No. 61-2015 APPROVE MINUTES Motion made by Ms. Buchanan, seconded by Mr. Coleman to approve the minutes from the April 22, 2015 Regular Meeting/State of the Schools. Roll Call: Buchanan, Coleman, Brassell, Nemeth and Moore voted “Aye.” Motion Carried. No. 62-2015 APPROVE TREASURER’S REPORT AND RECOMMENDATIONS Motion made by Ms. Buchanan, Seconded by Mr. Coleman to approve the following: *Report/Presentation - Approve the receipts, expenditures and balances for the month ending April 30, 2015 as presented by the Treasurer. COMMUNITY INPUT AND COMMUNICATIONS • Pastor Jim Hurst, Minister Association and Columbia Community Foundation – requests

use of CHS cafeteria, kitchen, tables, chairs, on November 25 and 26, 2015 for a community Thanksgiving Dinner.

• Pam Coleman, Columbia Youth Basketball – requests waiver of hourly fee for weekend

and after-hour use of the CMS/CHS gymnasiums for youth basketball practices held in December, 2014 and January/February, 2015 – total cost $1,375.00.

BOARD COMMENTS/ITEMS No. 64-2015 APPROVE THE USE – CHS CAFETERIA Motion made by Ms. Buchanan, Seconded by Mr. Moore to approve the use of the CHS cafeteria, kitchen, tables, chairs on November 25 and 26, 2015 for a community Thanksgiving Dinner, at a cost of $0.00. Roll Call: Buchanan, Moore, Coleman and Nemeth voted “Aye”, Brassell abstained. Motion Carried. No. 65-2015 APPROVE THE REQUEST – WAIVE HOURLY FEE Motion made by Ms. Buchanan, seconded by Mr. Moore to approve the request to waive the hourly fee associated with the use of the CMS/CHS gymnasiums for youth basketball practices held in December, 2014 and January/February, 2015, in the amount of $1,375.00. Roll Call: Buchanan, Moore, Brassell, and Nemeth voted “Aye”, Coleman abstained. Motion Carried No. 66-2015 APPROVE THE USE OF COLUMBIA FOOTBALL FIELD Motion made by Ms. Buchanan, seconded by Mr. Moore to approve the request to use the CHS Football Field on one Saturday in the month of September, 2015 for two games, by the Columbia Youth Football Program and the Raider Youth Football Program if so desired. Roll Call: Buchanan, Moore, Brassell and Nemeth voted “Aye”, Coleman abstained. Motion Carried No. 67-2015 ACCEPT RESIGNATION – CHARLES RHEAUME Motion made by Ms. Buchanan, seconded by Mr. Coleman to accept the resignation for retirement purposes from Charles Rheaume effective July 31, 2015 Roll Call: Buchanan, Coleman, Brassell, Nemeth and Moore voted “Aye”. Motion Carried No. 68-2015 APPROVE CERTIFICATED PERSONNEL ITEMS Motion made by Mr. Coleman, seconded by Ms. Buchanan to approve the following: *Non-Renew Long Term Sub - Phillip Plas - Non-renew the one-year substitute teacher contract of Phillip Plas due to the anticipated return to the classroom by the regular teacher in accordance with ORC §3319.11(E). *Resignation – Elizabeth Becher - Accept the resignation from Elizabeth Becher, school teacher, effective at the end of the 2014/15 contract year. *Resignation – Alexandria Bakos - Accept the resignation from Alexandria Bakos, special education teacher, effective at the end of the 2014/15 contract year.

*Summer Groundskeeper - Employ Edward Houston as a summer Groundskeeper, $12.83 per hour, not to exceed 12 weeks, hours assigned by the Maintenance Supervisor, pending ESC, ORC, and District requirements. *Contract Services – Meta Solutions – Approve the following for the 2015/16 school year: Master Service Agreement/Core Services; Reciprocal Service Fee Agreement; Library Automation: INFOhio. EXHIBIT 10 *Free and Reduced-Price Lunch Program - Participate in the National School Lunch Program and provide free and reduced-price lunches to qualified students during the 2015/16 school year. *Grants – Participate in all Local, State, and Federal grant programs for the 2015/16 school year. *Membership – Ohio High School Athletic Association - Renew membership in the Ohio High school Athletic Association for the 2015/16 school year for Columbia High School and Columbia Middle School: WHEREAS, the Columbia Local School District of 25796 Royalton Road, Lorain County, Ohio has satisfied all the requirements for membership in the Ohio High School Athletic Association, a voluntary unincorporated association not-for-profit; and

