Tasks being Performed Potential Safety Risks/Hazards Safety Controls / Mitigations This is a TEMPLATE Set up Machine & work area (delineate as necessary) Note: Traffic/ Pedestrian control plan maybe required (Penetration Permit Required) Core drill holes Saw cut Use a Wet Vac to clean up slurry, A second person maybe needed for set up & the capture of slurry. (and remove these directions). Risk of Injury to bystanders Work area should be delineated off from Un-Authorized personnel & signs posted All personnel must use caution when in or around saw cutting areas Proper PPE shall be worn by adjacent personnel, as required by their proximity to the work task. Risk of hitting underground utilities. Ensure all Areas to be cored drilled have been scanned. Follow Penetration Permit Risks of injury from improper set up or operation. All connections, mountings, guards & controls on the equipment must be inspected prior to use. Ensure there has been a Penetration Permit briefing & permit is signed & on site. All operators of this equipment shall be trained on that piece of equipment. Operator must wear proper PPE. (Face shield, safety glasses & hearing protection minimum). Risk of exposure to silica based products Half face respirators are required for core drilling into concrete. Respirator Program with medical eval & fit test shall be submitted for review & acceptance by EH&S prior to the start of work Follow LBNL SWPP Program as required 1 of 1

If storm drains are below work areas ensure the drain covers are surrounded by waddles, lined with silt mess covers & de-chlorination tablets are in place.

Shop Vac shall be used not to allow slurry out of work area for proper cleanup
