1 Purpose The purpose of this procedure is to provide a process to protect people against risks to health and safety associated with access and egress, work within and in the vicinity of a confined space.

This procedure is to be used in conjunction with the Confined Space and Permit to Work documentation located in the CPM Health and Safety Portal.

2 Scope This requirements of this document are aligned with Fatality Risk Management Standard 8 – Confined Space Entry and CPM Golden Rule # 5 “I will work with a permit”.

This document applies to all CITIC Pacific Mining Pty Ltd (CPM) employees, contractors and subcontractors who are required to work within confined spaces, perform associated tasks with confined spaces, including trained and authorised sentries and first response rescue personnel.

3 Definitions and Abbreviations Term Meaning

Atmospheric Monitoring

The continuous measurement of oxygen concentration and airborne contaminant concentration levels over an uninterrupted period of time.

Atmospheric Testing The immediate or short term testing of oxygen concentration and airborne contaminant concentrations.

Authorised Gas Tester (AGT)

A competent and authorised person who conducts atmospheric testing and monitoring of the confined space prior to and during the entry.

Authorised Person A person adequately trained and experienced in the procedures and precautions necessary for the elimination of any risk associated with a confined space, and has a satisfactory knowledge of the fire, explosion, toxicity, and other hazards associated with work in confined spaces.

Confined Space (as per Part 4, Div 1, Section 4.2; Mines Safety and Inspection Regulations 1995) References AS 2865:2009.

An enclosed or partially enclosed space that is not intended or designed primarily for human occupancy, within which there is a risk of one or more of the following: (a) an oxygen concentration outside the safe oxygen range; (b) a concentration of airborne contaminant that may cause impairment,

loss of consciousness or asphyxiation; (c) a concentration of flammable airborne contaminant that may cause

injury from fire or explosion; (d) engulfment in a stored free-flowing solid or a rising level of liquid that

may cause suffocation or drowning.

Contaminant Any dust, fume, mist, vapour, gas or other substance in liquid or solid form, the presence of which may be harmful to the health and safety of personnel.

Confined Space Entry Certificate

A written certificate outlining the inherent/introduced hazards and minimum control measures associated with confined space entry. The certificate is used to authorise entry into and control access to a confined space.

Hot Work Any welding method, thermal or oxygen, or heating and any other operations that may generate heat or produce an ignition source.

JHEA Job Hazard Environment Analysis - In relation to confined space entry and Confined Space Entry Certificate, the JHEA is a low level task based hazard identification tool used by those individual undertaking work within a confined space. Identified hazards are assessed and appropriate risk control measures are put in place before the activity commences.

Lower Explosive Limit (LEL)

In relation to a flammable contaminant, the concentration of the contaminant in air below which the propagation of a flame does not occur on contact with an ignition source.

Oxygen Deficiency The atmosphere within any space is considered as deficient where the oxygen level is less than or equal to 19.5 % oxygen by volume.

Oxygen Enriched The atmosphere within any space is considered as enriched where the oxygen level is equal to or greater than 23.5 % oxygen by volume.

Permit Holder With respect to Confined Space Entry, a person adequately trained and deemed competent to accept a Permit to Work from the authorised person with a view to ensure all control measures stated on the Permit to Work and the Confined Space Entry Certificate are implemented and disseminated to the confined space entrants.

Practicable Technically feasible, economically justifiable and contributing to the reduction of risk.

Purging For the purposes of this procedure, purging will be considered as the process of introducing inert gas or atmospheric air in sufficient quantity to clear contaminants in order to create a respirable atmosphere. Purging agents shall not be pure oxygen or gas mixtures with oxygen concentration greater than 21%.

Shall / Must A mandatory requirement. Standby Person (Sentry)

A competent person assigned to remain on the outside of, and in close proximity to, the confined space and capable of being in continuous communication with and, if practical, observing those inside. In addition, where necessary, the competent person may operate and monitor equipment for the safety of personnel in the confined space and initiate emergency response.

SWI Safe Work Instruction - a task specific instruction that provides guidance on how to perform all aspects of a task in a safe manner.

