This policy and accompanying procedures were developed by the College of Medicine (CMED) in collaboration with the office of the Associate Dean for Academic Affairs at CMED, the Associate Dean for Clinical Affairs at CMED, and the Vice President for Medical Education.

The Office of the Dean of the College of Medicine is responsible for overseeing and monitoring the implementation of this policy and accompanying procedures.

Purpose

Purpose of this policy is to describe:

• the appointment processes and procedures of clinical faculty with primary employment at an affiliated institution outside of Qatar University;

• the criteria used in determining the academic rank at appointment;

• the preparation of the appointment application and the supporting documents.

Policy Description

The Qatar University College of Medicine (CMED) is actively affiliated with several healthcare delivery systems in Qatar with whom it collaborates closely for the purpose of fulfilling its mission. Clinical faculty appointment is a process where the appointees contribute to the education of the medical students whether at the bed side, the classroom or at the college. Through clinical appointments with CMED, the clinical faculty can contribute to the education of the future generation of doctors. In addition to being conferred a faculty title, clinical faculty will have access to the electronic resources of the university as well as a QU email address.

Clinical faculty at the affiliated hospital / clinic are expected to be actively engaged in patient care, education and scholarly activities at their institutions. They will be expected to contribute to a significant degree to the academic and clinical teaching programs at the college.

The rank at which the clinical faculty is appointed will depend on the credentials, the extent of clinical experience, teaching experience, contribution to scholarly activities, and the number and impact of scientific publications. Applications for clinical faculty appointments are reviewed by the Joint Clinical Faculty Appointment Committee, which recommends the appropriate ranking, as outlined in section 4, article 7 of the present policy.

Policy Statements

Section 1: Definition and Eligibility

Article 1: Definitions:

• Clinical faculty refers to an individual, licensed to practice medicine in Qatar and who holds a full or part time appointment at Hamad Medical Corporation (HMC), the Primary Health Care Corporation (PHCC), or other healthcare institutions affiliated with CMED.

• Consultant is defined as a senior hospital-based physician who has completed his or her specialist training and been placed on the specialist register in their chosen specialty.

• Educational leadership is broadly defined as teaching / training of students, residents, clinical fellows, research fellows, peers, mentorship and administrative teaching leadership roles.

• Extramural funding is broadly defined as funding that comes from outside QU that is used to support a program or project. It may come from federal, state or local governments, business, private foundations, or individuals.

• Fellowship Program is broadly defined as a postgraduate training after completion of a specialty training program in a relevant sub-specialty.

• Guidelines and protocols are broadly defined as nationally and / or internationally recognized documents that are guiding decisions and criteria regarding diagnosis, management, and treatment in specific areas of healthcare including summarized consensus statements on best practice.

• Internship is defined as the period when a physician is in clinical training after completion of medical school, but before obtaining a full license to practice medicine unsupervised.

• Investigator funding is broadly defined as individual-related funding that is used to support talented early-career scientists with a route into a principal investigator position.

• Joint Clinical Faculty Appointment Committee refers to the governing body at CMED that reviews the academic appointment of clinical faculty, according to this policy, and in line with the organizational structure of CMED. Its Terms of References are outlined in the College Committee Overview approved by the Dean of the College of Medicine.

• Postgraduate highest professional degree is broadly defined as the degree in a specialty, which qualifies its holder to be a specialist in a clinical domain.

• Research includes, but is not limited to basic, translational, and clinical research including epidemiology, outcomes, health services, patient safety and quality statistics, as well as social sciences, ethics, informatics and health economics.

• Residency is broadly defined as the period of post-graduate training in a given specialty. Approved Residency Programs refer to Residency Programs that are accredited by nationally and / or internationally recognized organizations for medical education including, but not limited to Certificate of the American Board, the Royal College of Physicians of Canada, or the Arab Board of Medical Specialization.

• Scholarly achievement is broadly defined to include peer-reviewed publications, poster presentations at regional and international conferences, educational materials in all types of media, policy statements, and contributing to the development of assessment tools, protocols, and guidelines for patient care.

Section 2: Responsibilities and Rights

Article 2: Responsibilities of clinical faculty

• Teaching and training of students of the CMED at the affiliated clinical training sites or at the college campus, in alignment with the rank of appointment.

• Teaching, consulting and engaging in academic activities that leads to scholarly achievement with reference to the CMED.

• Supporting the mission, goals, objectives and activities of QU and complying with all its bylaws, policies, and rules.

• Clinical faculty are primarily accountable concerning his/her teaching and educational responsibilities at the CMED to the Associate Dean of Academic Affairs for the academic aspects and to the Associate Dean of Clinical Affairs for clerkship aspects of the curriculum. These responsibilities and reporting relationships may be further defined at the time of appointment.

Article 3: Rights of clinical faculty

• Clinical faculty will be appointed at an academic rank suited to their credentials according to the QU regulations for appointment of clinical faculty.

• Clinical faculty will benefit from all academic facilities and may participate in QU educational and research activities, and faculty development program and will have access to its educational resources.

