Treatment protocol

-starting dosage: 15 mg/kg/d (20 mg/kg/d for children)

-increase by 5 mg/kg/d every 8 wks;

until mild myelosuppression or max 35 mg/kg/d

-maintain neutrophils >2,000/µl and platelets >80,000/µl
