3. ALL CONSTRUCTION AND MATERIALS SHALL CONFORM WITH THE VIRGINIA DEPARTMENT OF

TRANSPORTATION ROAD AND BRIDGE SPECIFICATIONS DATED 2016, AND ROAD AND BRIDGE

STANDARDS, DATED 2016, AS AMENDED BY CONTRACT PROVISIONS AND THESE PLANS, AND CITY OF

RICHMOND RIGHT-OF-WAY EXCAVATION AND RESTORATION MANUAL.

4. LOCATE ALL EXISTING UTILITIES PRIOR TO THE START OF CONSTRUCTION. IF ANY UTILITY DIFFERS

THAN WHAT IS SHOWN ON THE PLAN, CONTACT THE ENGINEER IMMEDIATELY.

5. IF THE ELEVATIONS SHOWN ON THESE PLANS ARE FOUND TO BE DIFFERENT THAN FIELD CONDITIONS,

CONTACT THE ENGINEER IMMEDIATELY.

6. ACQUIRE ALL REQUIRED PERMITS PRIOR TO CONSTRUCTION. ALL FEES ASSOCIATED WITH PERMITS

SHALL BE PAID BY THE CONTRACTOR UNLESS OTHERWISE SPECIFIED.

7. THE CONTRACTOR SHALL BE AWARE OF THE POTENTIAL WEIGHT LIMIT ON THE EXISTING BRIDGE OVER

REEDY CREEK (SHOWN ON THE ATTACHED PLANS). EQUIPMENT EXCEEDING 20,000 LB OPERATING

WEIGHT SHALL NOT BE DRIVEN OVER EXISTING BRIDGE WITHOUT WRITTEN APPROVAL BY THE

ENGINEER OF RECORD.

8. EXISTING CONDITIONS SHOWN HEREON COMPILED FROM CITY OF RICHMOND GIS INFORMATION AND

FIELD SURVEY DATA FROM TIMMONS GROUP DATED AUGUST 31, 2018.

CONSTRUCTION NOTES

1. ALL WORK SHALL BE SUBJECT TO INSPECTION BY DPU INSPECTOR. NOTIFY APPROPRIATE CITY

OFFICIALS 72 HOURS PRIOR TO START OF WORK.

2. CALL "MISS UTILITY" OF CENTRAL VIRGINIA 1-800-552-7001 (TOLL FREE) PRIOR TO CONSTRUCTION.

VERIFY LOCATION AND ELEVATION OF ALL UNDERGROUND UTILITIES SHOWN ON THE PLANS IN

AREAS OF CONSTRUCTION PRIOR TO STARTING WORK. CONTACT ENGINEER IMMEDIATELY IF

LOCATION OR ELEVATION IS DIFFERENT FROM THAT SHOWN ON THE PLAN, IF THERE APPEARS TO

BE A CONFLICT, OR UPON DISCOVERY OF ANY UTILITY NOT SHOWN ON THE PLANS.

3. IF ENCOUNTERED, REMOVE UNSUITABLE MATERIAL AND REPLACE WITH FLOWABLE FILL OR AS

DIRECTED BY THE ENGINEER.

4. WITHIN THE PROJECT LIMITS, PROVIDE TEMPORARY DRAINAGE DURING CONSTRUCTION OR TO

RELIEVE AREAS THAT MAY CAUSE DAMAGE TO PATHS AS DIRECTED BY THE CITY OF RICHMOND

INSPECTOR.

5. CLEAN ALL DRAINAGE PIPES AND STRUCTURES OF DEBRIS AND ERODED MATERIAL AT ALL STAGES

OF CONSTRUCTION.

6. REFER TO THE LATEST VERSION OF THE VIRGINIA DEPARTMENT OF TRANSPORTATION ROAD AND

BRIDGE STANDARDS AND SPECIFICATIONS.

7. DO NOT BEGIN WORK UNTIL THE PERMIT INSPECTOR HAS BEEN NOTIFIED, A PRE-CONSTRUCTION

CONFERENCE HAS BEEN HELD AND MISS UTILITY HAS MARKED ALL UTILITIES.

