You will need your Social Security number. If no Social Security number enter 9999

If no Driver’s License number enter 9999.

Be sure you use an email address that you actually check.

Be sure you know the mailing address for the Office of Admissions for each school you need to

send your high school transcript. (See screenshot below for which information you will need)

You only want one copy per school. For each school you want to send a transcript to you will

have to add that college and their information.

Reason for Request is for College.

Do not use this to send a transcript for a scholarship. Ask at your high school the procedure for

requesting an official transcript.

Be sure you read the yellow box and put in your initials.

You get 5 free copies. After that you must pay $10.00 for each transcript.

Remember if you are sending to a NC college (public or private or community) you can do that

for free at CFNC.org.