Section 3: Ranks of Appointment

Article 4: Ranks There shall be five ranks of clinical faculty:

1. Clinical Teaching Assistant

2. Clinical Lecturer

3. Clinical Assistant Professor

4. Clinical Associate Professor

5. Clinical Professor

Criteria for clinical faculty ranks are based on the educational background, professional qualifications, clinical experience, teaching experience and scholarly activities. The University will grant a rank for an applicant using one of the above titles, provided that the credentials are obtained from recognized institutions and provided that the applicant has fulfilled all the requirements of the appointment to the clinical faculty rank as outlined in the following article:

Clinical Teaching Assistant

Educational qualification Relevant bachelor degree (MBBS / MD or equivalent).

Clinical experience At least two years of experience in the profession after internship.

Clinical Lecturer

Educational qualification Relevant bachelor degree (MBBS / MD or equivalent) plus postgraduate degree in a specialty, such as board certification or equivalent professional degree.

Clinical experience • Completion of an approved structured Residency Program and certified as a Specialist by

either a University or Specialty Board (e.g., Arab Board) in the field as defined by the Hamad Promotion Committee and the Supreme Council of Health, Qatar Council of Health Care practitioner (QCHCP) Licensing Department as a Specialist.

• Holds a specialist position or equivalent for at least one year.

Clinical Assistant Professor

Educational qualification Similar to Clinical Lecturer plus highest terminal professional post-graduate degree such as those granted by the UK, Ireland, Canada, Australia and New Zealand (e.g. MRCP, FRCS, MRC Psych, FFARCS, MRCOG, FRCR) or the Certificate of the American Board, the Royal College of Physicians of Canada, or the Arab Board of Medical Specialization (or equivalent, e.g. Specialist status), or other qualifications equivalent to PhD recognized by the University and allows its holder to be appointed as Consultant at HMC, PHCC or other healthcare organizations in Qatar and recognized by QCHP for registration as a Specialist.

Research experience Evidence of scholarly achievement at national/ international level, published a minimum of three papers in indexed peer reviewed journals or holding PhD degree.

Teaching experience • Experience of teaching at the undergraduate and/or postgraduate level of at least 3

years.

• Evidence of excellence in teaching needs to be confirmed by the clinical department at the affiliated institution.

Clinical experience • At least 3 years of clinical experience post the “highest terminal professional or doctoral

degree” with evidence of providing excellent clinical care.

• Completed a Fellowship Program in a Subspecialty or got promoted to Consultant at the employing healthcare institution in Qatar.

Reputation • Appointment as Clinical Assistant Professor requires as a minimum a strong local

reputation at “institutional level”. (To be confirmed by the clinical department at the affiliated institution.)

Clinical Associate Professor

Educational qualification • Similar to the Clinical Assistant Professor.

Research experience • Contributed to research by publishing at least 7 scientific papers in indexed peer

reviewed journals or holding a PhD degree plus at least 4 scientific publications in indexed peer reviewed journals.

• Evidence of significant scholarly achievements at national level (list of talks + presentations) and preferably regional level.

Educational experience • At least 5 years of experience in teaching of medical and /or health professions students

or residents in postgraduate programs.

Clinical experience • Has at least 6 years of post-specialization experience “post highest professional degree”.

• A minimum of 6 years as a Consultant with evidence of providing excellent clinical care.

Reputation • Appointment as Clinical Associate Professor requires a strong national reputation within

Qatar and preferably a regional reputation in other Gulf Cooperation Council Countries. (To be confirmed by the clinical department at the affiliated institution.)

CLINICAL PROFESSOR

Educational qualification • Similar to the Clinical Associate Professor.

Research experience • Contributed to research by publishing at least fifteen peer-reviewed papers indexed in

PubMed or holding a PhD title plus a minimum of 12 peer-reviewed papers indexed in PubMed.

• Evidence of scholarly achievements at national /regional and international levels.

Educational experience • At least eight years teaching experience with medical and/or health sciences students

and/or postgraduate students.

Clinical experience • Has at least 10 years of post-specialization experience “post highest professional

degree”.

• A minimum of 6 years as a Consultant with evidence of providing excellent clinical care.

Reputation • Appointment as Clinical Professor requires a strong regional reputation and preferably

international reputation with excellence in clinical experience, education and scholarly productivity. (To be confirmed by the clinical department at the affiliated institution.)

Article 5: Recognition of demonstrated Areas of Excellence In order to allow assembly of a profile that reflects the unique combination of activities and accomplishments of each clinical faculty member, appointments should not only be categorized based on the defined quantitative criteria. The appointment process will recognize the broad range of faculty activities that contribute to the academic mission of the healthcare institution and CMED. The sum of an individual’s achievements, significant contribution to the development of the healthcare system in Qatar, major administrative and institutional service, and support to the mission of CMED will be considered in the evaluation for appointment. Candidates with outstanding performance in either one of the following areas of excellence might be exempted from certain minimum quantitative requirements by the Joint Clinical Faculty Appointment Committee in order to acknowledge his/ her exceptional achievements.

