LOCATIONS All locations see children in ECE (age 3 and above) through 12th grade. Children ages 2 and under enrolled in a Denver County ECE center can be seen at any site listed that is marked with an (*). DENVER HEALTH’S SCHOOL-BASED HEALTH CENTERS Denver Health’s School-based Health Centers (SBHC) are open to all Denver Public Schools students, including those enrolled in Early Child Education Programs, offering specialized pediatric-focused care to treat most health conditions that affect school-aged children. SBHC are convenient care options that limit the amount of time students are out of class and help parents/ guardians with their work schedule. In many cases, students are able to get same-day or next-day appointments. SERVICES Well Child Checks which meet the requirements of school, sports, daycare and camp physicals. This may include routine lab tests and immunizations. Prescriptions and medications Care for chronic conditions such as asthma and depression Care for acute injury and illness Individual, group and family mental health counseling Dental screening, sealants, routine cleanings, fluoride varnish applications, and dental X-rays (at select locations) Parent and child health education Testing and treatment of sexually transmitted infections Reproductive health services including pregnancy testing and birth control Facebook.com/SchoolBasedHealthCenters | [email protected] | DenverHealth.org/DPS PARENTAL CONSENT A parent or guardian must sign a consent form before their child may use any health center service. Once this is done, the child may use the health center at any time during the current school year. Supporting family communication is a principle goal of the health center. Clinic staff encourages patients to discuss their health care with their parents. COST & INSURANCE There is no charge to the patient or their family for services offered in the health center. Patients will not have to pay co-pays, deductibles, or fees to be seen. Insurance may be billed but without cost to the family. Community Health Care Advisors are available to assist students and families with questions, eligibility, and enrollment into the following programs: Health First Colorado (Medicaid), Child Health Plan Plus (CHP+), Connect for Health Colorado (C4), and Discounted Health Care Programs (DFAP & CICP). Los Centros de Salud Escolar de Denver Health (SBHC) ofrecen atención pediátrica enfocada y especializada para tratar la mayoría de los trastornos médicos que afectan a los niños en edad escolar. Los servicios están disponibles durante todo el año escolar para los estudiantes de las Escuelas Públicas de Denver (DPS) y niños que asisten a un centro de Educación de la Primera Infancia (ECE) afiliado a DPS en el Condado de Denver. Los SBHC ofrecen atención conveniente que reduce la cantidad de tiempo que los estudiantes tienen que estar fuera de clases y que los padres o tutores tienen que estar fuera del trabajo. En muchos casos, los estudiantes pueden conseguir citas para el mismo día o para el día siguiente.

CONSENTIMIENTO DE LOS PADRES El padre, madre o tutor debe firmar un formulario de consentimiento cada año escolar antes de que su hijo(a) pueda usar un servicio del centro de salud. Con el consentimiento firmado, su hijo(a) puede usar el centro de salud en cualquier momento. Apoyar la comunicación familiar es un objetivo principal del centro de salud. El personal del centro anima a los pacientes a hablar sobre su salud con sus padres.

COSTO Y SEGURO No hay ningún costo para el paciente o su familia por los servicios que se ofrecen en el centro de salud. Los pacientes no tendrán que pagar copagos, deducibles ni cuotas para que se les atienda. Es posible que se envíe una factura al seguro medico, pero sin costo para la familia. Hay asesores de salud de la comunidad disponibles para ayudar a su familia y al estudiante para volver a determinar elegibilidad o para inscripción en programas de descuento privados o estatales.
