Introduction This document is a composite Action Plan for both the Peak Community & Consumer Participation Council (PCCPC) and the Community Participation Unit of the NSLHD. The Terms of Reference for the Peak Community & Consumer Participation Council state that ‘the PCCPC will promote, monitor and report to the Board on how NSLHD is increasing ‘consumer and carer involvement in

decision making’ by engaging with community stakeholders and adopting care planning and service delivery models that engage the patient as integral to the care team’s planning and delivery of care’. The main guiding documents and plans that inform community participation in Northern Sydney Local Health District are the NSLHD Strategic Plan 2012-2016 and the Clinical Services Plan 2012-2016; National Safety & Quality Health Service Standards - Standard 2 -

Partnering with Consumers, and the Clinical Excellence Commission’s Patient Based Care Challenge. The Action Plan is organised around Standard 2 - Partnering with Consumers to reinforce the adoption of the national standards. The PCCPC is also keen to ensure that the situation described in the Francis Report in relation to Mid-Staffordshire Health Trust should not occur in NSLHD. The PCCPC supports and advocates for initiatives, infrastructure and models of care that enable community, consumer and carer participation in care processes and health service governance eg. In Safe Hands, health literacy, Carer Support, Between the Flags and REACH (Recognise, Engage, Act, Call, Help is on its way).

1. GOVERNANCE 1.1 Establish, maintain and publicise governance structures to facilitate partnerships with the community, consumers and carers

Ensure community members, consumers and carers are involved in the governance of the LHD. Establish participatory partnerships with individuals and community groups that reflect the diverse range of backgrounds in the population served by the LHD. Include patient experiences & stories at the beginning of senior service meetings (CEC 1.1)

Spend more than 25% of the board’s meeting on quality (CEC 1.2)

3.2 Staff - Implementing training for clinical leaders, senior management and the workforce on the value of, and ways to facilitate consumer engagement and how to create, sustain and develop partnerships. (NSQHS)

6. REPORTING & ANALYSING QUALITY & SAFETY 6.1 Inform community, consumers and carers about the NSLHD’s safety and quality performance in a format that can be understood and interpreted independently. (NSQHS)

Request PCCPC and CPC advice on methods for distributing information to community, consumers and carers regarding safety and quality performance. Provide Health Service performance data at each CPC meeting. Develop a consumer friendly method (dashboard) to share performance information about quality & safety with our community.
