EXPERIENCE

Internship

As an intern at Riyadh Central Hospital, King Abdulaziz University Hospital and Riyadh Military Hospital, I worked under supervision examining, checking and updating patient's records and investigations. My training involved attending clinical seminars and outpatient teaching group clinics. During this period, I saw a wide range of medical and surgical cases and their management. I performed minor surgeries as appendectomy, hernioraphy, minor anal surgery. I also performed normal deliveries.

General Surgery

Following my internship, I worked for one and a half years as a Resident in General Surgery at King Khalid University Hospital and Riyadh Central Hospital, as well as 6 months in each of Neurosurgery and Accident and Emergency.

I was responsible for the care of inpatient and accident cases and was encouraged to take clinical decisions. I have participated when on-call for emergencies in the diagnosis and management of acute abdominal conditions, urological problems, multiple injuries and head injuries. In Accident and Emergency Department, I was managing surgical, medical and orthopaedic emergencies.

The operations that I have performed on my own included appendectomy, hernial repairs, emergency and elective laparotomy, cholecystectomy and anal surgery, as well as reduction of fractures and drainage of haematoma following head injuries. This period also included treatment and management of A/E patients for surgical and medical conditions as well as intensive care unit rotation. I have also assisted in many major operations.

Orthopaedic Surgery

During my Resident training in Trauma and Orthopaedic Surgery, I have participated in the diagnosis and treatment of fractures, dislocations, hand injuries and elective orthopaedic cases. I cared for the inpatient and attended teaching ward rounds and assisted in major operations such as congenital dislocated hips, hip replacement, internal fixation of complex multiple fractures and spinal surgery.

As a Registrar & Senior Registrar, I have performed many operations including fixation of spinal fractures and dislocation and disc surgery and spinal fusion, upper and lower limb trauma, tendon and nerve repairs, bone grafting procedures, meniscectomy,

cruciate surgery and arthroscopy (diagnostic and therapeutic). I have managed and treated conservatively and operatively cases of CDH and CTEV, bone tumours and arthritis cases. I was also responsible for outpatients as well as inpatients; diagnosing and managing problems and following up outcome of treatment.

I was also actively involved in Postgraduate Residency Training Program in Orthopaedics and teaching Orthopaedics for undergraduate medical students. I supervised junior Orthopaedic surgery trainees and guided them during their training.

During my stay in the U.K., I was managing cases of arthritis surgery both as outpatients and inpatients. I worked with Mr. Souter, Mr. Howie and Mr. Abernathy in upper and lower limb arthritis and replacement surgery, assisting at first then performing primary total hip replacement, total knee replacement, TER and TSR and later on performing revisions of THR and TKR. I have also spent 7 months with Mr. Hooper in the hand unit concentrating on elective and arthritis hand conditions, but also treating other hand pathologies. I have participated in three studies since then, one of which is published. At the end of my training period, I obtained a certificate of fellowship in joint replacement from Edinburgh University.

I am currently in charge of the reconstructive surgery at King Khalid University Hospital and a member of Committee on Bone Banking in Saudi Arabia. I am concentrating on arthritis and joint replacement surgery of hip, knee, shoulder, elbow and hand with future plans to extend the service to ankle replacement surgery. The majority of my practice now is total joint replacement, the rest is trauma and sport injuries. I run one operative theatre day as well as two clinics per week. I was in charge of the Orthopaedic Residency training in King Khalid University Hospital until 2002 in addition to organizing the interdepartmental teaching activities and undergraduate teaching.

Achievements as Head of Orthopaedic Department As a head of the Orthopaedic Department, I have continued the work of my predecessors in expanding on the services the department can give and giving it a special identity. I have expanded the services given by the spinal and arthroplasty division as well as the other existing subspecialty.

I have initiated a 2-yearly International Orthopaedic Symposium under the umbrella of our department as well as mini-symposia in sub-specialty.

I have generated the changes done in the Orthopaedic curriculum given to the undergraduate students through a 6 weeks course. I have formed a committee to study the old curriculum and suggest changes in the theoretical as well as the clinical parts of the course. I have emphasized the need to introduce problem solving to the curriculum and change the form of the tutorials. I have also assigned one of the teaching staff in-charge of

the students’ affairs in the department. The new curriculum was implemented starting from Academic year 2004-2005. A further review of the curriculum was done to reduce the course to 5 weeks, in accordance with the general new plans of the College.

To encourage the cooperation between different disciplines in the care of Orthopaedic patients, I have started an unpreceeded meeting between the Head of Orthopaedic Department and representatives from Orthopaedic wards, clinic, as well as operating room, physiotherapy and plaster technicians. These meeting were aimed at discussing any problems facing the care of orthopaedic and trauma patients, facilitating the smoothness of work and updating and improving the instrumentational needs for these disciplines. These meeting were scheduled 3 times a year and proved very useful. Some of the things which resulted from these meetings are; the construction of a physiotherapy room in the female orthopaedic ward, assigning a head nurse to the orthopaedic outpatient clinic, putting the plaster technician under the administration of the Orthopaedic Department and assigning a Head to Plaster Technicians. We have also worked through these meeting to increase the manpower in these disciplines to improve service.

