• Why do we have pre-ops? • We have to do them to ensure equipment functions • Periodic maintenance • It is the law. • Make sure your checklist suits the equipment. • Record all defects until corrected. • The Operator needs to know when to park the equipment. • Observe pre-ops. • Follow up with your Supervisor on any defects you find. • Mike Mascioli: While working at Coleman, Mike went to the driller’s workplace to ask him about

his pre-op. The driller identified that his biggest hazard was at the face: popping, bursting, loose muck.

• What if you have an engine fire? (climb over unit) • How do you check your ansul? The driller could not ID the ansul point. • Be sure to ask questions in the field.

Lenel Pass Prior to Indocs – Aaron Hutchings

• Vale will be re-introducing Delivery Indocs. • To get a Lenel card, get approval from your Vale contact. • Plant Maintenance Personnel: Contact your Vale contact at the Plant. • All Mines approvals can be done by one mine site. • The Pass Office will accept pictures via email for large groups. • For large groups, the pass office will set up remotely. • Plant Manager to sign for gate access- process unchanged. • Any Vale contact person can sign for a lenel card. • Stickers will still be added to Norcat cards. • July 4th is the deadline to have your lenel pass. • There will be a transition period until July 31st. • Forms located on extranet site:

• Equal system is being replaced by the Lenel & VES system. • Lenel cards are required to be on site. • You do not need a new card if you already have one. • Renew your lenel card when you renew your Norcat. • If you do Orientations, you need your Lenel card. It is how your orientations will be recorded. • If you are escorted by Vale personnel, you do not need a lenel card. You must sign a waiver,

however. • Vale will let the Contractors know how to manage employees moving to other companies. • Call Norcat if your lenel pass needs updating; Norcat will flag it.

Vale Alcohol & Drug Instruction– Keith Hanson

• There have been no changes to the policy since 2010. • Employees must be fit for work. • Possession of alcohol, drugs, or paraphernalia is a violation. • Vale will perform testing if there are observable signs of use present. • A Vale Manager must approve testing. • The Supervisor of the Employee being tested must attend the test. • A refusal to sign is a refusal to test. • Positive test results result in removing the Employee from Vale property indefinitely. • Post incident testing: If the person is at fault, they will be tested. • Don’t let the person go home. They cannot be tested if they leave the property. • Supervisors can be tested. • If an employee is sent home, they can come and pick up their vehicle the next day. • Document employee drop offs in case something happens. • One in five tests is positive. • Marijuana: It is not considered a prescription drug. • No alcohol permitted on Vale property. • Do not leave alcohol or empties in your vehicle while parked on Vale property.

• TRIF at this time last year was 5. Why? • Industry uncertainty • Lack of consistency in work. • Dennis Sobey: People are reporting more, culture change? • Duration of jobs? More mob and demob? • Lots of smaller maintenance work going – shorter jobs. • Not as many larger capital jobs. • People are doing different jobs. • Senior people are being moved back into working roles. • Employees may be complacent, not looking at the right things. • Get back to basics, have open ended conversations. • Observe Employees and Supervisors in the field. • PMO group is fortunate to have senior people to mentor junior people. • We are still hurting people. We get away with high potential incidents. • Most reports are routine hazards. • Don’t beat people up for reporting First Aids. Continue reporting. • Who uses the stop and correct data in their monthly talks? Are you using the data? • Barricades and Signs was the latest theme. • Do we assume that mob and demob are routine tasks? • Was a PHR done on set up? • Have workers participate in PHRs. They are designed for them. • Do interactive audits. Understand requirements from start to finish. • Consider young workers. • People are reporting hazards • Ask Jim or Chris. They can be available for safety talks, data, etc. • We are losing good experienced people. Competitive industry. • There were 1411 stop and corrects. There should be more of them. • An act of safety is a stop and correct.

• Entry Supervisor requires rescue • Entry Supervisors need to have their training done with Vale Learning & Development. • It is 1 ½ days, and can be scheduled with the PMO. • If you need any assistance, contact Jim Mathiasen.

Open Discussion

• High Visibility work wear is required by July 1st. • Must be: all orange, red, or yellow/green. • MOL has not given Vale an order yet for non-compliance. • If you do not have high-vis clothing by July 1st, then you must have a compliance plan arranged

with the MOL to work on Vale property. • There is a possibility that companies may not get a response from MOL until the law becomes

effective. • Vallen has stock available. • The MOL may not agree with compliance plans.

Why Should a Pre-Op Inspection be completed?

• It’s the Law. Both Mining and Construction Regulations have provisions to ensure maintenance and inspections are performed (854-S7, 213/91-S94)

• The Operator can ensure the equipment is safe to use and will not cause personal injury to themselves or other workers

• The Operator can identify deficiencies that may cause damage to the equipment itself

Pre – Operational Inspections

• Be certain the checklist used is applicable to the specific equipment and covers all desirable components

• It should be understood that defects are to be noted as long as they remain defects

• Operators should be informed as to how defects will be addressed and when

• Operator training should include understanding when a defect requires the equipment be parked and tagged out and when it does not.

