Trial Design: The CARISA trial was a randomized, double-blind trial of ranolazine (750 mg 2x/d; n=279),ranolazine (1000 mg 2x/d; n=275), or placebo (n=269) in patients with symptomatic chronic angina alreadytreated with other anti-anginal drugs. The primary endpoint was treadmill exercise duration at troughranolazine levels (i.e., 12 hours after last dosing).

Results• Half of study patients had history of prior CAD.• Total cholesterol, LDL, and triglycerides significantly and HDL significantly in fenofibrate arm compared to placebo (p<0.001 for each).• Fenofibrate was associated with smaller decrease in minimum lumen diameter (-0.06 vs. -0.10 mm, p=0.029), smaller increase in % diameter stenosis (2.11 vs. 3.65%, p=0.02), and a nonsignificantly smaller decrease in primary endpoint of mean segment diameter (-0.06 vs. -0.08 mm, p=0.171).• There was a mild correlation between angiographic changes and changes in lipid profiles; effect of fenofibrate was similar in all subgroups.
Conclusions• Although the primary endpoint was not significantly different between groups, the reduced rates of disease progression measured by parameters of focal disease including % diameter stenosis and min lumen diameter suggests that lipid-lowering therapy in diabetics can result in the slowing of focal coronary atherosclerotic disease.

Results• Mean duration of follow-up was 27 months.• Time to first ischemic event was similar; frequency of cardiac events was 38% in both groups (p=0.6).• There was no difference between groups in early or late major cardiac events or all-cause mortality.• There was no difference in the incidence of PTCA in the groups.• In the angiographic substudy of 477 patients, there was no difference in disease progression (47% for quinapril vs. 49% for placebo, p=0.71).
Conclusions• Among patients with CAD and preserved LV function, treatment with quinapril 20 mg/day was not associated with a decrease in ischemic events or progression of disease at 2 years.• This result stands in contrast to the larger HOPE study, which demonstrated a reduction in events with the ACE-Inhibitor ramipril during a mean 5 year follow-up period.

nTrial Design: REVASC was a multi-center randomized trial of AdVEGF121, an angiogenic growth factor(n=27) delivered via a minithoracotomy with direct intramyocardial injections throughout the free wallof the left ventricle vs control (n=29) in patients with severely symptomatic CAD who were notcandidates for conventional revascularization. Patients were followed for 26 weeks. The primaryendpoint was exercise treadmill time to additional 1-mm ST depression at 12 weeks.