Upper explosive limit (UEL)

Highest concentration of flammable vapour mixed with air that will burn with a flame.

Venting Natural, forced or mechanical methods to maintain a safe atmosphere VO Ventilation Officer. Competent through qualifications and is appointed by the

Registered Manager to ensure controls are sufficient to minimise exposure to atmospheric contaminants.

4 Responsibilities Role Responsibilities

Management The Registered Manager and Business Unit General Managers or Directors are ultimately responsible for ensuring overall implementation of this procedure. This includes ensuring that area managers understand and support this procedure and that adequate resources are provided to implement this procedure.

Health &Safety Superintendents

Oversee and monitor implementation of the confined space entry procedure and assist in establishing methods to improve the efficiency and quality of compliance with the procedure.

Responsible Area Manager

Provide a safe system of work for persons working in their area.

Area Superintendent and Supervisor

Accountable for minimising the risk to persons required to work within confined spaces. Approve the First Response Rescue Plan and ascertain the following have been completed:

• Task Description, • Non ERT equipment requirements, • Initial Emergency Response; and • photo/sketch of the space (showing entry/exit location)

Permit Issuer Authorised in writing by the Area Manager, to authorise and issue PTW and Confined Space Entry Certificate. This authorisation may extend to either CPM or contractor supervisors nominated for specific areas to act in the capacity as a Permit Issuer where the location is remote to the permit office and PTW are required.

Permit Holders Appointed and authorised in writing by the Area Manager. Responsible for ensuring the Confined Space Entry and PTW procedure are being adhered to. They are also responsible for ensuring that the work being conducted is within the scope of work and all agreed hazard controls are implemented as outlined on the permit and certificates.

Standby Person (Sentry)

A Standby Person (Sentry) shall be present at the work site for the entirety of the Confined Space Entry and activate the Rescue Plan where required.

Confined Space Entrants

All individuals entering the space are accountable for their own safety and are trained in CSE to a nationally recognised competency standard and complete tasks compliant to the scope of work

Emergency Response Personnel

Emergency response personnel are accountable for ensuring they have contact with the Sentry via an appropriate communication system and Provide appropriate response and rescue where required.

Ventilation Officer Determines if a ventilation plan is required. Where required, drafts the ventilation plan and provides determination of the type and frequency of monitoring.

Authorised Gas Tester

Trained in confined space gas testing to a nationally recognised competency standard. Accountable for ensuring gas testing is conducted in accordance with the CPM Confined Space Atmospheric Testing Instruction and follows the requirements of testing as per the Confined Space Entry Certificate and Scope of Works.

5 Requirements 5.1 Determination and Evaluation of Confined Spaces

Confined space determinations and evaluations shall be conducted by a team of competent personnel who will consider and evaluate all possible items of plant that could be classified as a confined space. Evaluations shall be performed using DR010904 Confined Space Determination and Evaluation Form.

Once a space has been defined as a Confined Space the evaluation team in conjunction with AS 2865 and DR033304 Confined Space Hazard Identification Guide complete DR033302 Confined Space Identification and Risk Analysis Worksheet to identify risks and apply appropriate controls.

5.2 Reclassification of a Confined Space For a confined space to be reclassified as a non-confined space, it needs to have undergone sufficient change in structure or usage to eliminate (without the need for risk control measures) all possible sources of inherent hazards that define a confined space. Any changes to a confined space would have to be such that a subsequent risk assessment would determine that it no longer meets the criteria for a confined space (Clause 2.4.6 AS 2865-2009).

Reclassification of Confined Spaces requires consultation with the General Manager Health & Safety and approval of the Registered Manager.

5.3 Confined Space Register A Confined Space Register shall be maintained by the responsible Area Manager. The register shall contain the following information:

• A description of the unique identification number of each space;

• A copy of the completed DR033302 Confined Space Identification & Risk Analysis Worksheet for each space;

• A copy of the Isolations Checklist for each space (if required);

• A copy of the DR028987 Confined Space Rescue Plan (if required);

• A copy of DR047957 Ventilation Plan (where required).

