Creekside
Creekside RTF (est 2009) is a 9-bed Residential Treatment Facility (and 3-bed Crisis Program) serving individuals who have a primary diagnosis of a major mental illness and who are stepping down out of higher levels of care – including the State Hospital. Creekside is a beautiful, two-story, non-locked (class 3), co-ed facility licensed by
AMH.
This program is located in the world famous Columbia River Gorge, which is situated only two blocks from a major shopping center. However, because of the program’s location on the bank of the gorgeous Mill Creek, it has a very quiet and almost rural feel.
Both shared (double) and individual (single) bedrooms are available, and residents have access to multiple shared bathrooms. One bedroom and bathroom is fully ADA-accessible.
The program is staffed 24 hours per day, 7 days a week by a caring team who provides professional residential services and supervision in a comfortable home-like atmosphere.
Staffing is flexible based on program needs, but typically runs at 2-2-1 staffing schedule to provide appropriatesupervision and supportwith the goal of promoting increased self-sufficiency, well-being, and health.
Services include but arenot limited to:
Nursing and medicalservices coordination;
Assistance withActivities of DailyLiving;
Psychiatric treatment,
Individual and grouptherapy,
Medicationmanagement, and
Supported Employment.
