1.0 EXECUTIVE SUMMARY Contractor entered into a contract agreement to construct the Insert Program, Polk Army Airfield (insert type) Project as part of the ID/IQ Contract administered through the MICC. This Task Order was awarded by letter dated Insert Date. In accordance with the plans and specifications for this project, FAA Advisory Circular 150/5370-2F will be followed as applicable.

This Construction Safety Phasing Plan (CSPP) has been prepared in conformance with U.S. Department of Transportation Federal Aviation Administration (FAA) Advisory Circular 150/5370-2F, “Operational Safety on Airports During Construction”. It addresses airport operational activities during construction, specifically at the Polk Army Airfield.

In order to facilitate the work, this Plan will be developed and approved in Phases. In addition, if circumstances change after approval of the Plan, the CSPP will be changed and approved accordingly.

Compliance with this CSPP is required for all UN-NAMED personnel, third parties, and subcontractors that participate in construction activities. UN-NAMED will exercise the responsibility for implementing this CSPP through the Project Manger; the Site Superintendent; and the Project Engineer via the Safety Plan Compliance Document (SPCD) provided by UN-NAMED. To that end UN-NAMED will:

1. Submit a SPCD as required.

2. Maintain an approved copy of the CSPP and SPCD in the Site Job Office at all times.

3. Comply with all aspects of the CSPP and SPCD.

4. Designate a Point of Contact who will be available to coordinate immediate response to correct any construction-related safety issues.

5. Provide workers who are familiar with the CSPP and SPCD; and supervisors who will continually monitor construction activity.

6. Restrict movement of construction vehicles by appropriate flags and barricades.

7. Provide escorts as required by regulations.

8. Ensure that no construction personnel enter any Air Operational Areas.

1.1 APPROVALS

The signatures on the approval page at the front of this document, certifies that the PE, PM, and SS have approved the plan for use during construction activities. Addenda or changes will require written approval by the Contracting Officer.

1.2 REGULATORY DRIVERS

Key parties for this plan include the MICC, DPW, FAA, and UN-NAMED.

If a contract modification includes an activity not already addressed in this CSPP, the CSPP will be re-evaluated and revised if necessary to ensure health and safety compliance related to that activity. This will be accomplished before the start of the changed activity.

1.3 PENALTIES

Penalties for non-compliance with airport rules, regulations and this CSPP may result in rescission of driving privileges or access to the AOA. Examples include: entering a movement area without permission or communications with air traffic control; failure to yield the right of way to an aircraft; driving to close to a parked aircraft; taking pictures of the airfield, personnel or equipment.

2.0 INTRODUCTION The UN-NAMED ID/IQ Contract # 12345 provides a vehicle for the Army to accomplish multiple small-scope construction projects using a single contractor. The contract Task Order that necessitates this CSPP is 34568 with a scope to (Describe work) at the Polk Army Airfield. The Task Order has an anticipated time duration of ? months from DEC 1 to FEB 1.

3.0 HEALTH AND SAFETY PERSONNEL UN-NAMED shall provide the site health and safety staff for this project in the following table and describe their functions in the following paragraphs.

3.2 PROJECT MANAGER

Un-named will be the PM for this project. He has the overall project management responsibility. Un-named has the responsibility to ensure that the CSPP and SPCD meets or exceeds FAA Advisory Circular 150/5370-2F. He will ensure that adequate resources are provided to the construction staff to carry out their responsibilities as outlined in this CSPP and SPCD. He will also ensure that adequate personnel and equipment resources are available to complete the project safely.

3.3 SITE SUPERINTENDENT

Un-named will be the Site Superintendent for this work. He will ensure that all site work is performed in accordance with contract requirements and in full compliance with the specifications and with this CSPP. The SS is responsible for implementing this CSPP and initiating changes to the SPCD with the concurrence of the PM and Project Engineer (PE). The SS has the authority to stop work if the CSPP or SPCD is compromised. The SS will also serve as the site emergency coordinator. The SS will conduct a daily walk-through of the site to ensure CSPP and SPCD compliance. Any discrepancies noted and corrective actions taken will be documented in a Daily Quality Control (QC) Report, which will be found in the approved Quality Control Plan. He has authority to stop work when it is determined that existing work conditions are unsafe.

3.4 QUALITY CONTROL MANAGER

Un-named will be the Quality Control Manager. He will be responsible for quality control during the project.

