DEAR DR. ROACH: My 8-year-old grandson has had bright-red blood in his stool many times in the past four to six months. The doctor ini-tially said he was dehydrated and needed to drink more wa-ter. Next, he prescribed a bowel cleansing, which was done about a month ago. The bleeding continues, two or three times per month. It seems to me that the doctor isn’t taking this seriously.

Is this a common prob-lem? What are the possible causes, and could it be something serious? — E.B.

ANSWER: Rectal bleed-ing in school-age children is not uncommon, and it usual-ly comes from a benign source, such as a small tear in the lining of the bowel from having a hard bowel movement. These usually are treated with fluid, stool soft-eners and lubricants. Howev-er, repeated episodes should bring up the possibility of a more serious issue. In this age group, the most common other causes are juvenile polyps and inflammatory bowel disease.

A juvenile polyp is a be-nign growth of tissue in the colon, and these frequently can bleed. They are diag-nosed via endoscopy - either a sigmoidoscopy or a colonoscopy. They are al-most never cancerous.

Inflammatory bowel dis-ease is a serious condition of the gastrointestinal tract. Chil-dren with IBD may have poor growth or weight loss, and usually, but not always, have abdominal pain or bleeding.

I am concerned and con-fused about “bowel cleans-ing.” I think it is past time to have your grandson thor-oughly evaluated by a pedi-atric gastroenterologist.

DEAR DR. ROACH: My husband drinks tonic water with quinine every day. He drinks as much as 1½ quarts a day. Is that much safe? If not, what is a safe amount?

The reason he drinks so much is because he gets leg cramps and he was told that tonic water with quinine will help alleviate the cramps. — D.V.

ANSWER: Many people have found that quinine re-lieves leg cramps. However, the Food and Drug Adminis-tration banned sales of quinine for leg cramps due to unprov-en effectiveness and the possi-bility of side effects. Quinine in large doses can cause ab-normal heart rhythms, blood problems and even organ fail-ure. However, the amount of quinine in tonic water is quite small, compared with quinine tablets. One common brand has 17 mg in a liter, so your husband is getting about 25 mg. The quinine tablets for-merly prescribed for leg cramps were 200 mg. Toxic effects are unlikely at the dose in quinine water.

Some people are allergic to quinine, in which case even the small dose in tonic water could potentially cause problems.

Your husband can keep on as he’s been doing if he finds that it helps.
