(P&A) began its operations in May 2004, to provide Professional Engineering and Surveying & Mapping Services to the public and private sectors in the fields of Highway and Bridge Design, Traffic Operations and Safety, Modal Development & Transportation Planning, Surveying & Right-of-Way Mapping and Construction Management & Engineering Inspection. Currently, our staff includes thirteen professional engineers, two professional traffic operations engineer, five professional surveyors and Mappers three engineers in training, three CADD technicians, three administrators and five three-man survey crews. Our staff has developed a clear understanding of the needs for cost efficient transportation solutions to improve the safety and mobility of all highway users. Our goal is to provide our clients, and the users of the facilities, with the safest/best transportation solution possible to improve the performance of the facility (highway, pedestrian, bicycle). Our approach to each project begins with the development of a clear understanding of our client needs and a thorough scope of work which allows us to complete each project meeting our client’s expectations. Our HIGHWAY DESIGN group includes professionals with over thirty years of experience in highway design, having performed work for several Florida Department of Transportation District offices in the State as well as Miami-Dade’s Expressway Authority and Miami-Dade County’s departments of Public Works, Transit and Traffic Division ranging in complexity from minor pushbutton design improvements to RRR improvements to minor reconstruction projects to total reconstruction projects to new construction projects. Our STRUCTURAL AND BRIDGE DESIGN group includes professionals with over thirty-seven years of experience in structural and bridge design, having performed structural related work for multiple clients including the Florida Department of Transportation, Miami-Dade’s Expressway Authority and Miami-Dade County’s departments of Public Works, Transit and Traffic Division. Our TRAFFIC OPERATIONS group includes professionals with up to twenty eight years of experience in the field whom are well known by the Florida traffic safety and operations community. Their experience ranges from level of service analysis to complex access management studies to detailed modeling of roadway networks. We provide full traffic data collection services to support our traffic operations and safety work. We have provided office and in-house support to District VI Public Transportation Office (PTO) office in all areas related to Rail, Transit, and Intermodal services including representing the Department at meetings such as the Annual Railroad Coordinators meetings. Our SURVEYING AND RIGHT OF WAY MAPPING group includes professionals with up to fifty-one years of experience in the field and whose experience includes all types of surveys related to highway improvements as well as all related work to the development of right-of-way mapping deliverables. We use state of the art total stations and GPS data collectors and maintain ourselves up-to-date with the latest versions of software. Our CONSTRUCTION MANAGEMENT SERVICES group includes professionals with up to 28 years of experience in the Transportation Engineering field, specializing in project management, construction engineering, inspection and management (CE&I), as well as maintenance of traffic plans design. Staff consist of well rounded and a uniquely qualified individuals having worked in the CE&I field for the state and consultants, and for contractors. SR 823 / NW 57 Ave / Red Road Reconstruction from 46 St to 84 St Client: Florida Department of Transportation Status: Ongoing District 6 Consultant Management Office Ref: Daniel Iglesias, P.E. Phone: (305)470-5289 Develop all components of the construction plans for the widening of this corridor limits including the reconfiguration of the typical section to accommodate improvements considering the existing canal on the west side of the corridor and right-of-way restrictions. Components include: roadway including drainage, bridge structures, signing and marking signalization, and lighting General Engineering Consultant – Roadway Design Client: HNTB Corporation (GEC-A) Status: Ongoing Miami-Dade Expressway Authority (MDX) Ref: Albert Sosa, P.E. Phone: (305)551-8100 Developed the conceptual plans for: • SR 836 / Dolphin Expressway (83629.100): this project included the development of 30% construction plans to be used by MDX to advertise a design-build project to redesign the westbound connection to SR 826 via a new CD system and improve operations and capacity on the eastbound on and off ramps to NW 87th Avenue Our construction plans included the realignment of the corridor, a new westbound bridge over NW 87 Avenue, a new flyover from NW 12 ST to westbound SR 836 amongst other improvements. • SR 924 / Gratigny Expressway (92405.100): this project included the development of 30% construction plans to be used by MDX to advertise a design-build project for the removal of the existing toll facility along the Gratigny Expressway to implement Open Road Tolling. Our construction plans included the realignment of the corridor in the approaches to the existing toll facility, the evaluation of existing right-of-way for the realignment and the computation of quantities to develop a construction cost estimate for the project. Construction Plans included: Roadway Plans and Signing & Marking Plans. • SR 874 / Don Shula Expressway (87409.100): this project included the development of 30% construction plans to be used by MDX to advertise a design-build project for widening of the corridor between the Palmetto Expressway and Kendall Drive / SW 88 ST. Three design alternatives were prepared including one that would accommodate future Express Managed Lanes. We performed survey work, traffic analysis and developed plans which included the roadway and signing & pavement marking components, along with a preliminary maintenance of traffic. • SR 924 / Gratigny Expressway (92404.100): this project included the development of construction plans to reconfigure the Eastbound off-ramp to NW 57 AVE. Our construction plans included the addition of an auxiliary channelized right-turn-lane, the realignment of the southbound

