Strategies to Enhance Patient CareIn 2017, CentraState was one of only 12 hospitals in New Jersey selected to participate in a New Jersey Hospital Association initiative to create high reliability organizations (HROs). The intent is to instill a distinct culture of safety—adopting and cultivating shared values and beliefs about how we act and interact—so that we can build upon our successes and share best practices in safety and quality. This structured focus on error prevention is an important pathway to ensuring that we are providing the best patient experience possible.

We are also in the third year of a five-year pilot project with the Centers for Medicare & Medicaid Services (CMS) to examine reducing the amount of time patients need to spend in the hospital following a total hip or knee replacement procedure. Research shows that earlier mobility contributes to healing following these surgeries. Using a coordinated care approach, clinicians are working to help patients get up and move soon after surgery so they are able to finish their recovery at home or in a rehabilitation center. To date, we have succeeded in significantly reducing length of stay, scored well in quality outcomes, and earned an “excellent” rating from the state—all while ensuring that patients are receiving the comprehensive care they need.

CENTRASTATE GOES SOLAR

CentraState is a leader in leveraging the power of the sun and advances in technology to

save energy, cut costs, and be environmentally responsible. In fact, the hospital has developed a 7-megawatt solar farm that now supplies up

to 30 percent of the electricity it needs.

People throughout the community used CentraState’s LIVE LIFE WELLSM services more than 41,000 times to help them improve their quality of life and manage or prevent chronic conditions.

Health coaches at CentraState provided more than 1,500 sessions of individualized health guidance.

A total of 17,500 people attended CentraState health fairs and screenings, including more than 1,800 seniors.

About 2,000 people benefited from nearly a dozen distinct CentraState support groups.

The Family Medicine Residency Program, which provides medical care to those unable to pay for their care or who pay a rate that is below market value, served nearly 15,000 patients. This program will be relocating early next year to the new

Freehold Family Health Center, a Federally Qualified Health Center, at 597 Park Avenue in Freehold, where patients will continue to have access to primary and specialty care physicians along with a variety of support services.

Surgeons at CentraState performed nearly 400 minimally invasive robotic surgeries.

CentraState provided more than 3,600 maternity and well-baby sessions, helping new and future parents ensure the healthiest beginnings for the newest members of our community.

Nearly 600 people attended health lectures and screenings focused on cancer and its prevention.

More than 57,000 patients received rehabilitation services at OceanFirst Rehabilitation Center.

CentraState educated more than 5,000 students about health issues and health care as a profession.

Nearly 600 people resided at CentraState’s three senior living facilities, with 900 people receiving rehabilitation services at The Manor Health and Rehabilitation Center.

At the Statesir Cancer Center at CentraState, the latest cancer therapies are adding years to lives.

PERSONALIZED CANCER TREATMENT SAVING MORE LIVESAt the Statesir Cancer Center at CentraState, the ability to identify and take advantage of what might make each patient’s cancer unique often facilitates better outcomes with fewer side effects. Whereas past chemotherapy options were somewhat limited and acted on the entire body, molecular and genetic testing can now be used to pinpoint targeted therapies for cancer treatment based on an individual’s genetic makeup and cancer type.

Immunotherapy is another type of precision cancer treatment offered at CentraState. These medicines harness the power of the body’s own immune system to recognize and attack cancer as it would any infection. Now used for many cancer types, immunotherapy is particularly effective for melanoma.

“When we find a unique feature of a patient’s cancer that we can treat with targeted therapy or immunotherapy, it’s a home run,” says Bhavesh Balar, MD, a board-certified hematologist/oncologist at CentraState. “These advances are making complete remission possible in cases where it wasn’t before—and transforming cancer into a chronic illness that can be managed effectively.”

New Treatment Helps Prevent Esophageal CancerBarrett’s esophagus is a condition often diagnosed in people with long-term gastroesophageal reflux disease (GERD). In some cases, Barrett’s esophagus can trigger a precancerous condition called dysplasia, which puts patients at higher risk of developing esophageal cancer.

