This directory provides a list of KaiserPermanente Senior Advantages networkproviders. To get detailed information about yourhealth care coverage, please see your Evidence

of Coverage.You will have to choose one of our networkproviders who are listed in this directory to beyour Primary Care Physician (PCP). The termPCP will be used throughout this directory.Generally, you must get your health carecoverage from your PCP.

PCPs include:

Doctors in Family Medicineprovidecontinuing care for individuals andfamiliesfrom newborns to the elderlyincluding sports medicine.

Doctors in Internal Medicinespecialize inadult medicine, with a focus on preventingand treating complex adult conditions andcaring for the elderly.

Doctors in Pediatricscare for children frombirth to 18 years old.

Please note: Female members may elect to seean OB/Gyn for their routine health maintenanceexams, while seeing their designated PCP if non-female specific problems arise.

The network providers listed in this directoryhave agreed to provide you with your health carecoverage. You may go to any of our networkproviders listed in this directory; however someservices may require a referral. If you have beengoing to one network provider, you are not

required to continue going to that same provider.In some cases, you may get covered servicesfrom non-network providers. Your PCP may alsocoordinate the rest of your care and may referyou to a specialist when needed. You can alsoconsult with most of our specialistswithout a referral. You may also self refer

(schedule an appointment without a physicianreferral) for consultation or routine visits to mostnetwork and Kaiser Permanente specialists.A self-referral covers routine office visits and

office-based services. You will still need aphysicians referral and written authorizationfrom Kaiser Permanente if you need additionalservices not provided in the doctors office,such as surgery, MRI, CAT scan, colonoscopy,or hospital admissions. The acupuncture, chiropractic, and hearing aidproviders listed in Section 2 are covered bythe Senior Advantage Plus Choice (HMO-POS)plan and Advantage Plus optional supplementalbenefits packages and may not be a part ofthe Senior Advantage Core, Silver, and Goldplan provider network. The listing informationis subject to change. For the most updatedinformation, please call Member Servicestollfree at 1-800-476-2167/ TTY:711, 8 a.m. to8 p.m., seven days a week. When calling toschedule services, please inform the provider ifyou have Kaiser Permanente Senior AdvantagePlus Choice (HMO-POS) or Advantage Plus.

If you receive care from an out-of-networkprovider and they ask you to pay for servicesat the time of service, its best to ask an out-of-network provider to bill us first. If you receive abill from an out-of-network provider, you shouldnot pay the bill and instead send it to our ClaimsDepartment for processing and determination ofyour cost-sharing amount, if any.

TTY: 711, seven days a week, 8 a.m. to 8 p.m.Our payment for covered services is based uponcharges that we determine to be reasonable inaccord with Medicare payment guidelines.

Emergency care can always be obtained in orout of the service area from the nearest availableprovider. When in the service area, you must useplan providers for urgent care. When out of theservice area, urgent care may be obtained fromthe first available provider. In addition, when

out of the service area, you can obtain dialysistreatment from any qualified dialysis provider.

You must use plan providers except in emergencyor urgent care situations, for out-of-area renaldialysis, for care that has been approved byus in advance, or for services covered underthe Kaiser Permanente Senior Advantage PlusChoice (HMO-POS) plans point-of-servicebenefit. If you obtain routine care from out-of-network providers, neither Medicare nor Kaiser

Permanente Senior Advantage will be responsiblefor the costs.