Work included embankment calculations, cross-sections, relocation of lighting fixtures, resetting of guardrail and drainage analysis. • SR 836 / Dolphin Expressway (83614.100): this project included the development of 60% construction plans to convert the existing outside shoulder between East of 87 AVE and NW 72 AVE to a thru lane in order to achieve thru-lane balance along the corridor. The work included roadway plans, cross sections, embankment reinforcement and earthwork calculations, signing and pavement marking analysis extending to SR-836 / Palmetto Expressway and maintenance of traffic. NW 37th Ave - Partial Reconstruction Client: A&P Consulting Transportation Eng. (Prime) Status: Ongoing Miami-Dade County Public Works – E04-PW-07 Ref: Eithel M. Sierra, P.E. Phone: (305)592-7283 Developed construction plans for the signing & pavement markings and signalization components to reconstruct NW 37th Avenue between South River Drive and NW 72nd Street. Plans adhered to all County Standards and we received minor comments from the County after phase submittals. SR-7 at Golden Glades Interchange Ramps Reconstruction Client: Florida Department of Transportation Status: Ongoing District 6 Consultant Management Office Ref: Ali R. Toghiani, P.E. Ninety-six work orders were completed through this contract including a wide range of minor design projects as well as two projects with a construction cost of over $1.5 Million. A brief description of these projects is provided below: • SR 860 / Miami Gardens Drive at NW 87th Avenue: this project included widening to an additional westbound lane through the intersection, and additional turn lanes. This project also included signalization, and minor drainage improvements. • SR 989 / SW 112 Ave from SW 220 St. to SW 224 St: this project included the installation of a new traffic signal at SW 224 Street, and new lighting and drainage system through the limits of the project.

(305)551-8100 Responsible for preparing crews for field work to collect traffic count information such as turning movement counts, bi-directional hose counts, vehicular travel time & delay, and intersection delay studies. In addition, we are providing traffic engineering services such as traffic forecasting, signal warrant studies, PD&E Studies, Concept Studies, SYNCHRO analysis, VISSIM simulation, intersection analysis, etc. District-wide Traffic Operations and Safety Studies Client: Florida Department of Transportation Status: Ongoing District VI – Traffic Operations Office Ref: Miguel Cardera (305)470-5323 This contract includes the district-wide evaluation of High Crash Locations. These evaluations include crash data reduction, crash analysis to identify crash patterns/probable causes, detailed evaluation of existing conditions including field reviews to verify the probable causes, developing/evaluating alternative improvement concepts to remedy the crash patterns. Perform all the data collection required for this contract District-wide Traffic Operations and Safety Studies Client: Florida Department of Transportation Status: 100% District VI – Traffic Operations Office Ref: Misleidys Leon, E.I. (305)470-5345 This contract includes the district-wide evaluation of High Crash Locations. These evaluations are performed through three study types. Study Type 1 (Level one analysis) includes crash data reduction and crash analysis to identify crash patterns and crash probable causes. In 1995, Carlos registered A&P Consulting Transportation Engineers with the Florida Department of State and in 1997 with the Florida Board of Professional Engineers to provide roadway design services to the department and by May 2004 the company grew from four to thirty-four engineers/employees. In May 2004, Carlos opened C H Perez & Associates (P&A) to have better control over the disciplines that the company would provide services on. The project entailed the redesign of one interchange, the reconstruction of three bridges and the widening of two additional bridges.

Project Experience with C3TS Jan 2006 – Sep 2010 US1 / SR 5 / 18 Mile Stretch, Miami-Dade County, FL 8/2006 to 10/2007 Assistant Project Manager for reconstruction of an existing 2-lane undivided roadway to a 2-lane divided facility. This corridor is part of the 18-mile stretch linking Florida City to the Florida Keys. The improvements include a new 14-ft wide median with concrete barrier wall designed to eliminate head-on crashes. In addition, there is a 1-mile section where passing lanes are being provided. SW 8th Street Interchange Modifications at the Florida Turnpike (Florida Turnpike Enterprise) 5/2004 to 11/2005 Assistant Project Manager in the development of Construction Documents for a new southbound HEFT exit ramp to westbound SW 8th Street in order to alleviate the mainline weave condition that results from the heavy movement between the southbound HEFT entry from westbound SR-836 traffic crossing the southbound HEFT existing to SW 8th Street.