5.4 Hazard Identification In and Around Confined Spaces All hazards associated with the space, or with work required to be performed on or inside the space, shall be identified, risk assessed and appropriate control

measures implemented to eliminate, or otherwise minimise, the risk posed by those hazards. In the absence of a suitable Safe Work Instruction, hazard identification shall occur by creating a task based JHEA.

The Confined Space Identification & Risk Analysis Worksheet shall be reviewed as part of this risk assessment process. Where changes to the space and inherent risks are identified, these shall be updated as required.

5.5 Ventilation Plan Requirements Confined spaces may contain atmospheres that are flammable, toxic or whose oxygen level has been depleted or enriched.

Although a single rule or group of rules is unlikely to cover all of the ventilation requirements applicable to confined spaces, the general objectives of ventilation in confined spaces are to:

• remove contaminated air (flammable or toxic) from the space and maintain safe concentration levels in terms of exposure standards (ES) or lower explosive limit (LEL);

• provide fresh and breathable air inside the space;

• create a more comfortable environment inside the space.

Ventilation requirements to enter and work in confined spaces depend on the nature of the space, its content, and the operations that will be performed inside the space.

The CPM Hygiene department must be contacted to determine if a ventilation plan is required if the confined space:

• has an atmosphere containing or likely to contain potentially harmful levels of contaminant;

• the space normally contains a potentially harmful contaminant;

• potentially harmful contaminants or work activities such as hot work, painting or use of adhesives are being introduced into the space;

• has or is likely to have an unsafe oxygen level; or

• is of a nature or is likely to be of a nature that could contribute to a person in the space being overwhelmed by an unsafe atmosphere or contaminant.

The Ventilation Officer or appointed delegate is responsible for completing DR047957 CPM Confined Space Ventilation Plan.

If a Ventilation Plan is required, the Hygiene department are to be given a minimum of 48 hours’ notice of intended works. Requests are to be made using DR048671 Ventilation Request Form.

5.6 Signage and Tagging All confined spaces shall be identified, evaluated and signs erected at the entry points denoting that a permit is required prior to entry. Signage must comply with AS 1319 -1994 Safety Signs for the Occupational Environment. Please refer to Appendix A.

Ventilation equipment shall be clearly identified and tagged to prevent unauthorised operation to comply with AS 2865–2009 Confined spaces clause 3.4.20 Control of Mechanical Ventilation Equipment.

5.7 Isolation Prior to entering a confined space all potentially hazardous energies, as identified in the Confined Space Identification and Risk Analysis Worksheet attached to the space, JHEA or other relevant documentation, shall be positively isolated where it is possible to do so. Where it is not possible to do so, a separate procedure shall be developed and appropriate control measures implemented to minimise the risk to personnel.

An Authorised Isolator (electrical/mechanical) will carry out all isolations and apply permit tags and locks in accordance with the Permit to Work process.

5.8 Lighting and Other Portable Electrical Equipment For all work in a confined space, lighting and portable electrical equipment shall comply with provisions of AS 2865-2009, clauses 3.4.44 to 3.4.47.

Temporarily installed lighting should have an emergency backup power supply.

Portable electrical equipment

Portable electrical equipment should be either:

• connected, individually or collectively to an earth-free, protected extra low voltage supply from an isolating transformer;

• protected through a RCD located outside the space.

Where there is potential for combustibles, portable equipment should be intrinsically safe.

Supply cables

Portable electric equipment should be fitted with a flexible supply cable not inferior to a heavy duty type. The cable should be located, suspended or guarded to minimise accidental damage.

Double – insulated tools

Where available, it is recommended that double–insulated electrical tools be used with the appropriated IP rating.

5.9 Atmospheric Testing Gas testing should be carried out in accordance to DR033453 CPM Confined Space Atmospheric Testing Instruction. No person shall enter a confined space until an Authorised Gas Tester has verified that:

• The confined space contains safe levels of oxygen (between 19.5 and 23.5%);

• The concentration of any flammable contaminant in the atmosphere of the space is below 5% of its LEL.

A competent Authorised Gas Tester shall carry out atmospheric testing of the confined space at a time and frequency as identified on the Confined Space Entry Certificate. A person entering a confined space to perform atmospheric testing/monitoring shall have a Confined Space Entry Certificate to do so and comply with the requirements of this procedure.

