Clinical Paperwork Checklist Medical Professions II Mr. Humphrey Med Pro II students have a unique list of requirements for the clinical portion of the program. Here is a list of items that need to be completed prior to the clinical experience. Before Winter Break is preferred/encouraged. Please feel free to mark the completed items as they are completed and submitted to Mr. Humphrey. As stated in the Nursing Program Manual, there are dress code requirements. The items that need to acquired by each student nursing assistant are as follows: White socks – Tall enough to cover the anklebone of the wearer. White, all-leather sneakers – NO canvas (such as Converse), clogs (such as Dansko™) or Crocs™. Running shoes give better support for the work environment than tennis shoes. New Balance™, Adidas™ and Nike™ style shoes are suggested. If all white shoes are not available, such as those with colored accents, the accent color must match the student uniform. No fluorescent or awkward colored shoes. Wristwatch with second hand; No digital watches allowed. No leather band. An elasticized wristband is also preferred. Students with tattoos must have them covered. Items such as Ink Armor and similar are available on-line. The suggested price range for students to adhere to is $13-$25. Single-tubed style is preferred opposed to dual-tubed style as they may rub together making it difficult for students to auscultate the appropriate sounds. Excellent website: www.allheart.com. They offer nurse & student kits as well. Student Nursing Assistant Uniform – The school scrub uniform color is Navy Blue, with the preferred brand being Cherokee Workwear. A vendor (Scrubs & Beyond) will visit the classroom to measure students and place orders. The date will be announced. Arizona Department of Public Safety Fingerprint Clearance Card – A vendor will visit the classroom to record student fingerprints for this purpose. The date will be announced. The following forms are located on Pages 20-27 of the Nursing Assistant Program Manual. Health & Safety Documentation & Healthcare Provider Form (pgs. 21-22) – Must be completed by students’ primary care provider with supporting documentation for vaccinations/titers attached. Student Information Form (pg. 20) Student & Parent Nursing Assistant Program Agreement Form (pg. 23) Work-based Learning Contract Covering Driver’s License & Transportation Requirements (pgs. 24-25) Covering Insurance & Emergency Information (pg. 26) Absenteeism Agreement (pg. 27) Copy of Student Drivers License Copy of Arizona Department of Public Safety Fingerprint Clearance Card Urine Drug Screen – Must be completed within 30 days of the first day of Clinical. Forms will be distributed to students within the appropriate time frame.
