The direct and indirect impact of stroke on patients, families and communities has far-reaching implications related to survival, recovery, rehabilitation, and returning to being productive members of society. Although stroke is now the 5th leading cause of death in the US, it remains the leading cause of disability and as such, should be a primary focus for healthcare providers in every sector of the healthcare system. Coordinated efforts have been undertaken (1) to increase stroke awareness in the community; (2) to optimize stroke symptom recognition among emergency medical personnel; (3) to achieve a rapid evaluation by emergency department providers; (4) to initiate a patient-centered recovery care plan; and (5) to implement a focused rehabilitation program to direct patients and families toward a successful secondary stroke prevention lifestyle.

These partnerships have been established through stroke systems of care, and through the collaborative work of multiple disciplines with stroke-focused training. The 8th Annual Brass Stroke Symposium will outline the importance of partnerships in each domain of stroke care that, through the knowledge of best practice, can achieve successful outcomes for patients and their families recovering from stroke.
