Incident Definition
Any event which is not consistent with the routine operation of the University and that adversely affects or threatens to affect the well being of the employees, faculty members, research assistants, students, visitors, casual workers, or property of the University, regardless of whether an actual injury is
involved or not.
Incident Definition
An incident might result in: \
•Property damage
•Personal injury
•Near miss
Near Miss Incidents
• Are incidents that did not result in an exposure, injury, illness, or damage but had the potential to do so.
• All near miss incidents should be reported as evaluation can help in preventive measures and safe work environment.
• Motor vehicle accidents
• Equipment failure
• Flood/water leakage
Property Damage
• Needlestick / Sharps injuries
• Slip/trip/fall
• Back injuries
• Heat stress
• Biological/Chemical splash
• Physical trauma
• Electrical shock

Incident Reporting
Everyone is encouraged to report incidents
Reporting incidents is in no way used to place blame or to take punitive actions against any individual.

Why Reporting?
• Provide record of incident and record the facts.
• Provide base from which staff can further investigate.
• Analyze data to identify problems areas that need special attention.
• Identify how similar incidents can possibly be prevented in the future.
Why Reporting?
• Receive the needed medical care in case of bodily injury.
• Failing to report incidents may deprive employees from eligibility to medical and financial benefits of the Labor Law, Legislative Decree No. 136, which regulates workers' compensation and governs work related incidents in addition to death and disability benefits.

Procedure
Responsibilities
• The involved person shall report all incidents causing injury or property damage, or where there was the potential for such injury or property damage, to the supervisor immediately.
• Any person in proximity of the incident shall ensure that the injured person receives proper medical care, either at the Emergency Department (ED) or at the University Health Services (UHS), when needed.
Responsibilities
• It is the responsibility of the supervisor for employees and faculty members, and the supervisor of the area where the injury occurred for incidents related to students, contractors, visitors, and property damage to complete the Incident Report Form using the electronic format.
• In case of several levels of supervision, the head of the department is required to countersign the incident report, acknowledging awareness of the incident.
Responsibilities
• If the incident takes place outside regular working hours and the supervisor is not available, then the most senior person of the concerned department and the Protection Office shall complete and sign the Incident Report Form.
• The supervisor shall countersign the Incident Report Form on the next working day, acknowledging awareness of the incident.
Responsibilities
• It is the responsibility of the supervisor to investigate any incidents of unsafe conditions or resulting in personal injury. Depending on the severity of the incident, other personnel may be involved.
• Departments are required to retain copies of the completed Incident Report Forms for their records.
Responsibilities
In Case of Property Damage
• Report the damage to any of the EHSRM representatives, if available, immediately.
• Take photographs.
• Fill in an incident report and send it to EHSRM with the estimated cost and any other relevant documents.