8. CONCRETE AND ASPHALT CUTS SHALL BE AS CLEAN AND STRAIGHT AS POSSIBLE, WITH NO OUTLINE

DIMENSIONS LESS THAN 3 FEET WITHOUT SPECIAL APPROVAL OF THE PERMIT INSPECTOR. REFER

TO DETAIL ON SHEET 2A FOR PAVEMENT RESTORATION.

9. FINAL ACCEPTANCE BY THE CITY SHALL NOT BE MADE UNTIL ALL WORK SHOWN ON THE APPROVED

PLANS IS COMPLETE.

CONSTRUCTION SEQUENCE

1. CONSTRUCT PROJECT ACCORDING TO THE SEQUENCE OF CONSTRUCTION DESCRIBED

BELOW. THE CONTRACTOR MAY SUBMIT AN ALTERNATIVE SEQUENCE OF CONSTRUCTION

PRIOR TO THE START OF CONSTRUCTION FOR APPROVAL BY THE ENGINEER.

2. ACQUIRE ALL PERMITS PRIOR TO CONSTRUCTION. ALL FEES ASSOCIATED WITH PERMITS

SHALL BE PAID BY THE CONTRACTOR UNLESS OTHERWISE SPECIFIED.

3. CALL "MISS UTILITY" AT 1-800-552-7001 AT LEAST 48 HOURS PRIOR TO CONSTRUCTION.

CONTACT THE ENGINEER IMMEDIATELY IF LOCATION OR ELEVATION IS DIFFERENT FROM

THAT SHOWN ON THE PLAN. IF THERE APPEARS TO BE A CONFLICT, OR UPON DISCOVERY

OF ANY UTILITY NOT SHOWN ON THE PLANS.

4. PERFORM CONSTRUCTION SURVEY STAKEOUT FOR PROPOSED IMPROVEMENTS AND

CONSTRUCTION LIMITS. ALL SURVEYING OPERATIONS MUST BE PERFORMED BY A

VIRGINIA LICENSED SURVEYOR.

5. INSTALL EROSION AND SEDIMENT CONTROL MEASURES BEFORE COMMENCING ANY LAND

DISTURBING ACTIVITIES.

6. PROTECT EXISTING TREES LOCATED NEAR THE WORK ZONE WITH TREE PROTECTION.

7. REMOVE EXISTING HANDRAIL WHERE STILL INTACT.

8. SAW CUT AND DEMOLISH EXISTING PAVEMENT AS SHOWN ON PLANS.

9. CLEAR LOOSE RUBBLE AND DEBRIS FROM WORK AREA, INCLUDING BEHIND THE DRY

STACK RETAINING WALL. CLEAR TREES FROM WORK AREA AS SHOWN ON TYPICAL

SECTION. NO OTHER TREES SHALL BE REMOVED WITHOUT THE APPROVAL OF THE

ENGINEER.

10. REMOVE ABANDONED CLAY PIPE.

11. REPOINT EXISTING FIELD STONE WALL WITHIN PROJECT LIMITS.

12. RECONSTRUCT AND REPAIR THE DRY STACK RETAINING WALL USING EXISTING ON-SITE

DRY STACK ROCKS AND SUPPLEMENTAL DRY STACK ROCKS, AS NEEDED. PLACE

SMALLER STONES AND MORTAR TO FILL VOIDS BETWEEN STONES ALONG BACKSIDE OF

THE RETAINING WALL.

13. RE-ESTABLISH RIPRAP ALONG THE OUTER FACE OF THE WALL.

14. INSTALL FLOWABLE BACKFILL AS NECESSARY TO FILL VOIDS IN WASHED OUT PATH AREA.

15. INSTALL ONE LAYER OF STONE SUBBASE INTO TOP SURFACE OF FLOWABLE BACKFILL

WHILE THE FLOWABLE BACKFILL IS TACKY.

16. INSTALL AND GRADE STONE SUBBASE TO ACHIEVE APPROPRIATE CROSS-SLOPES OF

PAVEMENT.

17. INSTALL SURFACE PAVEMENT COURSE.

18. INSTALL NEW HANDRAIL ALONG THE EDGE OF THE PAVEMENT PATHWAY AS SHOWN ON

PLANS.