Gas testing shall occur at least 30 minutes after mechanical ventilation has been isolated and not greater than 30 minutes before first entry of the confined space. Where confined spaces have been cleared of all personnel for greater than a 30 minute period, testing must re-occur prior to re-entry.

Where entry certificates remain open, atmospheric testing must occur at intervals not greater than 12 hours.

5.10 Cleaning, Purging and Venting A safe atmosphere can be achieved within the confined space using methods such as cleaning, purging and ventilation. Where contaminants are present within a confined space, they are to be cleaned out prior to entry by the use of a suitable purging agent that is not pure oxygen or gas mixtures with oxygen content greater than 21%. Confined space ventilation should be by natural, forced or mechanical methods to establish and maintain a safe atmosphere.

Where activities inside a confined space introduce toxic fumes (i.e. welding, grinding, gluing) constant venting/purging of the atmosphere is required. Air drawn from fans need to be clean air upwind and 10m away from any generators/pumps.

Exhausted air must be expended into the atmosphere at a location where it does not present a risk to personnel in the vicinity.

Each department with ventilation fans must ensure they are kept secure (i.e. in the PTW Office) to prevent unauthorised or misuse. Fans are to be inspected and tested on a monthly basis to verify functional reliability.

NOTE: Information tags are to be placed on the power board connected to the ventilation fan(s) to prevent disconnection during confined space entry activities.

Where the atmosphere cannot meet the safe levels required, only competent persons with self-contained breathing apparatus or airline apparatus and emergency back-up can enter a confined space.

Atmospheric monitoring and testing shall be performed consistent with the hazards identified and in accordance with the Confined Space Atmospheric Testing Instruction.

5.11 Issuing of the Confined Space Entry Certificate A Confined Space Entry Certificate can only be issued by Authorised Permit Issuers.

CSE certificate will only be issued after the completion of:

(i) associated Permit to Work with JHEA / SWI;

(ii) First Response Rescue Plan;

(iii) a ventilation plan (if required as per section 5.5); and

(iv) atmospheric monitoring conducted by a competent person(s), for the work to be performed.

The JHEA, which is required for all Permits to Work authorised under the Confined Space Entry Certificate, shall consider:

• if the need to enter the space can be eliminated by performing the work from outside the space;

• the nature of the confined space;

• all hazards; and

• effective control measures to minimise the risk of hazards (e.g. cleaning, purging, atmospheric testing, breathing apparatus, ventilation, barricading, communication equipment etc.).

The Confined Space Entry Certificate shall be prominently displayed at the entry point to the confined space for the duration of the work and shall indicate:

• the time and date the permit is valid;

• the name of the person in control of the work (Permit Holder);

• special precautions required for the safe entry and control of the work;

• the date, time and signature of any person(s) entering and leaving the confined space;

• the date, time and signature of the Standby Person (Sentry);

• the results of atmospheric testing;

• a register for all Permits to Work issued for working inside the space.

Associated documentation that shall be kept at all times with the Confined Space Entry Certificate includes:

• First Response Rescue Plan;

• Ventilation plan (when required);

• Atmospheric Monitoring Sheet and CSE Entry/Exit log (where ongoing or periodic monitoring is required).

• Permits to Work with their relevant JHEA / SWI and certificate(s).

Where associated paperwork identifies additional control measures to those already stipulated for general entry to the space i.e. increased gas test frequency, temperature monitoring, or hearing protection, then the need for additional controls must be indicated in the Confined Space Entry Certificate .

Confined Space Entry Certificates may be transferred from one Permit Holder to another during the course of its validity. The person accepting the permit / certificate as the Permit Holder accepts all the responsibilities and accountabilities to ensure that the conditions of the permit / certificates are met, that hazards are constantly assessed and control measures are in place and effective.

If there is more than one access point to the confined space, the other access points shall be sign posted and barricaded to prevent involuntary and unauthorised entry but allows emergency egress.

5.12 Revalidation / Relinquishment Re-validation at beginning of shift or task commencement, and relinquishment at end of shift or task, is required for all Confined Space Entry Certificates.

