CANDIDATES’ CURRICULUM VITAE1. This booklet contains the biographies of all the candidates together with their
photographs.2. Should you not know which candidate to vote for, the information contained in
this booklet will give you details on every candidate in order for you to make an informed choice.
3. Voting Instructions are in the Ballot Paper sheet.4. Please note that the biographies have been reproduced from CV Forms
submitted by candidates.
VOTING RULES, PROCEDURES AND CURRICULA VITAE1. Only principal members of SAMWUMED that are active and in good standing
as at 15 July 2019 are eligible to participate in the process of voting for the members of the Board of Trustees. Suspended members, as well as spouses, and beneficiaries are not allowed to vote.
2. The candidates’ summarised Curricula Vitae (biographies) and photos can be viewed in the attached CV Booklet.
3. A principal member is entitled to vote for no more than six (6) candidates, one of whom must be a pensioner, from the list appearing on the attached ballot paper. Should you wish to vote for fewer candidates inclusive of a pensioner, your vote will remain valid.
4. Voting for more than six (6) candidates will result in your vote being spoilt.5. The six candidates receiving the most votes in a descending order shall be
appointed as trustees one of whom must be a pensioner.6. The election process is independently managed by The Elexions Agency (Pty)
Ltd and SNG-Grant Thornton, the Scheme’s external auditors.

METHOD 1:POSTAL BALLOT – AVAILABLE TO ALL MEMBERS1. Enclosed in this pack is a Ballot Paper with Instructions, a CV Booklet with
candidates’ biographies and a pre-paid self-addressed envelope.2. Vote for up to six (6) candidates of your choice by placing a mark in the box next to
your choice. One of them must be a pensioner.3. Fill in your SAMWUMED Membership Number and your ID Number in the tear-off
counterfoil slip section on the ballot paper. DO NOT TEAR OFF THE STUB AS IT IS FOR THE ELECTORAL OFFICER’S USE ONLY.
4. Fold your ballot paper and place it in the pre-paid self-addressed envelope and post it at the nearest Post Office.
5. You can also return your ballot paper via the following methods:

6. Do not write any messages on the ballot paper other than the mark next to the candidate(s) of your choice.
7. The following will render your ballot paper SPOILT:
• Voting for more than six (6) candidates.• Failing to duly complete the counterfoil slip.• Writing your personal details on the ballot paper other than on the counterfoil slip• Writing incorrect personal details on the counterfoil slip.• A ballot paper returned without the counterfoil (tear-off) slip.• Using someone else’s ballot paper.
METHOD 2:ONLINE VOTING – AVAILABLE TO MEMBERS THAT HAVE EMAIL ADDRESSES (REGISTERED WITH THE SCHEME) AND CELL NUMBERS
1. Members with email addresses will receive an email with a link to the voting platform.a. Click on the link in the email received.b. Enter your
i. ID Numberii. SAMWUMED Membership Number and iii. Unique Ballot Paper number to access the voting platform.
c. Read the voting instructions.d. Study their biographies in order to know who to vote for.
e. Vote for up to six (6) candidates of your choice by clicking on the candidate(s) picture provided, ensuring that one of them is a pensioner.
f. Post your vote electronically by clicking submit at the bottom of the screen. g. You will receive confirmation on the screen that your vote was successful.
2. Members with cellphones will be able to dial *130*676# and;a. Enter your:
i. ID number;ii. SAMWUMED Membership number;iii. Unique Ballot Paper Number.
b. Choose Option 1 if you have received the Unique Ballot Paper Number and;i. View the voting instructions and the biographies of the candidates (CV
Booklet) on the SAMWUMED website;ii. Vote for up to six (6) candidates of your choice including a pensioner.
c. Choose Option 2 if you have not received the ballot paper yet, and;i. Enter your ID number and SAMWUMED membership numberii. A notification will be sent to the Returning Officer to contact you.
d. In the same way your ID and Membership numbers are UNIQUE to you, so is the Unique Ballot Paper Number. IT CANNOT BE SHARED WITH OTHER MEMBERS.
METHOD 3:ON-SITE VOTING – ONLY ON TUESDAY 10 SEPTEMBER 2019Members will be accorded the opportunity to vote at the municipalities listed in the attached Voting Instructions and Ballot Paper from 09h00 to 16h30 on Tuesday 10 September 2019 ONLY. At the voting station, the following process will be followed in order for you to vote:1. Produce your document and membership card.2. The above details will checked against the voters’ roll and the member will be provided
with a ballot paper that will have the unique ballot paper number written on it.3. Be shown to a voting both to make your choice in secrecy. Please note that you
have to complete the counterfoil slip in full in addition to making your choice.4. Fold your ballot paper and deposit in the marked “SAMWUMED 2019 Trustee
Elections” ballot box and exit the voting area.
METHOD 4:AGM VOTING – FOR MEMBERS THAT HAVE NOT TO VOTE BY 12 SEPTEMBER 2019Members that were not able to vote before 12 September 2019 will be able to vote at the Annual General Meeting (AGM) scheduled for 27 September 2019 from 08h30 to 11h00. Please bring your ID smartcard/book and your SAMWUMED Membership card for identification purposes.
VOTE COUNTING AND RESULTS DECLARATIONBallot papers will be counted by The Elexions Agency and independently audited by SNG-Thorton auditors. All the valid votes by means of all methods mentioned above will be consolidated at the AGM to produce an outcome.
THE ELECTION REPORTThe Election Report and audit report will be submitted to the Curator who will consider them. If the Curator and the Registrar are satisfied that the election was in keeping with the registered rules:1. The Curator will notify successful candidates in writing of the election outcome.2. The election results will be announced at the Annual General Meeting and posted on
the SAMWUMED website. In terms of Rule 24.4.11, in appropriate circumstances, The Curator may announce the results of the elections at a later stage being no more than 30 days from the date of the AGM.

REASONS FOR WANTING TO BECOME A TRUSTEE:The reason for wanting to become a trustee of SAMWUMED board is, I have been a loyal member of SAMWUMED for 11 years without termination. To represent members of the scheme in Limpopo as there is a lot of members and don’t have any representatives person To monitor solvency and improve benefits of the medical scheme. Investigate complaints and settle disputes in relation to the medical aid scheme in Limpopo province.