19. EXCAVATE ANY BUILT-UP DEBRIS UPSTREAM OF AND WITHIN THE CULVERT JUST NORTH

OF THE WASHED OUT AREA.

20. MAINTAIN ALL EROSION CONTROL DEVICES DURING CONSTRUCTION AND REMOVE ONCE

THE DISTURBED AREAS HAVE BEEN STABILIZED AND RELEASED BY THE CITY INSPECTOR.

MAINTENANCE (SEE MINIMUM STANDARDS FOR ADDITIONAL INFORMATION)

ALL EROSION AND SEDIMENT CONTROL MEASURES SHALL BE CHECKED DAILY AND AFTER EACH RUN-OFF

PRODUCING RAINFALL. THE FOLLOWING ITEMS SHALL BE CHECKED IN PARTICULAR:

1. CHECK THE SILT FENCE BARRIER FOR UNDERMINING OR DETERIORATION OF THE FABRIC. REMOVE SEDIMENT

WHEN THE LEVEL OF SEDIMENT DEPOSITION REACHES HALF WAY TO THE TOP OF THE BARRIER.

2. CHECK THE SEEDING AREAS TO ENSURE THAT A STAND OF GRASS IS MAINTAINED. FERTILIZE AND RESEED AS

NEEDED.

3. CHECK THE CONSTRUCTION ENTRANCE PERIODICALLY SO IT IS MAINTAINED IN A CONDITION WHICH WILL

PREVENT TRACKING OR FLOW OF MUD ONTO PUBLIC RIGHTS-OF-WAY. ALL MATERIALS SPILLED, DROPPED,

WASHED, OR TRACKED FROM VEHICLES ONTO ROADWAYS OR INTO STORM DRAINS MUST BE REMOVED

IMMEDIATELY.

4. CHECK INLET PROTECTION AND CHECK DAM MEASURES FOR NECESSARY REPAIRS AS PER THE HANDBOOK

REQUIREMENTS. ANY SEDIMENT THAT HAS ACCUMULATED MUST BE REMOVED.

CONFERENCE, ONE WEEK PRIOR TO THE COMMENCEMENT OF LAND DISTURBING ACTIVITY, AND ONE WEEK

PRIOR TO THE FINAL INSPECTION.

ES-3:ALL EROSION AND SEDIMENT CONTROL MEASURES ARE TO BE PLACED PRIOR TO OR AS THE FIRST STEP IN

EROSION AND SEDIMENT CONTROL NARRATIVE

PROJECT DESCRIPTION

THE PURPOSE OF THIS PROJECT IS TO MAKE REPAIRS AND IMPROVEMENTS TO A SECTION OF PEDESTRIAN PATH IN

FOREST HILL PARK IN THE CITY OF RICHMOND. THE PROJECT WILL REPLACE A WASHED-OUT SECTION OF PATH

AND INTRODUCE NON-ERODIBLE BASE MATERIALS TO PREVENT FUTURE WASHOUTS. THESE IMPROVEMENTS WILL

NOT REQUIRE UTILITY RELOCATIONS AND WILL NOT REQUIRE ANY ACQUISITION OF PERMANENT OR TEMPORARY

EASEMENTS. THE TOTAL LAND DISTURBANCE FOR THIS PROJECT IS APPROXIMATELY 0.21 ACRES.

EXISTING SITE CONDITIONS

THE SITE CURRENTLY CONSISTS OF A SECTION OF ASPHALT PATH WHICH HAS BEEN ERODED AND WASHED OUT,

EXPOSING UNDERLYING SOIL AND BEDROCK. THE SURROUNDING TOPOGRAPHY OF THE PROJECT SITE CONSISTS

OF STEEP, WOODED SLOPES SURROUNDING THE PATH.

ADJACENT PROPERTY

ALL ADJACENT PROPERTY IS FORESTED PARK LAND THAT IS MANAGED BY CITY OF RICHMOND PARKS &

RECEREATION.