The Permit Holder shall ensure that the workplace copy of the Permit to work and the Confined Space Entry Certificate is relinquished to the Permit Issuer upon completion of works for the shift. Relinquishment must not occur until:

• all personnel have left the confined space and all openings of the confined space are barricaded and sign posted to prevent unauthorised entry;

• all personnel have removed their personal locks and danger tags from the Group Lock Box;

• all personnel have signed off the entry/exit log; and

• the work site has been left in a clean and safe condition.

At the commencement of shift the Permit Holder shall re-validate the Permit to Work and Confined Space Entry Certificate by signing the re-validation section of the original and duplicate copies.

5.13 Safety Equipment All safety equipment identified in the JHEA / SWI as being required shall be provided, including equipment for:

• personal protection;

• communication equipment (i.e. two-way radios, horns, phone);

• emergencies, including rescue;

• first aid; and

• fire suppression.

Safety equipment shall be appropriate for the work and maintained in good condition.

When the risk assessment determines that fall protection equipment is the best control measure, fall restraint equipment/techniques shall be the first choice, unless the potential for a free fall cannot be eliminated, in which case fall arrest equipment shall be used.

5.14 First Response Rescue Appropriate First Response Rescue with first aid procedures / provisions and emergency response shall be planned, established and rehearsed as per the Confined Space First Response Rescue Plan for the specific confined space. All First Response Rescue Plans shall be developed in consultation with on-site Emergency Services.

The work group intending to work in the confined space must obtain the relevant First Response Rescue Plan from the Permit to Work Office and complete the following prior to the issue and acceptance of the Confined Space Permit to Work:

• task description;

• exclusion zone(s);

• non ERT and personnel equipment requirements – particular to the specific confined space;

• initial emergency response – i.e. what will the workgroup team members do to remove any person from a confined space in the event of an emergency;

• ensure a sketch or photo of the confined space showing the controlled entry exit locations;

• the nominated ERT pickup point;

• site emergency contact phone numbers;

• communication.

The responsible work group Supervisor shall review and approve the completed Plan. In cases of response by Emergency Services, those persons involved in the response shall be made aware of the conditions in the confined space prior to any entry:

• all rescue equipment must be readily available and regularly serviced for use by competently trained emergency response personnel;

• all persons who may be involved in any way with emergency response associated with a confined space, shall be made aware that:

- well-planned and well-rehearsed emergency response procedures are essential and shall be followed at all times; and

- openings for access and egress to and from a confined space should conform to the relevant Australian Standards to permit rescue of all persons who may enter the confined space.

6 Control, Responsibility and Accountability The generic accountabilities and responsibilities of the various parties have already been indicated, however it is essential that the specific responsibilities are set out very clear to ensure no misinterpretation or confusion when applying the procedure. The specific responsibilities of the parties are listed:

• The Permit Issuer;

• The Standby Person(s) (Sentry);

• Authorised Gas Tester;

• The Permit Holder (Permit to Work);

• The confined space entrant(s); and

• Ventilation Officer.

6.1 Permit Issuer Permit Issuers must comply with all general requirements as listed in the DR044607 CPM Permit to Work Guideline and specifically to Confined Space Permits:

• when work is scheduled involving confined space entry, the Permit Issuer raises the Confined Space Entry Certificate;

• provides the Authorised Gas Tester with existing risk assessment information identifying the potential atmospheric contaminants that require testing;

• determine that risk assessments and atmospheric testing methods adequately identify all potential atmospheric hazards that may exist in the confined space;

• withdraws the Confined Space Entry Certificate if a breach of hazard controls is observed.

6.2 Standby Person / Sentry The Standby Person (Sentry) is the vital link in the control of safe entry into and work within the confined space.