To help prevent cancer in patients with Barrett’s esophagus, CentraState recently began offering an advanced, nonsurgical procedure called BarrxTM radiofrequency ablation. This outpatient procedure, which takes about 20 minutes per session, uses heat energy to destroy diseased tissue in the lining of the esophagus. The treatment has been shown to be more than 90 percent effective in removing precancerous tissue.

The Latest Robotic TechnologyOver the past eight years, CentraState’s specially trained surgeons have performed more

than 1,850 minimally invasive robot-assisted surgeries to treat colon, prostate, and kidney cancers, along with a range of other conditions. In 2017, the hospital acquired the da Vinci Xi® robotic system, the latest in minimally invasive surgical treatment options.

“In addition to improved visualization, the new robot has rotating arms that enable multi-quadrant surgery, so complex procedures requiring more than one access point take less time,” explains Troy Sukkarieh, MD, a board-certified urologist at CentraState. “For patients, this means a more efficient surgery with less time under anesthesia, along with the benefits of a shorter hospital stay and a faster recovery. At CentraState, we’re performing all the major robotic surgeries available at larger centers nationwide.”

A Nonsurgical Option for Eradicating Skin Cancer Patients at CentraState

now have access to a nonsurgical alternative to Mohs surgery for the treatment of basal and squamous cell skin cancers. Whereas Mohs surgery removes thin layers of cancerous skin until only cancer-free tissue remains, skin brachytherapy can destroy small, non-melanoma skin cancer using radiation, offering an improved cosmetic result.

With skin brachytherapy, a special applicator delivers high-dose, targeted radiation only to the affected area. Patients receive outpatient treatment sessions over the course of a few weeks, with each session lasting only a few minutes. Skin brachytherapy is particularly effective for areas like the face, where scarring can be a concern.

Radiation Therapy in Sync with BreathingThe Statesir Cancer Center recently began offering respiratory gating technology featuring deep inspiration breath hold (DIBH), which is a highly effective cardiac sparing technique to treat certain patients with left-sided breast cancer. DIBH takes the heart’s proximity to the breast and patient breathing into account during the treatment planning process, so that radiation is more precisely targeted to the tumor.

“This new technology is like a motion detector in that it enables us to pause the radiation dose if breathing moves the heart into the precisely defined treatment field,” says Edward Soffen, MD, a board-certified radiation oncologist and medical director of the Radiation Oncology Department at CentraState. “It’s the next level of sophistication in radiation therapy, giving us another option for sparing more normal tissue and further minimizing the risk of side effects.”

Reducing Treatment Time for Breast CancerWhen radiation therapy is needed for breast cancer, patients traditionally have received daily treatments for at least six weeks. At CentraState, women diagnosed with early-stage breast cancers may be candidates for hypofractionation, an accelerated, updated approach to radiation therapy following lumpectomy surgery.

Hypofractionation delivers the same biologically equivalent dose of radiation as traditional therapy, but in nearly half the time, using external beam radiation with a linear accelerator that focuses powerful beams of X-rays on cancer cells. This reduces a six-week protocol to roughly four weeks. For women trying to resume a normal life after a cancer diagnosis, fewer treatments can make an enormous difference.

It Takes a Team to Transform LivesWhether a patient is dealing with stroke, Parkinson’s disease, multiple sclerosis, or another neurological condition, a comprehensive approach at CentraState is facilitating improved function and quality of life. In addition to the patient and family, other care partners include neurologists, primary care physicians, nurses, dietitians, social workers, neurosurgeons, rehabilitation medicine physicians, and physical, occupational, and speech therapists.

“Our personalized treatment protocols paired with focused rehabilitation can be life-changing for patients,” says James Ware, MD, a board-certified vascular neurologist at CentraState. “We tackle problems from different aspects by examining our patients’ global needs. Everything is available under one roof to help them improve movement, cognitive function, and overall quality of life.”