SOILS

IMMEDIATELY ADJACENT TO THE PROJECT SITE, THE PREDOMINANT SOIL IS CLASSIFIED AS

"WATEREE-WEDOWEE-ROCK OUTCROP COMPLEX, 45 TO 60 PERCENT SLOPES". THE PREDOMINANT SOIL

CLASSIFICATION NEAR AND IN THE CREEK IS "JOHNSTON MUCKY LOAM, 0 TO 3 PERCENT SLOPES, FREQUENTLY

FLOODED".

OFF-SITE AREAS

NO OFF-SITE DISTURBANCE IS PLANNED.

CRITICAL EROSION AREAS

CRITICAL AREAS AREA AS FOLLOWS:

1. EXISTING STREAM AREAS. CONTRACTOR SHALL TAKE GREAT CARE TO AVOID ANY SEDIMENT DEPOSIT IN

REEDY CREEK. WORK NEAR THE STREAM SHALL BE AS QUICK AND EFFICIENT AS POSSIBLE. CONTRACTOR

SHALL LIMIT, TO THE GREATEST EXTENT POSSIBLE, THE LENGTH OF TIME WORKING NEAR THE CREEK.

2. AREAS BEHIND THE DESTROYED DRY STACK RETAINING WALL.

3. AREAS DIRECTLY UNDERNEATH THE PROPOSED PATH RECONSTRUCTION.

EROSION & SEDIMENT CONTROL MEASURES

UNLESS OTHERWISE INDICATED, ALL VEGETATIVE AND STRUCTURAL EROSION AND SEDIMENT CONTROL

PRACTICES SHALL BE CONSTRUCTED AND MAINTAINED ACCORDING TO THE MINIMUM STANDARDS AND

SPECIFICATIONS OF THE VIRGINIA EROSION AND SEDIMENT CONTROL HANDBOOK. THE MINIMUM STANDARDS OF

THE VESCH SHALL BE ADHERED TO UNLESS OTHERWISE WAIVED OR APPROVED BY A VARIANCE GRANTED BY THE

CITY OF RICHMOND. ALL MEASURES SHALL BE MAINTAINED FOR THE ENTIRETY OF THE PROJECT AND SHALL NOT

BE REMOVED UNTIL APPROVED BY THE CITY. THE CONTRACTOR SHALL DETERMINE A STOCKPILE AREA (IF

REQUIRED), IF AREA IS OUTSIDE OF THE LIMITS OF DISTURBANCE AS SHOWN IN THESE PLANS, THE LOCATION

MUST BE SUBMITTED TO AND APPROVED BY THE CITY OF RICHMOND PRIOR TO CONSTRUCTION.

MINIMUM STANDARDS

MS-1: APPLY PERMANENT SOIL STABILIZATION TO DENUDED AREAS WITHIN SEVEN DAYS AFTER FINAL

GRADE IS REACHED ON ANY PORTION OF THE SITE. APPLY TEMPORARY SOIL STABILIZATION WITHIN

SEVEN DAYS TO DENUDED AREAS THAT MAY NOT BE AT FINAL GRADE BUT WILL REMAIN DORMANT

(UNDISTURBED) FOR LONGER THAN 14 DAYS. APPLY PERMANENT STABILIZATION TO AREAS THAT

ARE TO BE LEFT DORMANT FOR MORE THAN ONE YEAR.

MS-2: STABILIZE OR PROTECT TEMPORARY SOIL STOCKPILES WITH SEDIMENT TRAPPING MEASURES.

PROVIDE TEMPORARY PROTECTION AND PERMANENT STABILIZATION OF ALL SOIL STOCKPILES ON

SITE AS WELL AS SOIL TRANSPORTED FROM THE PROJECT SITE.

MS-3: ESTABLISH A PERMANENT VEGETATIVE COVER ON DENUDED AREAS NOT OTHERWISE

PERMANENTLY STABILIZED. PERMANENT VEGETATION SHALL NOT BE CONSIDERED ESTABLISHED

UNTIL A GROUND COVER IS ACHIEVED THAT IN THE OPINION OF THE ARCHITECT/ENGINEER, IS

UNIFORM, MATURE ENOUGH TO SURVIVE AND WILL INHIBIT EROSION.