Responsibilities include:

• be trained, and assessed as competent in confined space entry;

• be trained and competent in the use of gas monitoring equipment;

• have no other duties other than Standby Person / Sentry;

• have knowledge of the isolation points for the specific plant or equipment;

• have knowledge of the hazard control requirements for the Confined Space Entry Certificate and associated Permit(s) to Work that will apply to the activity inside the confined space;

• provide all conditions, precautions and controls to those entering the space and any emergency response personnel;

• ensure that additional hazard controls required are indicated on the Confined Space Entry Certificate;

• ensure that all movements in and out of the confined space are recorded on the Entry Exit Log;

• be positioned outside the confined space entry point at all times while personnel are within the space. The Sentry shall be in continuous communication with any person inside the space via radio equipment or other appropriate means (visual if possible);

• operates and monitors any safety plant and equipment used to ensure the health and safety of those inside the confined space as required;

• monitor any changes to the hazards and risks within the confined space and the surrounding environment and takes appropriate action where there is risk to health and safety;

• activate the First Response Rescue Plan or where required, initiate site emergency response;

• assists the Permit Holder, where required, in advising emergency response personnel of conditions within the confined space prior to entry;

• when it has been determined that there is a potential for injury during access or egress to or from a confined space, or at any time where height safety is a concern, the Sentry(s) shall ensure that all persons entering the space are attached to a safety line if reasonably practicable to do so.

6.3 Authorised Gas Tester Authorised Gas Testers (AGT) must be trained in confined space gas testing to a nationally recognised competency standard. The candidate must complete Gas Test Atmospheres:-Theory and Practical Assessment which includes three (3) different practical tests within the business area. On successful completion of the test the area manager will be able to appoint the candidate. See DR035106 PTW Appointment Form. The appointed AGT must be able to demonstrate ongoing compliance to the gas test of atmospheres in accordance to this procedure and AS 2865 -2009.

An Authorised Gas Tester is accountable for ensuring that:

• gas testing is conducted in accordance with the CPM Confined Space Atmospheric Testing Instruction;

• gas testing equipment is calibrated and in good working order as per OEM instructions;

• analysis and monitoring of the confined space for an irrespirable atmosphere is performed prior to any entry;

• they record the initial atmosphere testing results on the Confined Space Entry Certificate and any on-going measurements are entered on the Atmospheric Monitoring Record Sheet.

6.4 Permit Holder Permit Holders are accountable for workers safety by ensuring that:

• they have viewed and understand the Permit to Work, Confined Space Entry Certificate and supporting documents;

• after reviewing the Confined Space Identification and Risk Analysis, participate in the development of, and understand the risk controls in, the SWI / JHEA(s) with consideration given for undertaking the activity within the confined space;

• the SWI / JHEA(s) and all relevant documents (i.e. SDS, rescue plans) are reviewed by all persons entering the confined space;

• ensure all conditions of the Confined Space Entry Certificate are adequate and additional controls identified in the Permit to Work are implemented prior to confined space entry;

• at the end of shift, or if otherwise required, ensure all personnel are clear of the confined space and relinquish the Permit to Work to the Permit Issuer;

• upon completion of the work within the space, the Permit Holder closes the Permit to Work off in the Confined Space Entry Certificate.

6.5 Ventilation Officer The registered Ventilation Officer (VO) is responsible for making the final decision as to whether a ventilation plan is required prior to entry of a confined space. This decision is made with full consideration of potential chemical contaminants, welding fume and oxygen levels arising from both the local environment and the proposed works. The VO also calculates the required air flow and atmospheric turnover within any given confined space, to provide the appropriate level of safety for any workers entering that space. This is recorded in the ventilation plan.

6.6 Confined Space Entrants All individuals entering the space are accountable for their own safety by ensuring that:

• they are trained in confined space entry to a nationally recognised competency standard and hold a current VOC in accordance with DR022088 Verification of Competency Procedure;

• they participate in conducting, or read and understand, the JHEA prior to entry as well as any applicable SWI’s, rescue plans, or SDS’s

• those individuals entering a confined space are responsible for ensuring they familiarise themselves with all applicable instructions for the task to be undertaken within the confined space;

• if a Safe Work Instruction (SWI) fails to completely cover every aspect of the task to be undertaken within the confined space, then a task based risk assessment (JHEA) must be conducted for the intended task prior to entry;

• if the scope of work or conditions change from those present in the JHEA / SWI, they stop work and conduct a re-evaluation analysis;