I have also initiated the Bone Bank facilities in the university hospital and had, in achieving that, great help and cooperation from the Blood Bank director under whom this facility was eventually put. I have also reactivated the Quality Management Committee in the department and created a Research Committee.

As Head of the Department, I have also initiated a two-year Fellowship programme, one in Paediatric Orthopaedics and the other one in Joint Reconstructive Surgery, under the umbrella of King Saud University. The Paediatric one started on January 2004 and the Joint Reconstructive Surgery on the same year. Under my Chairmanship, the specialties in the department were transformed from units to divisions and the elected heads were assigned. These divisions are Paediatric Division, Reconstruction, Sports & Oncology Division and Foot and Ankle, Spine & Arthroplasty Division. I have also put the guidelines for election of the heads of Divisions including their rank, duration of chairmanship and numbers of times to be re-elected. These were agreed upon through the Board of the Department.

I have also suggested and worked on establishing the pre-admission – Anaesthesia Clinic to deal with cases in need of special care preoperatively. This was greatly supported by the administration as well as the surgical and Anaesthesia department. This clinic started receiving patients in the year 2004.

I continued the work of my predecessors in establishing the Saudi Orthopaedic Association which was finally approved in May 2005 after applying twice and negotiating with the higher authority and is now located at College of Medicine, King Saud University. I was elected as president of the Saudi Orthopaedic Association during the first general assembly meeting held on 19/12/1426 (19 January 2006) for a 3 year period.

During the early time of the year 2005, I negotiated and initiated the establishment of trauma fellowship program for training Saudi doctors in Germany. This is pending the official agreement and approval of both authorities in both countries. The 1st fellow started the program on January 2007.

To improve the filing system in the Department of Orthopaedics, I have established a filing system for the communications between our department and administrative, clinical and other pertinent departments in the College of Medicine both in Arabic and English communication.

I have suggested to the College Board few suggestions related to academic affairs as follows:

Suggestion to reactivating the “Administration committee of the University Hospitals”. This was approved by College administration.

Giving more rights to the Vice-Chairman of the Department at College Board. This was rejected.

Teaching medical students in Arabic language. This is under discussion.

I have established a bank account for the Department of Orthopaedics to receive donations used in supporting Academic and Continuous Medical Education activities.

ACHIEVMENTS AS PRESIDENT OF SAUDI ORTHOPAEDIC ASSOCIATION(SOA)2006-2009&2009-2012.

After being elected as president , I worked with the eight other members of the administrative board , to achieve the goals I have set. I wanted this association to be a true representative of all orthopaedic surgeons in the country, most of whom are expatriates and to have a major rule in continuous medical education as well as giving our members some benefits.We have also set a committee to set the internal rules of the association and opened a bank account for the association.

Having the members of the board from different area of the country as well as representing more than one health provider did help a lot to agree on giving the members 30-50%discount in any scientific activities. We have also started negotiating with different orthopaedic clubs to join us and this has materialized at the end on the agreement to have ALL orthopaedic clubs in the Kingdom under the SOA, as well as the branches of international associations. I have also hired secretaries and set up the administrative work of the association. I started a long negotiation with orthopaedic and pharmaceutical companies to support the cause of the SOA and this has led to a very active CME program of workshops, seminars, club meetings and sponsoring forty of our members for courses outside the country. It was only after a year that we have succeeded in organizing our FIRST annual international meeting in Riyadh 2008 ,followed by the second in Jeddah 2009 , the third in Alkhobar 2010,and Abha 2011,planning our fifth meeting in Madina 2012. We have also carried TWO review courses for residents with the help &cooperation of King Faisal Univ. in Alkhobar

It was one of my main goals to establish a cooperative relations with other regional and international associations, and so I started mailing the presidents of the Egyptian, Pan Arab, GCC orthopaedic associations as well as AAOS,EFFORT,AO &SICOT. This has led to a special discounted price for our members for the OKO site and the SOA becoming an international sister association to the EFFORT and both EFORT&SICOT agreement to help provide fellowships for our members.

I have also started and established, with the help of some members& and the well of the president of the British trauma society, an agreement with the injury journal by witch it became our official trauma journal and we have distributed the journal free to our members as part of membership benefits.

ON the national front I started active dialogue with Saudi Commission for health specialties and with the help of members of the board, the commission has cooperated with us in strengthening the rule of the association as the prime supervisor of the CME activities of the orthopaedic clubs as well as CME activities submitted by other health providers and as an advisor in recognition of Orthopaedic training hospitals& postgraduate scientific degrees .

On the 3rd year I was elected as vice president of the GCC Orthopaedic Association and member of the board of Pan Arab Orhopaedic Association.

During my second term I have worked to accept the SOA as associate member of EFFORT

And have established the first trainee meeting of the SICOT organization along with our annual meeting in Abha 2011.

I have also signed the first cooperation agreement with the Morrocan Orthopaedic Association and in the way of signing similar agreements with other Arab and regional orthoapaedic associations.

We have also carried out joint scientific meetings with other hospitals like Military hospital in Dahran, SGH group and Kingdom hospital ,all in 2010.
