Managing surgical processes with precisionAn operating room (OR) is one of the most expensive settings in healthcare. Hospitals must carefully manage surgical resources to efficiently meet the demand for scheduled and emergency procedures. Trinitas Regional Medical Center adopted the Sunrise Surgical Care solution to support a single, continuous patient record across perioperative services and beyond.

Real-time, transparent data helps Trinitas achieve better care coordination, faster and accurate billing, as well as efficient staffing to meet surgical utilization demand. Trinitas has the goal to increase its utilization by 9%.

Moving perioperative services from a paper-based to electronic system

Established by the consolidation of St. Elizabeth Hospital and Elizabeth General Medical Center in 2000, Trinitas Regional Medical Center is a full-service teaching hospital serving people who live and work in Central New Jersey. Delivering quality care depends on a single electronic health record (EHR) that captures everything the patient has experienced across care settings, including the operating room (OR).

“Perioperative services’ clinical documentation was all on paper,” Lockett said. “Manual processes made it hard to retrieve data, create an audit trail, update preference cards, review charts, understand resource utilization and bill charges…Trinitas decided an electronic OR system with advanced reporting and analytic capabilities was the way to move the surgery department forward.”

Allscripts Sunrise serves as the EHR platform for the rest of the hospital, and Trinitas chose to extend it to its surgical processes. In 2015, Trinitas was one of the first organizations to build and implement Sunrise Surgical Care.

“We went live with in-room OR support on the first day, and by the second day, the OR Circulators were functioning on their own. It was an easy transition to Sunrise Surgical Care,” Lockett said. “Staff saw an immediate difference in the ease with which they could find information…If you ask them now, they’d never go back to the way they did things before.”

More up-to-date preference cards

Preference cards help clinicians prepare in advance for surgical procedures based on doctors’ specific preferences. Trinitas estimates it had about 7,000 preference cards to update manually.

Prior to Sunrise Surgical Care, Trinitas preference cards were Microsoft Word documents. Updating files was never current and it was challenging to track supplies, equipment and resources. Now the process is automated, with updated supplies interfaced. It is easier to make global or patient-specific changes, and the organization can keep preference cards current. Related charges are also more accurate.

“We have had a lot of changes in surgery over the past year,” Lockett said. “We’ve had many surgeons retiring and many new surgeons starting. The system now makes it easier to update preference cards and keep information current.”

Seamless record of care and charges

Sunrise Surgical Care enables better accuracy in documentation and greater access to information for the care team. For example, instead of relying on clinical paper documentation, Trinitas now has an electronic record of patient care that flows from one record of care to the next level of care, minimizing repetitive patient questions. Data analytics provides information in real time about procedures performed, participants on the OR team, start and end times, items used and clinical information across all departments.

“Clinicians have clear documentation with Sunrise. It’s readable and at their fingertips, instead of having to go through the last entry on paper to find something as simple as vital signs, physicians orders, progress notes, clearances or test results, etcetera,” Lockett said. “Immediate, current information is clearly important to appropriately care for the patient in a timely manner.”

The system also makes it easier to capture an accurate list of charges and get bills out the door faster. Trinitas can now post patient procedures within 24 hours, where in some cases it could take up to 30 days.

“Charges are transparent and immediate…Staff no longer have to rely on their memory or outdated preference cards,”she said. “Being able to drop bills immediately is significant for Trinitas, and it helps us improve cashflow.”

Improving operational efficiency with surgical planning and perioperative staffing

Data can help hospitals determine how well they are using OR time and resources and, more specifically, how surgeons are using their block allocation. Also, drilling down to out-of-block time enhances decision making about how to optimize a surgeon, group or services block schedule.

Trinitas implemented Sunrise Surgical Logic, an operations planning and management platform, in the first quarter of 2017. The analytics and modeling tool helped achieve operational efficiencies in surgical planning by intelligently managing OR time and resources, block scheduling, on-time starts, OR efficiencies and replacing time-consuming manual calculation reports with automated, robust reporting with drill-down validations.

It also enabled Trinitas managers to more efficiently build staffing grids that better matched surgical demand. It helps hospitals assign surgical team members with the right skills to the right shift at the right time.

“We’re able to look at data—by hour and days of the week—to remodel staffing requirements going forward,” Lockett said. “We’ve made modifications based on actual historical utilization and now have predictability about what resources are or are not required. With dynamic staffing the OR was able to flex its staff based on the demand.”

To date, the staff planning scenarios provide Trinitas the ability to restructure resources to meet the demand. Automated and accurate report data helped save 50% of the time it took to extract and analyze staff planning. In total, perioperative productivity has improved by decreasing idle time by more than 4%.

“It’s better for forecasting, too…using the ‘what-if’ and simulation modeling tools you can put in your parameters and choose the scenarios that are the best fit for the institution and share that information with the management team before implementing anything,” Lockett said. “It’s very meaningful data, necessary for making business decisions.”

What’s next for Trinitas?

As of the first quarter of 2017, Trinitas operating rooms had a 71% utilization rate. The organization’s goal is to reach a target of 80% through incremental increases in volume with additional medical staff. Upon reaching this target, Trinitas has the potential to increase utilization by 9%. Lockett anticipates a continued strong partnership with Allscripts in perioperative care and beyond.