• the conditions of entry on the Confined Space Entry Certificate and the SWI/JHEA must be discussed in detail with the Permit Holder prior to the individual(s) signing onto the permit and commencing work within the confined space;

• they fully comply with all control measures identified in the JHEA / SWI and Confined Space Entry Certificate;

• they sign on to the entry/exit log prior to entry and when leaving the space;

• the individual(s), while working within the confined space, must monitor changes to conditions of the space, changes in the tasks being performed and, if required, vacate the space and update the JHEA. It may be necessary to have the Sentry update and revalidate the permit before re-entry is allowed; and

• upon completion of the work the individual(s) exit the space and sign off the permit.

7 Competency / Training Requirements All CPM and contractor personnel required to work in confined space or conduct Gas testing shall hold a current National Unit of Competency. This competency shall be renewed through a registered training organisation every five (5) years. An on-site validation of competency shall be undertaken every 2 ½ years with CPM Training department to maintain competency

The minimum training competency of any person entering a confined space includes:

• RIIWHS202D – Enter and Work in Confined Spaces

• MSMWHS217 – Gas Test Atmospheres (Applies only to Authorised and Appointed Gas Testers)

All relevant training documentation is to be available for audit by designated CPM personnel. CPM employees are to provide the documentation to the CPM Training department prior to undertaking any confined space entry related tasks.

Only persons trained in confined space entry and this procedure are to be appointed and nominated to act as Permit Issuers and Permit Holders where

required. Nominated Permit Holders and Standby Persons must be competent through knowledge, experience and training to be able to effectively:

• initiate site emergency response procedures; • operate communication equipment as applicable; • operate and monitor any safety plant or equipment that may be required; • monitor any changes to hazards or risks within the confined space and take

appropriate action where any changes may result in a risk to health and safety.

Ongoing training and verification of competency shall be in alignment with DR022088 Verification of Competency Procedure.

8 Implementation The following points are general requirements when working within, or in association of a confined space:

• a Confined Space Entry Certificate is valid for a maximum of 30 days;

• only persons trained and competent in confined space entry, issue and acceptance of Confined Space Entry Certificates and this procedure are to be nominated as Permit Issuers and/or Permit Holders;

• only registered Permit Issuers are to raise and issue a Confined Space Entry Certificate;

• only registered Authorised Gas Testers are to conduct atmospheric testing and monitoring;

• only persons trained and competent in safe work in confined spaces shall sign onto DR033058 Atmospheric monitoring and CSE Entry / Exit log, and work within or on a confined space;

• only persons suitably trained and assessed as competent are to undertake the duties and responsibilities of a confined space Standby Person (Sentry);

• in the event of an emergency situation arising within a confined space, the Confined Space Entry Certificate is automatically cancelled;

• during an emergency situation within or on a confined space, the emergency response personnel and emergency response initiators are exempt from the requirements of this procedure. However, access to the confined space shall not be undertaken without the correct RPE being worn, minimising the risk of exposure to potential contaminates within the space.

9 Confined Space Entry Process 9.1 Details

The location or area, plant, equipment and space name are recorded in this field. It is important to provide as much detail in the field as possible to avoid confusion in an emergency situation.

Work cannot commence until a JHEA / SWI has been completed for the intended task to be performed within the confined space. The JHEA / SWI must be attached to the Confined Space Entry Certificate for all entrants to review prior to entry.

9.2 Communication The most suitable and practical form(s) of communicating an emergency and initiation of evacuation from the confined space is identified. The general means of communication between the Standby Person and the Permit Holder / work group is also identified.

9.3 Gas Testing and Monitoring With consideration of the risks identified in the associated JHEA (s) or SWI(s) requirements for gas testing frequency are identified.

Gas testing monitors and docks will be operated, calibrated and maintained as per OEM recommendations and manuals. Calibration and bump testing records will maintained with in a register by the departments.

9.4 Measurement of Atmosphere This section is to be completed by the Authorised Gas Tester (AGT). Prior to all confined space entries, the space must be tested to ensure a safe atmosphere. The AGT may re-assess the need for continuous monitoring, conditions of entry as well as the need and frequency of re-testing based on initial test results. Once the AGT has determined the atmosphere is safe, they must record the gas meter identification number, the AGTs name, signature and date against the initial measurements.