NEUROSURGICAL EXCELLENCE CLOSE TO HOMEWhen the intense pain of a neurological condition impacts daily life, neurosurgery at CentraState is providing immense relief for patients. CentraState is one of a few hospitals in the region to offer microvascular decompression surgery, which can eliminate the excruciating facial pain caused by trigeminal neuralgia. Patients with occipital neuralgia, a condition causing distinct and chronic headaches, have access to a highly effective surgical procedure called rhizotomy. In addition, chiari malformation, a condition in which brain tissue extends into the spinal canal, can be treated with specialized neurosurgery to relieve pressure on the brain and spinal cord.

NOVEL APPROACHES IN NEUROSCIENCE

Through a collaborative approach, CentraState is improving quality of life for patients with challenging

neurological conditions.

A STEADFAST FIGHT AGAINST PARKINSON’S DISEASEPreliminary research is showing that boxing training may help patients with Parkinson’s disease maintain function longer—and even improve balance, gait, and the ability to perform daily activities. CentraState has incorporated this concept into its array of therapies in the battle against the disease. It is one of only a few hospitals in the state to offer Rock Steady Boxing, a unique physical therapy and fitness program designed specifically for patients with Parkinson’s disease.

Patients travel from throughout the region to participate in the program, which empowers them to literally fight back against the disease in a supportive environment. In addition to its physical benefits, Rock Steady Boxing allows patients to find relief from the anxiety and frustration that often accompanies their condition.

Expanding Movement and Vocal AbilitiesCoupled with other management tools for Parkinson’s disease, the Lee Silverman Voice Treatment LSVT BIG® program at the OceanFirst Rehabilitation Center at CentraState is having an extraordinary impact for many patients. This four-week, whole-body training protocol promotes large-amplitude movements that can improve mobility and expand range of motion. To manage the impact on speech often accompanying Parkinson’s disease, the Center also offers the LSVT LOUD® program, which helps patients project their voices, improve vocal quality, and gain speaking confidence.

EAT WELLAs more research demonstrates how brain and body health are linked to nutrition, CentraState is providing new programs focused on evidence-based approaches for healthier eating. These include Plant-Powered Eating, Mediterranean Diet, and Mind Diet for Brain Health programs. In addition to nutritional consultations and classes, CentraState also offers Metabolic Screenings. This sophisticated technology calculates daily calorie burn—the first step in effective weight management.

MOVE WELLMeditative movement programs are among the latest offerings in the movement and fitness category at CentraState. Beyond the traditional tai chi, yoga, and balance classes, a new Release, Restore, Relax program was recently launched. This six-week holistic program relieves muscle tension with trigger point techniques using tennis balls, bands, and calming breathing exercises.

Motivation-Inspired FitnessIn 2017, CentraState’s Fitness and Wellness Center began offering a motivation-inspired experience called MovofitTM. Designed to focus on individual goals, tools include assessments to identify personal motivators, motivational profile reports, customized fitness plans, one-on-one meetings with trainers, and body composition assessments provided by a nurse. The new concept is rooted in the science of behavior change, helping people reach their full potential.

RELAX WELLAs part of an array of meditation and stress relief programs with topics ranging from music-based healing to aromatherapy, CentraState offers a Stress Less: From Anxious to Awesome program. This program explores various techniques to promote peace of mind and lower stress-related hormone levels.

MANAGE WELLManaging Chronic Conditions

Chronic conditions can be well managed through CentraState’s evidence-based programs, many of which feature registered nurse health coaches who serve as a virtual arm of the physician practice to help patients achieve individual health goals. Programs include those focused on diabetes, congestive heart failure, general chronic disease management, and hypertension. In particular, CentraState is one of only a few hospitals in New Jersey that is now launching a specialized, evidence-based, yearlong Diabetes Prevention Program to help those with risk factors dramatically reduce their chances of developing the disease.

