Since 1903, Cozad Community Health System has been committed to providing the region’s highest-quality healthcare. Today, the team at Cozad Community Health System can answer your family’s healthcare and wellness needs through a range of hospital, clinical and physical therapy services.

Cozad Community Health Systems offers comprehensive inpatient and outpatient services, 24-hour emergency care, Home Health and Hospice services, Physical Therapy, and a full array of other services.

Central Plains Home Health and Hospice has been providing home care since 1971.

Home Health Care is provided by skilled professionals on a part time and intermittent basis in the patient’s home. Home health care is always provided under a physician’s direction. Home health is a way for patients to continue to receive skilled nursing, physical therapy, occupational therapy or speech therapy following hospitalization or a change in health status. Elderly persons, who have a gradual decline in health and ability to function independently, may also benefit from skilled care at home in order to increase independence and ability to remain in their own home.

In-home physical therapy services that may be provided may include:

Exercises to increase and maintain muscle strength and endurance, gait training, learning to use a walker, cane or crutches and exercises to regain strength and mobility following orthopedic surgery.

Persons with closed head injuries and strokes may benefit from speech therapy services.

Occupational therapy services provided in home may include: Teaching to increase independence with activities of daily living, instruction in use of adaptive equipment, assessment of home safety and education on fall risk factors and prevention.

Cozad’s Golden Living Center, located at 318 W. 18th in Cozad, is a skilled, nursing facility offering rehabilitative services and therapeutic care as well as an assisted living unit, skilled nursing care, an Alzheimer’s unit and an advanced Alzheimer’s unit.

The Medicare, Medicaid certified and Veterans Affairs contracted facility also provides end-of-life care, and maintains a hospice room. The hospice room has extra space and added privacy.
