Clinical Practice, Interests, and AccomplishmentsINTERESTS Primary immunodeficiencies - diagnosis, managment and understanding the pathophysiology ofprimary immunodeficiencies, study of the underlying genetic defects, effect of particular immunologicaldefects on components and function of the immune system, laboratory and clnical correlation betweenimmunological tests and specific primary immunodeficiency phenotypes

Transplant immunology - understanding the effects of immunosupresion on immune competence andimmunological reconstitution, using laboratory -based immunological testingt to develop personalizedimmunosupressive regimens, reducing infectious co-morbidity due to immunosuprresion

Immune monitoring - serial and systematic evaluation of immune reconstitution and functional recoveryin relevant clinical contexts, e.g. use of biological immuno-modulatory drugs, immunotherapy orchemotherapy for the treatment of autoimmunity, cancer, transplatation

ACCOMPLISHMENTS

Primary immunodeficiency clinical laboratory (Cellular and Molecular Immunology) - The primaryimmunodeficiencyu clinical diagnostic laboratory was started in July 2005 and has since thensupported the practice of the Adult Immunodeficiency Cllinic and Pedicatric Immunodeficiency Clinicwithin the Division of Allergic Diseases and Dept of Pediatrics respectively. In the past, certainimmunodeficiency-related tests were done within the Hematopathology clinical laboratory; however, the setting of a clinical laboratory whose emphasis is on primary immunodeficiencies has allowedexpansion of our diagnostic capabilities in this area both for our internal and extramural practice. Since2005, approximately 30 new or modified clinical flow cytometry, cellular and molecular tests related toprimary immunodeficiencies or the assesment of immune competence have been developed andimplemented or will be implemented. The laboratory is constantly expanding its test "menu" forprimary immunodeficiencies and evaluation of immune competence.

The clinical laboratory, besides supporting the immunodeficiency practices, also provides a keyresource for the clinical and research assessmen tof immune reconstitution and immune competence

in several other clinical contexts, including autoimmune diseases, hematological Malignancies,solid-organ transplantation and bone marrow (stem cell) transplantation.

Mulidisciplinary clinical collaborations - Collaboration with clinicians in Pediatrics, Hematology,Gastroenterology, Rheumatology, Allergic Diseases, Transplantation, Cardiology, Nephrology,Infectious Diseases to support clinical use and interpretatin of immunological tests, implementation ofclinical research protocols to test and improve clinical practice and patient care related to reducinginfectious co-morbidities and evidence-based use of immunological intervention or therapy

Research InterestsPrimary immunodeficiencies - Identifying and developing state-of-the-art techniques to daignoseprimary immunodeficiencies, study of the pathophysiology of common variable immunodeficiency(CVID), identification of genotype-phenotype corerlations with TACI gene mutatins in CVID, laboratorycorrelation of specific immunological parameters with clinical presentation in CVID and other humoralimmunodeficiencies, identifying new methods to improve diagnostic criteria for impaired functionalantibody responses in humoral immunodeficiencies. Transplant immunology - Evaluation of immune reconstitution and assessmen tof immune competiencein hematopoietic stem cell transplantation, the effect of various conditioning regiments on immunereconstitution, the use of immune reconstitution and immune competence as the criteria for directingvaccination strategies post-transplant, developing evidence-based strategies based on immunologicalfunction to determine immunosuppression in solid-organ transplant recepients.

Immunobiology of primary systemic (light chain) amyloidosis - studing immunoglobulin light chainvariable gene usage in light chain amyloidosis, the effect of light chain gene usage on survivalpost-hematopoietic stem cell transplant, light chain gene usage and prediction of cardiac dysfunction inprimary amyloidosis.

Identification of biomarkers for sepsis and systemic inflammatory response syndrome (SIRS) - use ofquantitative CD64 expression on neutrophils and HLA DR and CD163 on monocytes to differentiatiebetween sepsis and SIRS in the intensifve care unit setting, evaluation of immunosuppressivemonocytes in neoplastic diseases.
