Garret Cook has completed a Thirty-Six (36) hours precepted Acute Care Hospital clinicalrotation for CSHS Nurse Refresher program.

Garret performed her clinical rotation period in the Emergency Department. He earned highpraise for his knowledge base and his hard work from the Emergency Department Charge Nurseas well as the Nurses she was teamed with.

He competently performed all nursing cares required for his assigned patients for Rapidassessment, Focused reassessment, EKG and specimen collection. He performed blood glucosechecks and competently inserted a urinary catheter. He demonstrated competence in monitoringpatients with hemodynamic instability. He demonstrated competency with Wound Assessment,measuring and treatrnent of wounds. He also demonstrated safe medication practices with theadministration of medications PO, NG and IV along with appropriate follow up documentation.He documented care utilizing the MediTech Electronic Medical Record System. Garret alsodemonstrated giving report with accuracy and thoroughness.

In addition, Garret demonstrates traits of an active learner and pursues opportunities forleaming. He is an excellent team player and is proactive in assisting others. He has been a

pleasure to work with and will be an asset for any facility that hires him.
