•Acute Stroke services•Emergency Surgery•Trauma services•Cardiology services•Acute medicine and long term conditions•Pharmacy provision•Radiology services•Non-clinical and other support services

Sunderland Community ‘Vanguard’ Programme based on 3 major transformations:

•City-wide Recovery at Home Service•5 locality based Community Integrated Teams•‘Enhanced Primary Care at scale’