MS-4: CONSTRUCT SEDIMENT BASINS AND TRAPS, PERIMETER DIKES, SEDIMENT BARRIERS AND OTHER

MEASURES INTENDED TO TRAP SEDIMENT AS A FIRST STEP IN ANY LAND-DISTURBING ACTIVITY AND

MAKE THESE MEASURES FUNCTIONAL BEFORE UP-SLOPE LAND DISTURBANCE OR TIMBERING

TAKES PLACE.

MS-5: APPLY STABILIZATION MEASURES TO EARTHEN STRUCTURES SUCH AS DAMS, DIKES AND

DIVERSIONS IMMEDIATELY AFTER INSTALLATION.

MS-6: CONTROL SURFACE RUNOFF FROM DISTURBED AREAS THAT IS COMPRISED OF FLOW FROM

DRAINAGE AREAS GREATER THAN OR EQUAL TO THREE ACRES TO ACCOMMODATE THE

ANTICIPATED SEDIMENT LOADING FROM THE LAND- DISTURBING ACTIVITY. THE OUTFALL DEVICE

OR SYSTEM DESIGN SHALL TAKE INTO ACCOUNT THE TOTAL DRAINAGE AREA.

MS-7: CONSTRUCT CUT AND FILL SLOPES IN A MANNER THAT WILL MINIMIZE EROSION. PROVIDE SLOPES

THAT ARE FOUND TO BE ERODING EXCESSIVELY WITHIN ONE YEAR OF PERMANENT STABILIZATION

WITH ADDITIONAL SLOPE STABILIZING MEASURES UNTIL THE PROBLEM IS CORRECTED.

MS-8: CONCENTRATED RUNOFF SHALL NOT FLOW DOWN CUT OR FILL SLOPES UNLESS CONTAINED WITHIN

AN ADEQUATE TEMPORARY OR PERMANENT CHANNEL, FLUME OR SLOPE DRAIN STRUCTURE.

MS-9: WHENEVER WATER SEEPS FROM A SLOPE FACE, PROVIDE ADEQUATE DRAINAGE OR OTHER

PROTECTION.

MS-10: PROTECT ALL STORM SEWER INLETS THAT ARE MADE OPERABLE DURING CONSTRUCTION SO

THAT SEDIMENT-LADEN WATER CANNOT ENTER THE CONVEYANCE SYSTEM WITHOUT FIRST BEING

FILTERED OR OTHERWISE TREATED TO REMOVE SEDIMENT.

MS-11: BEFORE STORMWATER CONVEYANCE CHANNELS ARE MADE OPERATIONAL, PROVIDE ADEQUATE

OUTLET PROTECTION AND ANY REQUIRED TEMPORARY OR PERMANENT CHANNEL LINING IN BOTH

THE CONVEYANCE CHANNEL AND THE RECEIVING CHANNEL.

MS-12: WHEN WORK IN A LIVE WATERCOURSE IS PERFORMED, TAKE PRECAUTIONS TO MINIMIZE

ENCROACHMENT, CONTROL SEDIMENT TRANSPORT AND STABILIZE THE WORK AREA TO THE

GREATEST EXTENT POSSIBLE DURING CONSTRUCTION. PROVIDE NON-ERODABLE MATERIAL FOR

THE CONSTRUCTION OF CAUSEWAYS AND COFFERDAMS. EARTHEN FILL MAY BE USED FOR THESE

STRUCTURES IF ARMORED BY NON-ERODABLE COVER MATERIALS.

MS-13: WHEN A LIVE WATERCOURSE MUST BE CROSSED BY CONSTRUCTION VEHICLES MORE THAN TWICE

IN ANY SIX-MONTH PERIOD, PROVIDE A TEMPORARY STREAM CROSSING CONSTRUCTED OF

NON-ERODABLE MATERIAL.

MS-14: MEET ALL APPLICABLE FEDERAL, STATE AND LOCAL REGULATIONS PERTAINING TO WORKING IN OR

CROSSING LIVE WATERCOURSES.

MS-15:STABILIZE THE BED AND BANKS OF A WATERCOURSE IMMEDIATELY FOLLOWING AFTER WORK IN

THE WATERCOURSE IS COMPLETED.

