (NP’s) Registered Nurses (RN’s), Patient Care Technician’s (PCT’s), and Machine Care Technicians (MCT’s) in the diagnosis, management, and treatment of the dialysis patient. Renal fellows and residents are also encouraged to attend this symposium.
OBJECTIVESParticipants of this activity should be able to:n Identify and apply new strategies to caring for
dialysis patients with hypertension, cardiovascular disease, and/or co-morbidities.
n Discuss new regulatory and fiscal requirements from CMS and NYS.
n Compare and assess available data for homedialysis therapies as described by recent studies.
