To print of more of these leaflets you must:-
Go to ‘FILE’ and the print option – Ensure to action the following before clicking main print icon.
1. Under Settings – In the dialogue box under Print, choose ‘custom range’ and then type in 2,3‘ (This will then only print pages 2 & 3)
2. Choose ‘Print on both sides flip on short edge’
3. Ensure paper orientation is set to ‘portrait’

Your Responsibility to UsLet us know if you change your
name, address or telephone number. Do not miss booked
appointments. Please be aware that the phone lines will be busy during the early morning; please call after
2pm regarding test results and other non-urgent matters. Test results
take time to reach us, so please do
not ring before you have been asked to do so. Enquiries about tests
ordered by the hospital should be directed to the hospital, not the
practice. Take responsibility for your own health and for that of your
children
Violent and Abusive PatientsThe NHS operates a zero tolerance policy with regard to violence and
abuse. The practice has the right to remove violent patients from the
patient list with immediate effect, in order to safeguard practice staff,
patients and other persons. Violence in this context includes actual or threatened physical behaviour or
verbal abuse which leads to fear of a person’s safety
Repeat PrescriptionsPatients on regular medicines
may request them on a monthly basis without the need
to be seen. The doctor will discuss with you how often you need a doctor or nurse check-
up depending on your individual circumstances. Your
medication will go on a computerised request list that will be printed for you. Please mark what you need and leave the slip at the surgery at least two working days before you
require your medication.
You can now also order your repeat medications online, please enquire at reception
regarding patient online access. This only applies to the items that appear on your repeat
request slip.Please note that requests for
repeat medications will not be taken over the telephone
Any requests put through the letter box will not be picked up
until the next working day. Therefore any repeat
prescription placed through the letter box over a normal
weekend will not be ready until the Wednesday
The Practice Staff
Nurses/HCAs
The practice is fortunate enough to have an experienced team of Nurses. They perform immunisations, dressings and give advice on preventative care. They run clinics to help
our patients with asthma, diabetes, blood pressure,
cholesterol, weight management and smoking
cessation advice and support. They also provide travel advice
and perform travel immunisations. The health care
assistant assists the practice nurses with routine work such as taking blood, heart tracings
and some vaccinations.
Administration
Mrs Georgina Cooke is the Practice Manager and runs the
administrative side of the practice. If you are concerned about any
aspects of the care you receive in the practice, or have any
suggestions, please contact the Practice Manager to arrange an
appointment to discuss your suggestions/concerns
Community Team
We are supplied in the community by a large team of district nurses, a community matron, health visitors and Macmillan nurses. Should you
require a visit by a community nurse please contact the surgery who will
arrange it for you.These staff are not based or employed by the practice.
Care Data Extraction
Under the Health and Social Care Act 2012. NHS England has the
power to direct the HSCIC to collect information from all providers of NHS care. The General Practice Extraction Service (GPES) will be
used to extract GP data each month. You have the right to prevent
confidential information about you from being shared or used for any purpose other than providing your
care. If you do not want information
that identifies you to be shared outside your GP practice, please visit your surgery to make a note of your
request in your medical record.
Access to InformationAll patients are entitled under the Data Protection Act (1998) and the Access to Health Records Act 1990 to check any data
concerning them, held in their medical records and on computer. Any requests should be made in writing to the
Practice Manager. Copies of the records can be provided for a small charge. We are registered with the Data Protection
Agency. If you have any questions regarding Data Protection, please contact the Practice Manager.
Confidentiality/ Access To Patient DataConfidential patient data will be shared within the practice health
care team and with other care professional’s to whom you are referred for care. Confidential patient data may also be required for
the broader purpose of public health, the provision of health care services, teaching and training. Data disclosed will be kept to the
minimum required to serve the purpose and if possible will be anonymised before disclosure. Confidential and identifiable patient information will not be disclosed otherwise without explicit consent
unless:1. It is a matter of life and death or serious harm to you or to
another individual.2. It is overwhelmingly in the public interest to do so.3. There is a legal obligation to do so.
In all of these circumstances only the minimum identifiable information may be revealed to someone with a legal entitlement to
access the data for that purpose.All individuals with access to your data have a professional and/or contractual duty of confidentiality. This means that all staff at the
practice are aware of the importance and legal implications of maintaining confidentiality for all patients. Anything you discuss with a member of staff be that a doctor, nurse or receptionist, will remain
confidential. If you are concerned about any of the ways in which your confidential data is accessed, further information is available
from the Practice Manager. You are entitled to register an objection, which will be respected if this is possible.
Freedom of InformationThe Freedom of Information Act 2000 obliges the practice to
produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice makes routinely available on
written request from our practice manager.
