COMBINED MUSCULO-SKELETAL IMAGING CLINICAL FELLOWSHIP PROGRAM OUTLINE The McMaster University, Faculty of Health Sciences, Academic Department of Radiology offers an MSK IMAGING FELLOWSHIP PROGRAM. The program is presently based at three hospital sites. St. Joseph’s Healthcare Hamilton Hamilton Health Sciences – Henderson General Hospital Hamilton Health Sciences – McMaster University Medical Centre The fellowship program draws from the strengths of all 3 healthcare facilities, resulting in a comprehensive MSK Imaging Training experience. The Program accommodates three Clinical Fellows. This outline describes the hospital sites and the MSK Fellowship Program. There are MSK fellowship-trained radiologists at each site who have an interest in all aspects of MSK imaging, with a particular expertise in MSK ultrasound. In addition, several other staff radiologists have a keen interest in this subspecialty. It is the Regional Referral Centre for rheumatology, nephrology, urology and thoracic surgery. The hospital is home to a busy general orthopedic practice with a special interest in trauma, sports medicine, spinal surgery and upper extremity orthopedics. There are also very active departments of rheumatology and physical rehabilitation. The Department of Diagnostic Imaging is fully digitalized and equipped with – a twin gradient 1.5 T MRI Scanner a 3T MRI Scanner ultrasound scanners PACS (Picture Archive & Communication System) a 16-slice CT scanner a 64-slice VCT Bariatric scanner Powerscribe (voice-activated dictation) The McMaster University Imaging Research Centre/Brain Body Institute located at St. Joseph's Healthcare, has a CT-PET scanner and a 3T MRI scanner offering excellent opportunities for research. HHS – Henderson General Hospital The Henderson General Hospital is one of three acute-care hospitals that comprise Hamilton Health Sciences. The Henderson has a busy oncology-focused imaging practice. The hospital is home to an active general orthopedic practice, specializing in joint replacement, as well as orthopedic oncology. The Juravinski Regional Cancer Centre is located adjacent to the Henderson Hospital, connected by a walkway. Patients with primary and secondary bone and soft-tissue tumors are referred to the Henderson Hospital for detailed imaging and care.

HHS - McMaster University Medical Centre (MUMC) MUMC is one of the three acute-care hospitals that comprise the Hamilton Health Sciences. It houses the McMaster Children’s Hospital, in addition to its adult services. The hospital is home to a busy general orthopedic practice, including several orthopedists with a sport-medicine and knee arthroscopy focus. There are also dedicated pediatric orthopedic surgeons who provided general pediatric orthopedic services, as well as pediatric oncology. The Department of Diagnostic Imaging is fully digitalized and equipped with –

• 16-slice CT scanner • 64-slice CT scanner • 1.5 T MRI scanner • Ultrasound Machines • PACS (picture archives & communication system) • Powerscribe (voice-activated dictation system)

CLINICAL FELLOWSHIP OVERVIEW SCHEDULE The week is divided as follows –

80% dedicated MSK Imaging (4 days per week) 20% protected academic time (1 day per week) If research is not undertaken this time is

reduced to 10%, for preparation presentations, lectures, teaching of residents etc. There is also a component of general imaging to maintain skills in cross-sectional imaging.

MSK Clinical Fellows spend four months at each of the sites: Henderson General Hospital, St. Joseph’s Healthcare and McMaster University Medical Centre. ACADEMIC DAYS All Fellows are allotted one academic day per week (see above). Henderson General Hospital – Wednesday St. Joseph’s Healthcare – Tuesday McMaster – TBA This day is designated for the purpose of conducting research, presentation preparation and personal study.

ON CALL Clinical Fellows participate in the General On-Call Schedule at the site where they are scheduled. The on-call commitment for Diagnostic Imaging Fellows varies between hospitals with a one in five weeknight and weekend and a one in six weeknight and weekend. At all sites, the MSK Fellow always provides on-call coverage together with a staff radiologist.

MSK IMAGING Fellows are to participate in all aspects of MSK imaging including plain radiographs, CT, MRI, US, fluoroscopy and interventional procedures. CLINICAL ROUNDS MSK Clinical Fellows must attend and participate in all relevant clinical and teaching rounds at each site. REMUNERATION / LEAVE Remuneration, annual leave and study leave is standard for all radiology fellows. Details are available through the Site Supervisors. RESEARCH Fellows will have ample opportunity to participate in research and are encouraged to present research findings at all North American meetings. It is expected that fellows will complete two academic/research projects during the course of their fellowship.

F. ANNUAL LEAVE / CONFERENCE LEAVE Clinical Fellows are entitled to –

four-weeks’ annual leave two (2) weeks’ conference or study leave, for a total of 6 weeks leave Where possible, leave should be divided equally between St. Joseph’s Healthcare, the

Henderson Hospital and McMaster University Medical Centre (i.e. 2 weeks during each of the four month blocks).

Important Note – Annual and Academic/Study Leave must be requested three (3) months in advance with the on-site supervisor at either hospital and with Kim Harper, Fellowship Coordinator for St. Joseph’s Healthcare.

G. RESEARCH

Clinical Fellows are required to complete two (2) dedicated MSK projects per year.

A list of potential projects will be available at the start of the year and these can be further discussed with the Site Supervisor at either hospital.

A staff radiologist will be assigned to oversee fellows with each individual project.

Clinical Fellows are encouraged to develop their own projects and include radiology residents into the research projects whenever possible.

Ongoing research will be discussed at the MSK imaging rounds once monthly.

Fellows are to present one to two talks to Radiology Residents per year. These will be given to residents at their weekly academic half days, or as a regional round. The subject of the presentations will be decided in consultation with the MSK Imaging Fellowship Supervisor and Chief Resident. The MSK radiologist will provide support for the fellow to prepare for the talks. Copies of all presentations will be maintained at the Hamilton Health Sciences and St. Joseph’s Healthcare Hamilton sites as a resource.
