INVITATION FOR NEGOTIATED PROCUREMENT Supply and Delivery of Various Goods for Diversified Livelihood Projects of

DOLE Northern Samar

1. In view of the three (3) failed public biddings, the Department of Labor and

Employment Regional Office No. VIII Bids and Awards Committee invites interested bidders to participate in the negotiation for the Supply and Delivery of Various Goods for Diversified Livelihood Projects of DOLE Northern Samar on July 31, 2019 at 2:00 PM at the DOLE RO VIII Second Floor Conference Room, Trece Martires St,. Tacloban City in accordance with Section 53.1 of the Implementing Rules and Regulations of Republic Act No. 9184, otherwise known as the “Government Procurement Reform Act” with an Approved Budget for the Contract amounting to Two Million Five Hundred Sixty-one Thousand Seven Hundred and Seven Pesos Only (₱ 2,561,707.00). 2. The following eligibility and technical documents as well as the Financial Proposal Form (Annex C) shall be submitted on or before August 1, 2019, @ 5:00 P.M. at the BAC Secretariat, Internal Management Services Division, DOLE Bldg., Trece Martires St., Tacloban City: a. PhilGEPS Registration Certificate which shows current and updated file of

the following Class “A” eligibility documents: i. Mayor’s/Business Permit or its Equivalent Document ii. Tax Clearance iii. Audited Financial Statement

b. Statement of the prospective bidder of all its ongoing government and

private contracts (Annex F), including contracts awarded but not yet started, if any, whether similar or not similar in nature and complexity to the contract to be bid; (shall indicate the following: name of the contract, date of the contract, contract duration, owner’s name and address, kinds of

goods, amount of contract and value of outstanding contracts, date of delivery, end-user’s acceptance or official receipts or sales invoice issued for the contract, if completed, attached to the statements); and

c. Statement identifying the bidder’s Single Largest Completed Contract similar to the contract to be bid (Annex G) within two years from receipt and submission of bids equivalent to at least 25% of the ABC (shall indicate the following: name of the contract, date of the contract, contract duration, owner’s name and address, kinds of goods, amount of completed contracts, adjusted to current prices using PSA’s consumer price index, if necessary, date of delivery, end-user’s acceptance or official receipts or sales invoice issued for the contract, if completed, attached to the statements).

d. Net Financial Contracting Capacity (NFCC) computation which must be at

least equal to the ABC or a Committed Line of Credit (at least 10% of the ABC) from a Universal or Commercial Bank.

NFCC = [(Current assets minus current liabilities) (15)] minus the value of all outstanding or uncompleted

portions of the projects under ongoing contracts, including awarded contracts yet to be started, coinciding

with the contract to be bid.

CLASS “B” DOCUMENT: Valid joint venture agreement (JVA) in case the joint venture is already in existence.

e. Bid Security issued in favor of DOLE shall be either in the following forms and

amount: (i) 2% of the ABC, if cash or cashier’s/manager’s check, bank

draft/guarantee or irrevocable letter of credit issued by a Universal or Commercial Bank;

(ii) 5% of the BAC, if surety bond with certification from Insurance Commission that such insurance company is authorized to issue such security;

(iii) Bid Securing Declaration. Sample form is attached as Annex D. f. Compliance with Schedule of Requirements (Annex A) and Technical

Specifications (Annex B); g. Omnibus Sworn Statement, attached as Annex E; h. Authority of the Signatory

3. Opening of the eligibility, technical documents and financial proposal will be on

August 2, 2019 at 10:00 A.M. 4. For further information, you may call the BAC Secretariat thru the Internal

Management Services Division at 053-832-0697 from 8:00 AM to 5:00PM.

5. The Department of Labor and Employment Regional Office No. VIII reserves the right to accept or reject any offer, to annul the negotiation process, and to reject all offers at any time prior to contract award, without thereby incurring any liability to the affected firms.
