    General Position Description

    Under the supervision of the Manager of Nursing Services, the nurse will provide direct patient care,

    engaging in education and consultation. The nurse will seek to promote, maintain and restore the health ofour patients. The nurse will be committed to working with the underserved.

    Duties and Responsibilities

    Provide direct patient care through patient education and guidance regarding medications, lifestyle

    modifications, and treatment plan

    Make preliminary assessment of clients health problems based on medical history and diagnosticresults and prioritize for medical attention

    Review and provide follow up for lab test results

    Actively participate in quality improvement projects with a particular focus on mens health, sexualhealth, and colorectal health

    Collaborate with clinical and non-clinical staff on patient plans of care

    Ensure quality standards are met

    Serve as a preceptor to nursing students and volunteer nurses

    Document patient care using an electronic medical record (EMR)

    Maintain data on patient outcomes

    Triage urgent patient care concerns as needed

    Qualifications

    Candidates for this position should meet the following qualifications:

    Registered Nurse in the state of Illinois, BSN or higher degree required.

    Minimum of one year of nursing experience preferred.

    Provide excellent quality of care in overall clinical and patient educational needs.

    Bilingual (English/Polish) strongly preferred.

    Full-time position with flexible hours. Some evenings and Saturdays are required.

    Excellent communication and follow-through skills.

    Good computer skills

    Must be a self starter, ability to work autonomously Must be flexible and have the ability to multitask