Three Areas of Excellence will be considered:

A. Teaching and Educational Leadership

Candidates must demonstrate scholarship which may include publication of original educational research, reviews, chapters, educational material in all types of media, and development of syllabi, curricula, web-based modules, courses and development of educational and assessment tools and policy statements.

• As Clinical Assistant Professor, candidates must have evidence of a strong local reputation as an active and highly effective clinical educator with increasing involvement and responsibility over time.

• As Clinical Associate Professor, candidates must have developed innovative teaching methods, curricula, educational policy or educational assessment tools, or have performed influential research related to education. Scholarship may take the form of influential first and senior author publications related to education, or educational materials developed by the candidate and adopted for use regionally or nationally.

• As Clinical Professor, candidates must demonstrate a sustained national, regional and in some cases international reputation as educational leader and innovator, and must be considered to be among the best in the country in the development of educational methods, curricula, policy and/or assessment tools, or in the conduct of educational research. Candidates’ expertise must be demonstrated through high impact scholarship that influences the field nationally and internationally.

B. Clinical Expertise and Innovation Candidates may have a reputation as an innovator in diagnosis, treatment application of new technology and development of care delivery models. Candidates must demonstrate scholarship in the area of expertise by publishing in peer-reviewed journals, development of guidelines and protocols. There should be a strong educational component in the clinical field and the individuals may participate in basic, translational or clinical research related to the clinical field.

• As Clinical Assistant Professor, candidates must have evidence of a strong local reputation as expert in a clinical field with a leadership role and/or a key role in activities that influence practice, such as approaches to diagnosis, treatment or disease prevention, applications of technology to clinical care and/or development or local adoption of innovative models of care delivery. Candidates must have first author scholarship related to the clinical field and should be teaching in the clinical field.

• As Clinical Associate Professor, candidates may have a strong regional reputation as an independent expert who has influenced the clinical field. He/she may have demonstrated innovation in approaches to diagnosis, treatment or disease prevention, the development/application of technology for clinical care and/or the development of novel models of care delivery that influence care at the local and regional level. Candidates must have an active teaching role and have influential first and senior author scholarship in the area of clinical expertise.

• As Clinical Professor, candidates’ expertise must be demonstrated through high impact scholarship. Candidates must have a significant influence on practice in the clinical field locally and internationally as a result of his/her teaching, scholarship and innovation.

C. Research

Scholarship may include senior author publications to original research or publication of large multi-disciplinary studies in which the candidate made documented significant intellectual contributions.

• As Clinical Assistant Professor, scholarship most often includes publications as first author; candidates may also be in another authorship position on publications from collaborative research to which he/she has made documented, substantive intellectual contributions. Candidates generally have some funding to conduct research, though not necessarily as principal investigator, and most often have evidence of teaching and supervision of trainees.

• As Clinical Associate Professor, candidates must have a national reputation as independent investigator and major contributor to the field. There must be a record of independent scholarship which most often will include first and senior authorship on high quality publications that have advanced the field; candidates may also be in another authorship position on publications from collaborative research that significantly advance biomedical science to which he/she contributed critical ideas or innovations, having taken the role of first or senior author on some publications. Candidates should have a successful extramural funding record, and must have evidence of teaching and supervision of trainees.

• As Clinical Professor, candidates must be leader of an exceptional independent research program and/or have key leadership roles in collaborative studies. Candidates must have a longstanding record of exceptional scholarship which most often includes senior authorship on high impact publications of original research, and have played a leadership role on, and made critical contributions to publications of high impact collaborative research. Candidates should have a sustained record of extramural funding, which most often will include principal investigator funding, and must have evidence of effective teaching and supervision as demonstrated by the number and stature of his/her trainees.

Procedure

Article 6: Application requirements

1. Applicants have to get approval of the primary employer indicating his current position in his/her institution. An approval letter can be downloaded on the CMED webpage. (http://www.qu.edu.qa/medicine/)

2. Applicants are required to submit the approval letter and the curriculum vitae using the CMED template indicating his/her clinical, teaching, academic activities and list of research publications into the link on the CMED webpage. (http://www.qu.edu.qa/medicine/)

3. For appointment at any rank, the applicant is required to include supporting documents in evidence of meeting the criteria. These include evidence and details of previous experience of teaching, list of publications or holding an academic rank in a university.

Article 7: Authorization for Approval

1. Applicants need to submit their application through the link on the internet including the required documents as indicated in Article 6.

2. Applications will be screened by the HR system and reported to the Joint Clinical Faculty Appointment Committee, as outlined by the respective Committee Terms of Reference.

3. Recommendations for appointment are made by the Joint Clinical Faculty Appointment Committee and shall be endorsed by the Associate Dean for Clinical Affairs and raised to the Dean of the College of Medicine. The final approval of all faculty appointments will be by the President of Qatar University.

Article 8: Duration of appointment The appointment will be for 3 years renewable based on the recommendations of the medical college and primary employer.
