Two-question format REQUIRED: Is respondent Hispanic/Latino? Y/N (REFERED TO AS ETHNICITY QUESTION) Respondent selects one or more races: (REFERED TO AS RACE QUESTION)

American Indian or Alaska Native Asian Black or African American Native Hawaiian or Other Pacific Islander White

NOTE: Institutions may expand categories If expanded must be able to rollup into required base categories

Guidelines for Collection Questions

Ethnicity question MUST come first Race selection question MUST read “one or

more” instead of “all that apply” Cannot have Hispanic as part of the Race

question Cannot have Ethnicity question after Race

question Cannot indicate they do not have to answer race

question if Hispanic is Yes
