SCENARIO:Johnny, 27 years old, unemployed, was brought by a relative in the ZCMC for consultation. Relative narrates that client woke up early in the morning of February 08, 2016, seemingly not in his usual self – client, looked neat and well-groomed instead of his usual messy appearance, and began speaking words of God. Three days prior to consultation , client broke up with his 3-year girlfriend and has been depressed since then. Client was involved in abuse of drugs and tobacco smoking as shown in his health history. Accordingly, client was not religiously active and curses a lot which prompted the relative to seek for medical attention.

Received standing along the hallway, wearing plain white shirt, orange jersey shorts and a crucifix necklace around his neck with a clean-cut hair, confidently giving gestures , such as widely opening his arms ,to catch people’s attention and loudly preaching about the resurrection of Jesus Christ.--------------------------------houseD: History of drug abuse, a chain smoker and a college dropout. Never been a part of any religious organizations. Never seen participating in any religious activities. Speaks in a loud tone saying “And now the Lord God has sent me and his spirit….” as if he has been preaching for a long time--------------------------------------------house

A: Recognized delusional system but did not attempt to argue about the facts. Respected the privacy and space needs by avoiding touching him unnecessarily. Interacted on the basis of things in the environment. Diverted from his delusion by engaging in reality-based activities such as playing simple board games and cards and talking about current events. --------------houseR: Appeared calm and relaxed during communication. Did not exhibit aggression and fear when touched. Participated in the interaction with the environment. Responsive to reality-based activities and played cards. -house
