JOB TITLE: Senior Community Living Manager IIDIVISION: Community Living Services (Residential)SUPERVISOR: Regional Director of Community Living ServicesFLSA STATUS: Exempt

JOB SUMMARY: Responsible for the day to day operations of a minimum of 2 Community Living Arrangements

MINIMUM QUALIFICATIONS: College degree with specific coursework necessary to meet Developmental Disability Professional title

PREFERRED QUALIFICATIONS: Bachelor's Degree in social services related discipline. Three years of supervisory experience.

SPECIAL LICENSES, REGISTRATION OR CERTIFICATION: GA Driver’s License and vehicle insurance; must have ability to utilize Microsoft Office Suite (Word, Excel, Power Point, Outlook, etc.)

SUPERVISORY RESPONSIBILITY: Supervises Direct Support Professionals and other paraprofessional staff as assigned

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

I. Training/Program Responsibilities

A. Implements activities as coordinated by Consumer Support team which reflect training objectives in each individual’s service plan.

B. Ensure that each resident is engaged in activities of choice, which reinforces ISP goals and are person centered.

C. Promptly reports any emergency/crisis situation to supervisor and complies with internal and external reporting requirements.

D. Coordinates transportation and supervision for residents as dictated by individual program. E. Ensures that all information regarding daily activities, problems and needs of residents are recorded

properly in daily shift note.F. Provides training and assistance in changing appropriate apparel on a daily basis to ensure comfort and well

being for residents.G. Ensures that all staff is properly trained on van safety. Ensures that proper maintenance is performed on the

van when necessary. H. Completes pre-ISP required documentation. Participates in and attends annual ISP meeting for each

resident.I. Coordinate residents’ outings for community involvement and shopping for their personal supplies. J. Is responsible for staffing the group home, including overseeing AOD reporting, relief staff, and leave

scheduling, K. Complete HRST recommendation training for all staff before the end of 30 implementation period of ISP.L. Conducts monthly house meetings at residential settings and submits in-service forms to HR for personnel

recordsM. Provides 1:1 training/support to identified staff with performance concerns (daily shift notes, completion of

task, etc)N. Serves as liaison person between staff and the Consumer Support teamO. Participates in on-call rotation (with other members of the Consumer Support team)P. Works closely with peer mentor program and the dream committeeQ. Identifies person-centered opportunities in the community and ensures that each individual is participating

in community activities.

II. Responsibilities Relating to Health and SafetyA. Utilizing monthly site visit tool, inspects home to ensure that home meets all safety requirements, checking

for clutter and dangerous objects,.B. Corrects deficiencies and submits written reports as required by internal guidelines. C. Ensures evacuation drills, both fire and severe weather; records results of each drill are done per standards

and licensure requirementsD. Follow program plan for meeting all potential emergencies and disasters, such as medical, fire, severe

weather and missing persons. Discusses with Consumer Support team specific role relating to each emergency.

E. Notifies supervisor of all medical emergencies as outlined by policies and procedures, including submission of GERs and CIR if required.

F. Checks to ensure that supplies are clearly labeled.G. Reports any problem with equipment used by residents to proper individual or departmentIII. Housekeeping ResponsibilitiesA. Ensures menus are nutritionally sound and appropriate for all consumers in the homes; ensures menus are

posted weekly in accordance with regulations and previous three months menus are filed in the homeB. Ensure adequate food supply is in home on a weekly basis, in accordance with approved menus. C. Purchases needed offices suppliesD. Orders SMS and SME and ensures funding is available via Prior Authorization.IV. Administrative ResponsibilitiesA. Ensures compliance in the area of responsibility during assigned hours with local, state and federal rules

and regulations applying to Community Living Arrangement and UCP Community Living Services.B. Ensures compliance with the organization policies and procedures governing the use of behavior

management programs for controlling maladaptive or problem behavior.C. Ensures compliance with policies and procedures to ensure that confidentiality requirements are upheld.D. Supports and protects the fundamental human, civil, constitutional and statutory rights of clients and

families as defined by agency.E. Demonstrates competence in handling medical emergencies in accordance with UCP policies and

procedures.F. Maintains accurate and complete records of all resident’s personal spending monies in accordance with

rules and regulations; completes monthly reconciliation and submits report to Regional Director of Community Living Services for approval and final submission

G. Maintains accurate and complete records of house spending (including credit card purchases and petty cash purchases) in accordance with rules and regulations; completes monthly reconciliation and submits reports to Regional Director for Community Living Services for approval and final submission

H. Ensures that all staff is in full compliance with training and personnel requirements by reviewing Relias reports weekly and providing follow up and appropriate coaching to staff who are not in compliance.

I. Completes annual performance reviews of all staff.J. Schedules interviews and hiring of staff to fill budgeted positions.K. Completes status change and all other related HR forms related to processing of new employeesL. Observes all other personnel policies and works closely with supervisor to ensure compliance.M. Abides by code of ethics and serves as a good role model for clients.N. Ensures homes remain in compliance with budgets (staffing, food, household supplies, etc)O. Submits CAPs for 3 ratings to Regional Director by established deadline.

V. Record Keeping ResponsibilitiesA. Maintains records in the home to meet local, state and federal guidelinesB. Runs daily report of shift notes, ISP goal tracking and bowel tracking to ensure they are present and content

is thorough, accurate and meets minimum quality expectations.C. Maintains tracking records in accordance with Individual Service Plan to comply with program

requirements and expectations of supervisor.D. Completes monthly supervisory notes, including monthly contacts with family members.E. Reviews medication records in accordance with policy.F. Reviews records of evacuation drills.G. Completes monthly submission s records of van log data sheets and attendance logs

H. Ensures all annual required forms for each individual in services are submitted to Main chart. Completes additional submissions for chart audits generated by QI Department.

VI. Physical Health Service ResponsibilitiesA. Receives training on supervising the self-administration of medications and follows all policies and

procedures.B. Ensures safe handling and storage of all prescribed and over-the-counter medications. Supervises self-

administration of medications in accordance with established policies of Georgia Department of Human Resources and complete annual SMACCs of all employees.

C. Obtains annual TB test and ensures that results are signed by appropriate medical staff and on file in Center Personnel record.

D. Provides required documents for personnel file as requested, to include Georgia Driver’s License, First Aid/CPR certification, physical, etc.

VI. Other A. Adheres to all agency policies and procedures and Community Living Arrangement rules.B. Takes residents to their doctor appointments when necessary and approved by Regional Director C. Develops and maintain positive and productive relationship with consumers, families, co-workers, and

community members.D. Maintains objectivity in position in order to set appropriate limits while working with consumers.E. Adheres to all agency policies and procedures.F. Other duties as assigned.
