Welcome to the Continence Nursing Service. The team is: Helen Notley, Continence Specialist Nurse Anne Weale, Continence Specialist Nurse Sarah Lawrence, Continence Staff Nurse Teresa Wollerton, Continence Staff Nurse Susan Howes, Continence Health Care Assistant (HCA) Tracey Palmer HCA The service aims to provide good quality integrated continence care as outlined in Good Practice in Continence Services (DOH 2000). The service aims to provide a local, accessible service to reduce the incidence of bladder and bowel dysfunction by the provision of comprehensive and effective assessment, treatment, advice and review. Products for the containment of incontinence can only be provided following an assessment using the approved tool and documentation. Interventions to promote continence should be tried first where appropriate. The nurse may ask you about any medication you are taking so please bring a list with you. You may be asked to complete a fluid, bowel or food diary. This will be given or sent to you if required. If you need help to complete please ask the nurses. You may be asked to obtain a urine sample for the nurse, please place it in a sealed container. The urine sample should be fresh, samples more than 4 hours old can produce inaccurate results. Consent Consent, it is your choice When a nurse asks you to agree to any form of examination, treatment or care, REMEMBER, you have a choice. You are always free to say NO, or to ask for more information before you make up your mind. Before the nurse examines you they must ask for your consent or permission. You are free to request a chaperone during treatment. Clinics Clinics are held in several sites around the Great Yarmouth and Waveney area. Patients are given the choice where they would prefer to be seen. Regular clinics are held at: Nelson Medical Practice, Great Yarmouth Shrublands, Gorleston Patrick Stead Hospital, Halesworth Beccles Hospital Sole BayHealth Centre Martham Health Centre Kirkley Health Centre Bungay Medical Centre On arrival at the clinic, if you prefer not to say which clinic you are attending, please present your letter or this leaflet and you will be directed to the appropriate waiting area. Patients will receive a comprehensive assessment of their needs. An individualised programme of care will then be discussed and agreed. A treatment or management plan will then be implemented. Diagnostic assessments including bladder scanning, physical examination and uroflometry may be carried out. If a patient is housebound and unable to attend a clinic the continence nurses will carry out a home visit. Patients will be reassessed based on their clinical need. All patients on the home delivery pad service will be reviewed each year.
