Voluntary Benefits
• Lower Group Rates – Pricing for LAPCS as one, large organization versus pricing each school as an independent entity

Advantage of Benefits
• Guaranteed-issue coverage for members and dependents
• Benefits for accidents that occur both “on” and “off-the-job”
• Premiums don’t increase as members age• Coverage for open and closed dislocations and
fractures at the same level. Competitors reduce open fracture coverage by 50%.

Allstate Universal Life
• Accelerated Death Benefit for Long Term Care-Allows the death benefit of the Group UL certificate to be used to provide monthly benefits for qualified long term care services(confined or non-confined) after the insured has satisfied the 90 day elimination period. Issue ages 18 to 70. Premium per $1,000 varies by issue age and class. Not available for child certificates.
• Accelerated Death Benefit for Terminal Illness- Advances a portion(up to 75%) of the insured’s death benefit if a terminal illness occurs. Waives future monthly charges when acceleration is elected. Issue ages 0 to 75; terminates at age 93; maximum advanced-$100,000.
• Continuation of Coverage During Strike or Layoff Rider- Covers a member when he/she is affected by a strike or layoff as defined in the rider. This rider waives the monthly deductions for the certificate for up to six months. Issue ages 18-59, terminates at 60. No additional cost to issue. Must be indicated as elected option on enrollment form.

Here’s How to Enroll
• Open Enrollment Posters: Look for posters to be posted in your work area that include dates of Open Enrollment, call center phone number and hours, as well as featured benefits.
• Benefits Guide: A Benefits Guide explaining all your insured and non-insured benefits will be available electronically via www.LAPCSbenefits.com.
• Open Enrollment : Begins November 2, 2015. During this enrollment period, it will be your responsibility to contact an Enrollment Specialist to enroll in your selected benefits.
• Welcome Call: School administrators will receive a call from an Enrollment Specialist to confirm participation, collect employee information and discuss available benefits.
