B. D. Sharma University of Health Sciences, Rohtak If the form is required by post, a self-addressed envelope (23x10 cm) duly affixed with postal stamps of Rs.40/- may be sent alongwith request & with the above fees. Candidate must clearly state the name of the post (with category) for which form is required and also give his/her name and address in block letters.Lower and upper age limit for the post is 18-40 years. Upper age limit is relaxable by five years for candidates of SC/ST and BC including Economically Backward Classes (whose family income from all sources is less than Rs.3600/- Per Annum applicable from time to time) recognized in Haryana, by ten years for Physically Handicapped for the reserved category posts only, and in certain other cases as per Haryana Govt. instructions issued from time to time on the subject. More detailed information is available in the “Instructions for candidate” which will be supplied alongwith the application form. The candidates who are already working on contract basis will be given relaxation in age to the extent of their contract service rendered in this institution only provided they fulfil the required qualification and experience for the post. The applications complete in all respects must reach in the office of Medical Superintendent, A-Block (Room No. 8) PGIMS, Rohtak on any working day from 9.00 AM to 5.00 PM by 31.12.2012.  7. The posts of reserved categories will be filled up as per latest Govt. instructions/ applicable issued

from time to time.

8. In case of candidates belonging to Scheduled Caste and Backward Class category applies against

General Category post he/ she should fulfill the condition of eligibility regarding age/application fee

etc. as are meant for General Category candidates as per Govt. instructions issued vide letter No.

22/88/96-3GSIII date 25.6.97.9. If applications are received in large numbers, the appointing authority can short list the candidates

for interview by holding screening test or on the basis of an academic criteria etc. The decision of the

appointing authority in all matters relating to acceptance or rejection of application, eligibility / 

suitability of the candidates, mode and criteria for selection etc. will be final and binding on thecandidates.

10.  The eligibility of the candidate i.e. qualification, experience and age will be considered upto the last

date of receipt of applications i.e. 31.12.2012.

11. The applicant should ensure while applying for the post that he/she is eligible for the advertised post

of Staff Nurse as per advertisement.

12. Any attempt by the candidate to influence the members of the selection committee OR canvassing in

any form or manner for selection to the post will be an outright disqualification and the Universityhas a right not to consider the candidature of such candidates.

13. The candidates who have donated Blood must submit a documentary proof of the same with the