3.5 QUALITY CONTROL SYSTEMS MANAGER

Un-named will be the Quality Control Systems Manager for this project. He shall be responsible for overall management of CQC and have the authority to act in all CQC matters for the contractor.

3.6 SITE HEALTH AND SAFETY OFFICER

Un-named will be the designated Health and Safety Officer for the project. He will be responsible to monitor all activities to ensure that all work is accomplished in accordance with the Corps of Engineers Safety Manual.

3.7 SITE WORKERS

All site workers must be familiar with and comply with the terms and conditions of this CSPP, SPCD as well as all Airport and Base Safety and Security Regulations. All site workers shall receive site-specific safety training from the airfield safety program manager and sign the CSPP and SPCD Field Team Review Roster before beginning work. All site workers have the responsibility to immediately report any unsafe or potentially hazardous condition to the SS, stop work if an accident or injury is foreseen, and/or correct any work or safety hazard.

In addition, all drivers who will operate vehicles or construction equipment on Polk Army Airfield must pass a Flightline Driving Test prior to being able to operate a vehicle on the Airfield proper. UN-NAMED will ensure all potential drivers take this test and that records of passing the test are maintained with the airfield operations officer who will issue temporary flightline licenses which shall be maintained on their person while operating on the airfield.

4.0 PROJECT DESCRIPTION AND WORK PLAN This is a taxiway and parking painting project. Figure 1 shows the location of the work. The Airport will NOT be closed/restricted during construction activity; however, UN-NAMED will have exclusive access to a Work Area designated in Figure 2. To further ensure safety, a portion of Taxiway A will be closed to all Aircraft and Base vehicles for the duration of this work. Figure 2 also delineates the parts of Taxiway A that will be closed. Airfield operations will issue a NOTAM to announce this closure before construction begins.

The Airport Construction Project will:

1. Install improved.

2. Construct.

3. Repair and reinstall.

Accident prevention activities occur throughout every phase of this project and include inspections and employee training. Anticipated field activities for this project are listed below:

4.1.1 Notification Of Construction Activities to the FAA

UN-NAMED shall file FAA Form 7460-1 with the FAA per 14 CFR part 77. 4.1.2 Duration of Each Phase

UN-NAMED shall provide the anticipated durations of each phase in the SPCD. 4.1.3 Daily Work Start Time and Finish Time to Include “Night Only” Projects

UN-NAMED shall provide the anticipated daily start and finish work times in the SPCD. Night only projects would still require this entry. 4.1.4 Aircraft Operational Areas Affected by Construction

• Taxiway A between the Fire Station and Hanger 4295 will be closed to all base vehicle and aircraft traffic for the duration of the work. An existing 25 foot wide Fire Lane will be delineated and maintained clear during all phases of work.

• Existing (Abandoned) Helicopter Landing Pads adjacent to the North side of the project will be closed to air operations for the duration of the project.

4.1.5 NAVAIDS Potentially Affected by Construction

There are no NAVAIDS affected by this work. 4.2 SPECIAL PRECAUTIONS

4.2.1 Runway and Taxiway Marking and Lighting

This is project is outside the Runway Safety Area and will not impact the existing runway or aircraft takeoff or landings. Taxiway Lighting for Taxiway A, within the Project Site, will be out of service for the duration of work. These lights will be removed and reinstalled at the completion of work.

4.2.2 Protection of Runway and Taxiway Safety Areas (RSAs and TSAs)

UN-NAMED will protect the integrity of all appropriate RSAs and TSAs by means of barricades and Orange Construction Silt Fencing. UN-NAMED will isolate the Work Site from existing Taxiways using Multi Barrier, Model AR-10, HDPE Impact-Resistant, Water-Filled, UV Resistant Safety Barricades. These barricades meet the requirements of FAA Circular 150/5370-2F and will be equipped with Red Caution lights. These barriers are shown in Appendix B. 4.2.3 Protection of Obstacle-Free Zones (OFZs), Object-Free Areas (OFAs) and Mandatory Frangibility Zones (MFZs)

This Project will have no impact on Obstacle-Free Zones, Object-Free Areas or Mandatory Frangibility Zones. 4.2.4 Foreign Object Debris (FOD) Control