“We’re shifting our focus from acute disease management to helping people prevent the onset of disease altogether,” says James J. Matera, DO, medical director of population health at CentraState. “We’re well positioned to handle any complex risk factors on the horizon and empower patients to stay on track with their health.”

New Cardiovascular Model of CareCentraState is working with primary care physicians to pilot new models of care, such as the Centers for Medicare & Medicaid Services’ Million Hearts national initiative, which is designed to support physicians in their efforts to prevent heart disease and reduce costs to the health care system. Taking this a step further, CentraState recently initiated a process that guides these patients into a new Million Hearts health coaching and education program at the Star and Barry Tobias Health Awareness Center.

“Early indications show that we’re reducing the cardiovascular risk score among these patients,” says Kim Simers, assistant vice president of integrative health at CentraState. “This type of close connection involving the patient, primary care physician, and health coach distinguishes CentraState. It’s an approach that facilitates healthier individuals, a healthier practice, and a healthier population.”

Research Shows Benefits of Health CoachingA 2017 pilot study1 conducted at CentraState examined the impact of health coaching as provided by the hospital’s Managing Hypertension program. In the six-week program, health coaches consult with patients on topics including hypertension education, medication adherence, communication with primary care physicians, and the importance of home blood pressure monitoring. Study results found that 70 percent of program participants achieved lower blood pressure, and 100 percent of these patients self-monitored their blood pressure at home.

Making Self-Care More Accessible for WomenIn addition to offering a full range of women’s programs, CentraState recently placed a renewed emphasis on helping women recognize the importance of taking time for their own health at every age and stage of life.

“We know women have demanding schedules, so we cater to them by making programs unique, accessible, and fun,” says Mandi Wortman, director of radiology at CentraState. “We’ve applied this concept to mammograms, pelvic health, and other topics.”

CentraState offers an engaging new program that pairs wine and cheese with an intimate conversation about commonly experienced, avoidable women’s health issues, ranging from pain during sex to urinary leaking. In addition, Ladies Night Out events combine breast exams with wine, appetizers, gift bags, and the opportunity to socialize.

CentraState has also partnered with area Walmart and ShopRite stores to bring breast exams and other screenings directly to those in the community through the hospital’s LIVE LIFE WELLSM van. Likewise, a grant from Susan G. Komen is facilitating breast health education and mammograms for Hispanic/Latina women in the area.

CentraState offers the services and support women need to move through every important passage in life with confidence, energy, and vibrant health.

WOMEN’S HEALTH ACROSS THE LIFESPAN

Preventing Heart Disease in WomenThe Gloria Saker Women’s Heart Program at CentraState addresses the specific cardiovascular needs of women, which differ from men. The program identifies modifiable risk factors for cardiovascular disease and works with women to recommend lifestyle changes that decrease their risk, along with a focus on removing barriers to change. Services include individualized risk assessments, cardiac testing, the creation of a customized care plan, education, and referrals to any additional services that may be needed.

In a one-stop-shopping approach, women diagnosed with breast cancer at CentraState are now being referred to the program prior to treatment to assess any risk for heart disease. This approach is expanding to women receiving care for other types of cancer as well.

“Heart-healthy lifestyle changes don’t have to be drastic,” says Sangeeta Garg, MD, board-certified cardiologist and medical director of the Gloria Saker Women’s Heart Program. “We help women pinpoint small modifications that can make a big difference in preventing heart disease. These changes are more easily incorporated into a woman’s busy lifestyle, making them more sustainable.”

STRATEGIES FOR HEALTHY AGINGCentraState offers programs that can predict quality of life in later years while helping ensure optimal function.

