Dear Doctor I write this letter in reference to Mrs Julie Baker, 46 Medway Rd, Hollinwood. Mrs Baker’s family are genuinely concerned for her general physical health and psychological/ mental well being. Her family have identified very serious concerns of seeking to establish are the medications she is prescribed appropriate for her? and is it at all possible a full review of her medications can be undertaken. It is with this in mind I write this letter seeking your valued professional support. Her family have concerns of the pain she is in, constantly being told by her she is always in pain. Recognising she is on diverse medications are they working? Or has she become immune to them for which they are now not working as they should be? Are the doses of medications appropriate if she is still saying she is in pain? Her family have raised serious allegations in acknowledgement (although there is no proof) she is taking cannabis for the pain, given she is on prescribed medication they believe she is doing herself real harm. Her behaviour is becoming bizarre and irrationals, as well as hostile and totally out of character. To explain: 1. She visited her younger son’s place of work (building society) and caused a scene which resulted in her son becoming embarrassed. She had arrived with a bank statement of her former husband who had not paid alimony in which she asked her son to commit fraud by way of opening a bank account in her son’s father’s name and moving money around. Naturally this is illegal and certainly did not happen. 2. Concerns raised for her needing a blood test to establish if there is anything other than prescribed medications within her system. 3. A full review of all the medications she is taking is needed. Her family believe she is on prescribed medications they are concerned she become dependent on taking tablets she believes working. Rather than seek your medical opinion it would appear she maybe exacerbating the situation by taking other none prescribed as previously detailed. Surely this is not making a wise decision around her own health. Her family are aware that legislation identifies people can make unwise decisions, but taking illegal drugs coupled with prescribed drugs would be causing her more harm than good. Can you help? She is often lethargic, talking as if she is spaced/zoned out. She does not sleep and is up throughout the night, are you able to help and test for amphetamines?

Mrs Baker is a 52 year old white British female who has always put her family first. Having been a fun

loving, easy going, caring and warm gentle person with a pleasant personality. She has been through

three marriages and raised three children (who are all adults and have lives of their own). No matter

what situation she encountered she had previously maintained a roof over her own head and that of

her children, now she is in debt up to her eyes, turning to drugs as a coping mechanism which is

potentially killing her.

Her family are very worried that if she continues on this downward spiral path she will not be with

them for much longer, such was the recent case when she was taken to hospital 22.10.12. Can you

help, please?

Mrs Baker is aware this letter is being sent with reference to seeking a full review of the prescribed

medications she is taking. The remainder of information is being provided in consultation with her

children, sister who lives in Spain and other people who genuinely care for her, as her behaviour is

so very much out of character.

Her family understand she has been referred to a psychiatrist and that she may not be telling him

everything of what is going on, being selective of information she has provided . Which raises her

family’s very serious concerns for her family.