UN-NAMED will not contribute to FOD contamination of the taxiways. FOD Control will be included in the general training given to all employees on site. Haul Routes within the site, and shown in Figure 2, will be developed to minimize FOD and to keep vehicles and materials as far as possible from existing Taxiways. The SS and Foremen will continually inspect the work during operations and each Foreman will walk the site each day before ceasing operations. UN-NAMED shall list within the SPCD equipment on site for control of FOD i.e. tarps for dumpsters, tractor with brush, and scheduled usage. 4.2.5 Wildlife Management

UN-NAMED must carefully control and continuously remove waste or loose materials that might attract wildlife. Contractor personnel must be aware of and avoid construction activities that can create wildlife hazards on airports, such as:

a. Trash. Food scraps must be collected from construction personnel activity. b. Standing Water. c. Tall Grass and Seeds. Requirements for turf establishment can be at odds with

requirements for wildlife control. Grass seed is attractive to birds. Lower quality seed mixtures can contain seeds of plants (such as clover) that attract larger wildlife. Seeding should comply with the guidance in AC 150/5370-10, Standards for Specifying Construction of Airports, Item T-901, Seeding.

4.2.6 Security Fence Integrity

UN-NAMED shall notify airfield management if the integrity of the security fence will be violated and if so what measures will be taken to maintain security. 4.3 CONSTRUCTION PRECAUTIONS

4.3.1 Traffic Control

This CSPP constitutes the approved Traffic Control Plan for the Task Order. All traffic on the construction site, including construction equipment and support vehicles will be strictly controlled at all times. At no time will any vehicle be allowed to enter any Restricted Area, including the Work Site, without approval, proper two-way communication, and escort. All UN-NAMED and subcontractor vehicles will be prominently marked to ensure identification. All Construction vehicles will be inspected daily to ensure all safety features are operational. All vehicles will utilize hazard/warning lights as required by Regulations. UN-NAMED will control access to and egress from the Construction Zone by means of controlling the entry gate so that no vehicle can enter the site without permission. As noted in Section 3.6, all drivers operating any type of vehicle in the Work Area will be required to pass a Flightline Driving Test. 4.3.2 Area Delineation

The Construction site will be marked and separated from the remaining airport areas by means of approved (lighted) barricades as noted in Section 4.2.2. UN-NAMED will inspect the Site Perimeter on a daily basis and maintain positive control of all perimeter barriers. 4.3.3 Special Construction Limitations – PHASE 1

Phase 1 of the Work defines the Site Investigation. The construction barrier system will not be in place during this Phase of work. In Phase 1, three specific work scopes will be accomplished:

1. Geotechnical Crews will excavate Test Pits and Drill Bore Holes for geotechnical evaluation. Drill rigs, which may reach 100 feet into the air, will be on site for 1 Day. The Testing contractor will use a backhoe to excavate Test Pits. Test Pit work will take two (2) days.

2. Environmental Testing Crews will sample soils. This work will take two (2) days and will require a pick-up truck.

3. Survey Crew will perform a survey of the work area. This work will take three (3) days and will require a pick-up truck.

In order to manage this work, UN-NAMED will do the following:

• Ensure Orientation training is conducted prior to accessing the airfield. • Delineate the boundaries and corners of the Work Area using construction cones. • Provide full-time on-site supervision while any aspect of this work is ongoing. • Ensure all vehicle drivers have passed the Polk Airfield Flightline Driving Test. • Ensure that all vehicles and personnel are kept under constant visual control. • Ensure that no one will be allowed to leave the Work Area without escort.

There are no special limitations of construction in Phases 2, 3, or 4 of this contract and the terms of this CSP will govern operations. 4.3.4 Stockpile Locations

UN-NAMED shall include approved stockpile locations are designated on attached Figure 2. To avoid dust and FOD, all stockpiles will be covered with plastic at all times when not in continual use. 4.3.5 Hazardous Materials Management (Spill Response)

UN-NAMED shall notify airfield safety of the presence of hazardous materials on the airfield and insure proper procedures are used and applicable documents (SDS) are present at the job site. Spill response shall be address in UN-NAMEDs SPCD. 4.3.6 Trench and Excavation Identification and Cover

No Trenches are anticipated in any areas other than the Work Site. All trenches on the site will be promptly backfilled and covered, and identified by construction tape if they present a depression of more than 4 inches. All trenches and excavations in any traffic areas (Work Site Haul Routes) will be covered at the end of each work shift. 4.3.7 Erosion Control