WHEREAS, the Board of Education and its Administration desire for the schools with one or more grades at the 7-12 grade level under their jurisdiction to be voluntary members of the OHSAA; NOW THEREFORE, BE IT RESOLVED BY THE BOARD OF EDUCATION that Columbia High School and Columbia Middle School do hereby voluntarily renew their membership in the OHSAA and that in doing so, the Constitution and Bylaws of the OHSAA are hereby adopted by this Board as and for its own minimum student-athlete eligibility requirements. Notwithstanding the foregoing, the Board does reserve the right to raise the student-athlete eligibility standards as the Board deems appropriate for the school and students under its jurisdiction; and BE IT FURTHER RESOLVED that the schools under this Board’s jurisdiction agree to conduct their athletic programs in accordance with the Constitution, Bylaws, Regulations, Interpretations and decisions of the OHSAA and to cooperate fully and timely with the Commissioner’s Office of the OHSAA in all matters related to the interscholastic athletic programs of the schools. Furthermore, the schools under this Board’s jurisdiction shall be the primary enforcers of the OHSAA constitution, Bylaws and Sports Regulations and the interpretations and rulings rendered by the Commissioner’s Office. The administrative heads of these schools understand that failure to discharge the duty of primary enforcement may result in fines, removal from tournaments, suspension from membership and/or other such penalties as prescribed in Bylaw 11. *County Service Agreement - Approve the 2015/16 County Service Agreement from the Educational Service Center of Lorain County as presented. EXHIBIT 11 *Preschool Agreement/Lorain County Board of Developmental Disabilities - Renew agreement with the Lorain County Board of Developmental Disabilities to provide services to District preschool-aged special needs children during the 2015/16 school year. EXHIBIT 12 *Volunteer - Accept the following volunteer working with District athletic teams during the 2015/16 school year, pending District and OHSAA requirements: Chip Shope – Football *Transportation Release a. Release, for transportation purposely only, resident student Kaylyn Johnson to the Berea

City Schools for transportation during the 2015/16 school year to/from a caregiver in Berea to St. Joseph’s Academy.

b. Release, for transportation purposes only, resident student Joseph Felix to the Olmsted Falls City Schools for p.m. transportation during the 2015/16 school year from St. Mary of the Falls to a caregiver in Olmsted Township.

*Contract Services – Suburban School Transportation - Contract with Suburban School Transportation to provide supplemental transportation services, as needed, during the 2015/16 school year. Motion Carried No. 71-2015 DECLARE TRANSPORTATION IMPRACTICAL Motion made by Ms. Buchanan, seconded by Mr. Brassell to adopt the following resolution declaring transportation impractical for the 2015/16 school year, and authorize the Treasurer to offer to provide the parents/guardians with payment in lieu of transportation: WHEREAS, the students identified below have been determined to be residents of this school district and eligible for transportation services; and WHEREAS, after a careful evaluation of all available options, it has been determined that it is impractical to provide transportation for these students to their selected schools; and WHEREAS, the following factors as identified in Ohio Revised Code §3327.02 have been considered:

1. The time and distance required to provide the transportation; 2. The number of pupils to be transported; 3. The cost of providing transportation in terms of equipment, maintenance, personnel,

and administration; 4. Whether similar or equivalent service is provided to other pupils eligible for

transportation; 5. Whether and to what extent the additional service unavoidable disrupts current

transportation schedules; 6. Whether other reimbursable types of transportation are available, and

WHEREAS, the option of offering payment-in-lieu of transportation is provided in Ohio Revised Code. NOW, THEREFORE, BE IT RESOLVED that the Columbia Local School District Board of Education hereby approves the declaration that it is impractical to transport the students identified herein and offers the parents/guardians of students named.