Pre – Workplace Inspections

Before operating any piece of equipment, begin with a visual check of the work area itself. • Is it Level and clear of obstructions? • Overhead hazards? (ie wires, infrastructure,

etc.) • Is there any evidence of previous fluid

leaks? • Are there other people in the area?

Follow Up

• The work ethic displayed by the Supervisor in carrying out and documenting the beginning of shift inspection reinforces the importance of it to the personnel under them.

• By observation and occasionally accompanying personnel on their

pre-op inspections, the Supervisor ensures the inspections are done properly

• If workers are fully involved in identifying and correcting defects

and unsafe conditions, (Stop&Correct’s) the importance of these functions will be understood and taken seriously.

Context

Through an assessment of the orientation process, it has become apparent that of the 18,000 Contractors that take part in Vale Orientations, almost a quarter of them never make it to site.

In addition, Vale requires that all workers on the property who either perform work or deliver material have a Lenel Access Pass. This can be one of three types of passes:

• Vale Employee Pass • Contractor Pass • Delivery Pass

The issuing of the Lenel Access Passes is through the approval of the Contractor’s Vale Contact Person

Up until now, the Lenel Access Pass has not been a prerequisite to a contractor attending a Vale Orientation.

The Change

Coming into Vale Orientations, facilitators conducting the session will be requesting evidence of prerequisite training by way of the Lenel Access Pass. Having a Pass will qualify that:

• Norcat Training successfully completed

• Contractor’s employer has been awarded work at Vale

• Contractor’s Vale Contact Person has provided initial authorization the worker to access Vale property

• Contractor’s Vale Contact Person has authorized the worker to participate in the applicable Vale Orientation

Timelines

For the time being, instructors facilitating Vale Orientations will accept either Norcat Cards or Lenel Access Passes. The deadline to have Lenel Access Passes will be:

July 4th, 2016

After this date, contractors who do not have a Lenel Access Card prior to attending a Vale Contractor Orientation will be assumed to not have the appropriate approvals to participate and will be turned away.

How do I acquire a Lenel Access Pass

In order to acquire a Lenel Access Pass….

•The contractor must have a valid NORCAT card.

•All contractors requiring a “Vale Lenel Access Pass" must either: • Fax completed forms to the Pass Office at 705-682-6584. • Email completed forms to the Pass Office at [email protected]

•Setup an appointment to have a card photo taken by calling 705-682-6380 or 705-682-6017

Why the Change?

The reasons for this change come from several different groups. Some are related to business improvement while others are due to mandatory change dictated by infrastructure changes.

Current recording systems being decommissioned: Both EQUAL and PASS systems are being decommissioned in place of more current system applications. For the purpose of recording contractor training, contractors will need a Lenel Access Profile in the system. L&D cannot record a contractor’s training unless this profile exists as a prerequisite to Orientation.

Plant Security: Contractor’s who are banned from Vale property are maintained in the Lenel system (previously PASS). Banned personnel need to be flagged before they attend Orientation Training.

Redundant Orientations: Of the 18,000 contractors that attend orientation each year, approximately 25% of them are not required. By having approval for Lenel in advance of the Orientation, redundant orientations should be reduced.

Q&A

I haven’t needed one at the plant I’ve been going to up until now… do I need one? You have required one up until now, for whatever reason though you haven’t been identified in an audit…. Yes you need one.

I have one already… do I need a new one? If you already have a Lenel Card, it means you’re already in the system, we can in turn accredit your training… so no you do not need a new card.

I only make sales visits or site tours.. Do I need a Lenel? 1. Are you participating in Vale Site Specific Orientations? Then yes. 2. Are you not participating in Orientations but rather being escorted by somebody with

Orientation in accordance with the SPI-17? In that case, no. 3. Are you not participating in Orientation nor are you being escorted? You are in

contravention of SPI-17 and need Lenel AND an Orientation.

Where do I get the forms? 1. Protection Services Webpage on the intranet 2. PASS forms through PMO Extranet 3. Directly from you Vale Contact Person 4. Calling the PASS Office and request they sent through email 5. Walk in to the PASS Office for open stock forms

Removed bad signs and replaced with new ones. Cleaned off salvageable signs.

Vale was rehabing drift on weekend and their barricade fell down- unsupported ground. Reinstalled barricade

Workers in a manlift were doing repairs on exterior of gallery with plywood, 2x6 lumber and hammer drills. While work overhead was being done, one of the groundmen crossed diagonally through the roped off area instead of walking around the exterior of the roped off area. The groundman put himself in a dangerous position in relationship to the workers working overhead.

I was observing the overhead work when this near miss occurred. Walked over to the groundman and explained the reason for having the area roped off and the possible bad scenario he had just created. Also sent email to the site superintendent of the contractor explaining the near miss and asked him to review with both crews.

Barricade was down at the transfer raise on 2910. Thought the scoop was mucking but he wasn't even on the level.
