I, the undersigned, certify that the birds comprising the flock of
origin meet the following requirements:
a) the flock of origin has been continuously resident in Great Britain
for a period of at least 90 days prior to the export of the day old
chicks;

b) the flock(s) of origin are subject to routine veterinary supervision
and there has been no evidence of the following diseases during the
6 months immediately prior to export:
Newcastle disease, fowl pox, fowl cholera (Pasteurella multocida),
fowl typhoid (Salmonella gallinarum), infectious bronchitis (IB),
infectious laryngotracheitis (ILT), infectious synovitis, infectious
bursal disease (IBD or Gumboro disease), egg drop syndrome (EDS ’76),
pullorum disease (Salmonella pullorum), typhoid disease, ornithosis,
Marek’s disease, lymphoid leucosis, mycoplasmosis, chronic
respiratory disease (CRD), swollen head syndrome (avian pneumovirus),
chick anaemia agent, avian nephritis, leucocytozoonosis, infectious
coryza, avian spirochaetosis, avian encephalomyelitis, avian
salmonellosis, avian tuberculosis;
c) EITHER *(i)on (date), blood samples were
taken from a representative sample selected at random from the
flock(s) of origin and tested serologically at the Veterinary
Laboratory Agency (VLA) for Salmonella pullorum, S.typhimurium,
S.gallinarum and S.enteritidis with negative results in each case;
OR *(ii) the flocks of origin are subject to regular
microbiological sampling at the hatchery at least once per fortnight for
Salmonella enteritidis, S.typhimurium, S.pullorum and S.gallinarum
according to a Government controlled monitoring scheme and have given
negative results in each case during the past 6 months;
d) the flocks of origin have been inspected and showed no clinical signs
of communicable disease on the day of shipment;