9.5 Control Measures This section lists the minimum required control measures as determined by the Permit Issuer and the Permit Holder in conjunction with the requirements from the JHEA.

The aim of this section is to allow the Permit Issuer to determine which of the listed controls are necessary for a particular confined space entry. If a control measure has been identified then the Permit Issuer is responsible for ensuring that the control measure is in place prior to entry as well as informing the Permit Holder and the Standby Person of this requirement.

9.6 Permit Duties Acceptance by Permit Holder Following review of all submitted documentation including Confined Space Determination and Evaluation Form, First Response Rescue Plan, Ventilation Plan, JHEA(s) or SWI(s) and Confined Space Entry Certificate, the authorised Permit Holder acknowledges his/her duties and responsibilities by signing and dating this section

9.7 Permit Re-Authorisation The Permit Issuer records the validity period of the permit in this section. The maximum time a Confined Space Entry Certificate is valid for is 30 days. However, the permit must be relinquished and revalidated at the end of every shift (not the length of one shift, but rather until the shift in which the entry started is over).

10 Change of Standby Person / Sentry This section is used when the Standby Person / Sentry can no longer carry out the duties and responsibilities of the position within a normal shift. In which case, another suitably trained and competent Standby Person / Sentry must assume the duties and responsibilities and ensure the conditions of the permit are still valid.

It is not necessary to cancel the permit and raise a new permit within the same shift, but it is essential that the incoming Standby Person is aware of all conditions and control measures before transfer occurs.

The permit should be re-validated whenever it becomes evident that the duration of work will involve one of the following:

• A change of the person responsible for the direct control of the work in a confined space;

• A significant break in work continuity; or

• A significant change in risk, e.g. atmosphere, or work to be performed.

NOTE: The Confined Space Entry Certificate must stay at the point of entry of the confined space for the duration of the occupancy and shall not be removed during the transfer of the permit. In these situations the Permit Holder must be advised and attend the work area to sign authority to transfer the permit.

The permit cannot be transferred across shifts. If the work is to continue after a change of shift, the permit must be relinquished and re-validated.

11 Cancellation of Confined Space Entry Certificate The Confined Space Entry Certificate can only be cancelled by the Permit Issuer after verifying that:

• All persons are out of the space and signed off the permit;

• All tools and equipment have been removed;

• It is safe to return the space/equipment back to normal operations;

• DR033060 Confined Space Final Inspection has been completed.

12 Reporting / Recording and Auditing Reporting and recording is mainly completed through the Confined Space Entry Certificate as outlined under section 8. All parties involved are required to fill in all the detail in applicable sections of the certificate, and sign off in the sections as required. The document is not complete until it has been cancelled and signed off. These completed documents shall be retained in accordance with AS 2865:2009.

Training records of CPM personnel shall be kept on a training database for every employee, and will be retained for the term of the employee’s employment period. A hard copy of the actual documentation generated during the assessment after training, will also be kept on a personal file for this purpose.

Random audits shall be carried out on confined space work to ensure compliance with this procedure. Any non-compliance shall be reported to the Area Manager or Superintendent for further investigation and follow-up. Training records shall be checked randomly to ensure currency of training and will also be triggered through the CPM training database.

13 Records Management Once the certificate has been approved, the Permit Issuer will hand to the Permit Holder the workplace copy of the Confined Space Entry Certificate along with any other Permit to Work and supporting documents.

The Permit Holder will take the Confined Space Entry Certificate and other supporting documents to the Permit to Work users and walk through the requirements.

The Permit Holders will sign onto the work permit at the start of the job and off at the completion of the job.

On completion of the job, the Permit Holder takes all documents back to the Permit Issuer and the permit(s) are signed off.

The Confined Space Entry Certificate and supporting documents shall be retained on file for audits and statutory purposes. The Permit Issuer is responsible for maintaining the file.

All permits / certificates shall be retained in accordance with DR008575 CPM HS Document and Record Control Procedure requirements.
