A Continuing Approval Number is required for each grape product intended for export. To obtain a Continuing Approval Number please complete this registration form and forward to the Australian Grape and Wine Authority Compliance Division. Wine samples and labels are not required to be submitted with this application.

ABOUT THE PRODUCT REGISTRATION FORM

Exporter Id: The identification number issued to the exporter by the Australian Grape and Wine Authority. It is not mandatory to complete this item however it will assist the Authority in completing the approval process.

Exporter Name: The exporter is the entity submitting the product for continuing approval. The continuing approval number will only be issued to this entity. An entity may submit a product for continuing approval that may then be exported by another associated entity.

Product Description: The description of the product, for example, vintage, brand name, geographical indication and variety.Product Id: A unique identification number issued by the Australian Grape and Wine Authority for a blend of wine once

the product is entered in to the Wine Export Approval system. This will be entered by the Authority on the Initial application.

Blend No (if applicable): A unique identification provided by the exporter for that blend of wine.Organic/Biodynamic: Indicate here if any claim is made on the label that the product is organic, bio-dynamic, biological,

ecological or any other similar word. Organic produce requires certification under the Export Control Orders.

Value - Price per Carton (packaged) and/or Price per Litre (bulk): The price category for the sale of the product once it reaches its destination.

Certificate of Analysis: This section must be completed in full. For the purpose of granting Continuing Approval, the analysis is not required to be undertaken by a NATA accredited laboratory.

Label Description(s): This should list all the brands that are associated with this blend of wine. Please include any vintage, brand names, geographical indications and varieties listed on the label. Indicate if the label is intended for EU or Other Markets. Indicate the alcohol content quoted on the label. Product Composition: Required to validate any claims made on labels in respect to variety, geographical indication and

vintage that are subject to audit under the Label Integrity Program.

Grape Variety(s): List the variety(s) of grapes used to produce the wine and the percentage of each. Geographical Indication(s): List the geographical indication(s) where the grapes were grown and the percentage of

each. Vintage(s): List the vintage year the grapes were grown and the percentage of each.

This application is for: Indicate here if this is the first time that a Continuing Approval Number is being sought for this product (Initial) or if an approved product is to be renewed (Renewal).

Total blend quantity: Nominate the total amount (in Litres) of the blend.

Declaration: The exporter or their nominated representative must complete the declaration. By signing the declaration, the exporter declares that the matters declared on the application are true.

Package Type(s): Indicate the package type(s) that you wish to apply for Continuing Approval. (Alternative Packaging includes flagons, tetra-pak, aluminium, PET, etc).