Assessing and Improving Function Strength and mobility in middle age are striking predictors of function in later years. To help patients assess their health status and identify where adjustments are needed, CentraState now offers a full slate of Function for Life programs. Components include:

Grip strength test—A simple test that indicates overall strength and can predict future mortality

A Matter of Balance program—Assesses mobility and balance to detect and address function that falls below an age-matched norm

Mobility Screening Program—Uses OptoGait, the latest gait analysis technology, to evaluate mobility

Memory screening—Assesses cognitive function to detect and delay any impairments

Minds in Motion—Brain fitness classes designed to improve memory and build up cognitive reserve

“The evidence is overwhelming that when people maintain physical and cognitive function as they age, they have a longer lifespan and improved quality of life,” says Brian Mason, PT, DPT, clinical director of rehabilitation services at CentraState. “Our programs identify deficits and help people make adjustments to ensure optimal health in later years.”

New Posture ProgramCentraState recently launched a Posture for Life bone health initiative designed to mitigate and prevent osteopenia and osteoporosis. Participants undergo an initial assessment with a therapist and receive a customized program aimed at improving and even reversing any posture issues, reducing the risk of future spinal fractures.

One-Stop ScreeningThe new CentraScreen program at CentraState provides a one-stop health screening package for those over age 50. In about an hour, patients receive screenings of their heart rhythm, blood pressure, carotid artery, abdominal aorta, peripheral arteries, frailty index, and memory. The screenings are designed to identify any issues that could lead to a serious cardiac or vascular event, particularly in those who may be asymptomatic.

Five-Star CMS Rating: The Centers for Medicare & Medicaid Services (CMS) rated Applewood, CentraState’s senior living community, with five stars for its Health Center in the CMS Five-Star Quality Rating System. The Manor Health and Rehabilitation Center, which offers short-term rehabilitation and long-term skilled nursing services, also received five stars from CMS.

Best Nursing Home: The Manor Health and Rehabilitation Center has now been recognized as one of the nation’s Best Nursing Homes by U.S. News & World Report for two consecutive years.

Advanced Standing: Monmouth Crossing, CentraState’s assisted living community, has earned Advanced Standing distinction from the New Jersey Department of Health and Senior Services and the Health Care Association of New Jersey for the past six years, and Applewood has received this designation for the past four years.

ACUTE CARE FOR ELDERS UNIT DESIGNATIONIn 2017, CentraState became one of a select number of hospitals in the country to receive designation for an inpatient Acute Care for Elders (ACE) Unit. This 42-bed unit, now called the Statesir Family Acute Care for Elders Unit, is specifically designed to meet the complex needs of seniors recovering from injury or illness. Specialized care provided by a geriatric-trained team is aimed at maximizing independence, improving overall health and mobility, and reducing the risk for readmission.

Enhancing Applewood and the Statesir Family ACE UnitAs residents of Applewood, CentraState’s vibrant senior living community, Audrey and John Statesir have played a major role in the facility’s ongoing growth. In addition to upgrading the dining room in memory of Mrs. Statesir’s parents, who were also Applewood residents, their gifts are facilitating renovations of the bistro as well as the Dementia Memory Care Center in memory of Mr. Statesir’s parents. Their donations to the Statesir Cancer Center at CentraState have also enabled numerous advances in cancer care.

Through their new Statesir Family Charitable Foundation, the Statesirs recently made a major gift in support of CentraState’s Acute Care for Elders (ACE) Unit, which will now be named the Statesir Family Acute Care for Elders Unit. This funding will enhance senior-friendly aspects of the unit, such as floor lighting, bathroom renovations, and an activity room.

“As you get older, you begin to have a greater appreciation for services you may need in the future,” says Mrs. Statesir. “We’ve seen CentraState grow immensely, and we’ve been happy to support such an outstanding resource for the community.”

Philanthropy Expands Behavioral Health ServicesEvelyn Silvert, one of CentraState’s original founders, has been a steadfast supporter of the hospital’s growth in countless ways for more than five decades. Now age 95, she recently created the Evelyn Silvert Behavioral Health Endowment Fund to honor the memory of her grandson, Brian. As one of many initiatives that will be made possible through this fund, CentraState will now have a licensed clinical social worker on staff to provide outpatient behavioral health services.