MS-16: UNDERGROUND UTILITY LINES SHALL BE INSTALLED IN ACCORDANCE WITH THESE STANDARDS IN

ADDITION TO OTHER APPLICABLE CRITERIA:

1. OPEN NO MORE THAN 500 LINEAR FEET OF TRENCH AT ONE TIME.

2. PLACE EXCAVATED MATERIAL ON THE UPHILL SIDE OF TRENCHES.

3. FILTER EFFLUENT FROM DEWATERING OPERATIONS OR PASS THROUGH AN APPROVED

SEDIMENT TRAPPING DEVICE, OR BOTH, AND DISCHARGE IN A MANNER THAT DOES NOT

ADVERSELY AFFECT FLOWING STREAMS OR OFFSITE PROPERTY.

4. ACCOMPLISH RESTABILIZATION IN ACCORDANCE WITH THESE REGULATIONS.

5. COMPLY WITH ALL APPLICABLE SAFETY REGULATIONS.

MS-17: WHERE CONSTRUCTION VEHICLE ACCESS ROUTES INTERSECT PAVED PUBLIC ROADS, MAKE

PROVISIONS TO MINIMIZE THE TRANSPORT OF SEDIMENT BY VEHICULAR TRACKING ONTO THE

PAVED SURFACE. WHERE SEDIMENT IS TRANSPORTED ONTO A PUBLIC ROAD SURFACE, CLEAN

THE ROAD THOROUGHLY AT THE END OF EACH DAY. REMOVE SEDIMENT FROM THE ROADS BY

SHOVELING OR SWEEPING AND TRANSPORT TO A SEDIMENT CONTROL DISPOSAL AREA. STREET

WASHING SHALL BE ALLOWED ONLY AFTER SEDIMENT IS REMOVED IN THIS MANNER.

MS-18: REMOVE ALL TEMPORARY EROSION AND SEDIMENT CONTROL MEASURES WITHIN 30 DAYS AFTER

FINAL SITE STABILIZATION OR AFTER THE TEMPORARY MEASURES ARE NO LONGER NEEDED,

UNLESS OTHERWISE AUTHORIZED BY THE LOCAL AUTHORITY HAVING JURISDICTION.

PERMANENTLY STABILIZE TRAPPED SEDIMENT AND THE DISTURBED SOIL AREAS RESULTING

FROM THE DISPOSITION OF TEMPORARY MEASURES TO PREVENT FURTHER EROSION AND

SEDIMENTATION.

4. EROSION AND SEDIMENT CONTROLS SHALL BE MAINTAINED SO THAT SEDIMENT CARRYING

RUNOFF FROM THE SITE WILL NOT ENTER STORM DRAINAGE FACILITIES.

5. EROSION AND SEDIMENT CONTROLS SHALL BE MAINTAINED UNTIL THE DISTURBED AREA IS

STABILIZED.

6. PROPERTIES ADJOINING THE SITE SHALL BE KEPT CLEAN OF MUD OR SILT CARRIED FROM THE

8. STABILIZATION MEASURES SHALL BE APPLIED TO EARTHEN STRUCTURES SUCH AS DAMS,

9. DURING CONSTRUCTION OF THE PROJECT, SOIL STOCK PILES SHALL BE STABILIZED OR

1. SCHEDULE A PRE-CONSTRUCTION MEETING AND GIVE A 48-HOUR NOTIFICATION OF THE

MEETING. THE CERTIFIED RESPONSIBLE LAND DISTURBER (CRLD) MUST BE PRESENT AT THE

MEETING.

2. PROVIDE MINIMAL CLEARING TO INSTALL SILT FENCE, TREE PROTECTION, AND OTHER

BOUNDARY MEASURES ASSOCIATED WITH INITIAL CLEARING. ALL OTHER MEASURES,

INCLUDING INLET PROTECTION AND OUTLET PROTECTION WHERE REQUIRED, SHALL BE

INSTALLED WHEN PRACTICAL DURING CLEARING OPERATIONS. TAKE SPECIAL CARE TO KEEP

SOIL BUILD-UP FROM ENTERING THE EXISTING PATHS, ADJOINING ROADS, AND ALL OTHER