Soil erosion must be controlled to maintain RSA standards, that is, the RSA must be cleared and graded and have no potentially hazardous ruts, humps, depressions, or other surface variations, and capable, under dry conditions, of supporting aircraft rescue and firefighting equipment, and the occasional passage of aircraft without causing structural damage to the aircraft. 4.3.8 Special Inspection Requirements

The Site Superintendent will inspect the work site at the beginning and end of each work shift to ensure compliance with the CSPP and SPCD. UN-NAMED will develop and use a Checklist to facilitate these inspections and this Checklist will be included in the Daily Reports and maintained as part of the contract record.

4.3.9 Utility Location and Protection

The utilities potentially impacted by these operations are electricity for Taxiway A lights on the south side of the project, and an underground electrical power line on the far north side of the project. The Army has performed a Utility-Locate prior to the start work. All electrical runs will be prominently marked on the surface. Where the location of any utility is unclear, hand-excavations will be performed if there is any danger the excavation will impact the utility. UN-NAMED will note the location of all utility runs on As-Built Drawings. In the event of an unplanned utility outage, UN-NAMED will immediately contact the Airfield Flight Operations Office and Project Manager, and will begin repair actions as instructed. 4.3.10 Use of Temporary Visual Aids

No Temporary closed runway markers will be required for this project.

5.0 COMMUNICATIONS 5.1 NOTAM ISSUANCE

Only flight operations personnel will issue NOTAMs. UN-NAMED shall keep airfield operations manager updated on the progress of the project to ensure proper notification is given to airfield users through the NOTAM system. 5.2 ACTIVE AND PASSIVE COMMUNICATIONS

UN-NAMED shall provide company communication specifics in the SPCD.

All vehicles and each crew foreman will be equipped with 2-way radio communications, tuned to the approved frequency. The SS will continually monitor the Airport Operational and Emergency Frequencies.

The SS will be monitoring the designated radio frequencies for aircraft at all times during construction activities.

5.3 EMERGENCY NOTIFICATION PROCEDURES

In the event of a Medical Emergency on site, UN-NAMED will exercise the appropriate requirements of the Health and Safety Plan (HASP).

In the event of a non-Medical Emergency, UN-NAMED will stop all work and call a general evacuation of the site. Simultaneously, the SS or PM will notify the Project Engineer, Airfield Safety Office and Control Tower relating the nature of the emergency and the ongoing response.

6.0 TRAINING AND RECORD KEEPING Safety training for this project includes an initial project safety meeting, site-specific safety training, and Daily Safety Tailgate Briefings. The SS or designee will provide and document all training.

6.1 INITIAL SAFETY ORIENTATION

Before beginning field work, employees, subcontractors, and visitors will read the CSPP and SPCD and sign a Team Review Roster. Individuals refusing to sign the agreement may be prohibited from working at the site.

6.2 SITE-SPECIFIC TRAINING

No drivers can operate a vehicle without first passing the Polk Army Airfield Vehicle test as noted in Section 3.3 of this CSPP.

No site workers will be allowed to begin work onsite until the site-specific training is completed and documented by the SS on the CSPP and SPCD Field Team Review Roster, provided as Appendix A. This training will address this CSPP and all health and safety issues and procedures pertinent to site operations. As part of the site-specific training, the following topics will be covered:

• Radio Communications

• Scope of Work

• Vehicle Identification

6.3 SAFETY BRIEFINGS

UN-NAMED shall provide company requirements for safety briefings within the SPCD.

6.4 VISUAL COMMUNICATIONS

Construction equipment will be fitted with an amber strobe and will have two-way radio communication capabilities.

6.5 EVACUATION

UN-NAMED shall provide, in the SPCD, their procedures in the event of an emergency situation where evacuation is required:

• Sound three blasts with an air horn;

• Solicit the aid of the SS, or the nearest employee. Have that individual call for help. Notify the SS, if onsite, and give the exact location of the evacuated area. Direct all personnel in the affected area to evacuate and assemble upwind in a designated safe area;

• Establish the safety of all personnel and direct the administration of first aid as appropriate;

• Shut down all combustion equipment;

• Provide emergency equipment as appropriate; and

• Notify SS, or on-site representative.
