InstructionsINSTRUCTIONS FOR MAINTENANCE PIGGING REPORT1DIVISIONEnter your division, i.e., Gulf Coast, Mid-Continent; Rocky Mountain; Southern, Southwest, of Western2DISTRICTEnter the Operations District where the pig is launched3JURISDICTIONCheck to indicate the jurisdiction of the pipeline segment. Check with Regulatory Compliance Group for correctdesignation. DOT Jurisdictional lines should be on a separate page from Non-Jurisdictional lines.4PIGGING INFORMATIONSegment NameEnter the name of the segment, preferably as listed in the pigging database.Pig TypeEnter the type of pig using the drawings at the bottom of the form as a guide.Select the pig that most nearly looks like the actual pig used. Enter the Group numberand letter designation, for example, 1A would be a brush pig similar to the oneshown above the letter A. If more than one pig is run, say a brush pig followed by adisk pig it might be entered as "1A+2C", or entered as a separate run on another line.SizeSize (diameter) of the pig.Date InDate the pig was launched.If the pig is launched by one person and received by another the launcher's and receiver'sdata should be entered on the same report. It is the responsibility of the DistrictManager/Supervisor to assign the task of completing the form to theresponsible personnel.Launched ByEnter the initials of the individual who launched the pig.Date OutDate the pig was received or removed from the trap.Received ByEnter the initials of the individual who received the pig.Debris, Paraffin and Water RemovedEnter the approximate amount of material removed from the pipe. This may be water, sludge, paraffin or other material. When practical express the amount as a volume, i.e., one quart, one gallon, five gallons, etc.NOTE:At the end of each month, send Original to the Division Records Specialist and copies to your Supervisor and Corrosion Tech.
