Your grade is based solely on the number of points you accumulate. Possible points are as follows:

Grading scale for 60 point section exam:90-100% = A 363 - 404 = A 54 - 60 = A80-89% = B 323 - 362 = B 48 - 53 = B70-79% = C 282 - 322 = C 42 - 47 = C60-69% = D 242 - 281 = D 36 - 41 = Dbelow 60% = F 241 & below F 35 & below F

RULES:

1. YOU WILL BE ALLOWED TO MAKEUP ONE SECTION EXAM if you are absent at test time. Ten percentage points will be deducted from the makeup exam score regardless of the reason for your absence. THE MAKEUP EXAM WILL BE GIVEN IMMEDIATELY AFTER THE FINAL EXAM AT THE END OF THE SEMESTER.

2. THERE ARE NO MAKEUPS FOR CLASS QUIZZES —Miss the quiz, miss the points

3. Medical Record Homework Requirement: Record your answers to each question in the spaces provided in the text. Tear the pages of the completed assignment from the book or photocopy intact pages and staple together as necessary. Print your name and seat number in the upper right hand corner of the assignment and hand it in prior to class on the date due.

4. No medical record assignments will be accepted early or late .

5. Each section exam will include information covered in Chapters 1-4. You must sit for all section exams to receive credit for the course. It will be necessary for you to purchase five (5) ParSCORE TEST FORM scantrons (F-289-PAR-L) for testing purpose. They will be collected at the beginning of the second class meeting.

ACADEMIC DISHONESTY/CHEATING: An incidence of cheating will be reported to the Dean of Students with a recommendation for disciplinary action consistent with the Academic Honesty Policy and the Student Code of Conduct. Examples of cheating are:
