2. DO NOT CONDUCT MAIN LINE VACUUM TESTING UNTIL THE TEMPORARY

AT NO ADDITIONAL COST THE CONTRACTOR IS RESPONSIBLE FOR REPAIRING ANY DAMAGE TO EXISTING FACILITIES TO A CONDITION EQUAL OR BETTER THAN EXISTING, ABOVE OR BELOW GROUND, THAT MAY OCCUR AS A RESULT OF THE WORK PERFORMED BY THE CONTRACTOR. 4.THE THE REQUIREMENTS OF THE FLORIDA DEPARTMENT OF ENVIRONMENTAL PROTECTION & THE CITY OF MARATHON REGULATIONS SHALL GOVERN ALL UTILITY WORK.  WHERE A CONFLICT EXISTS IN THE REQUIREMENTS OF A REFERENCED MATERIAL OR INSTALLATION STANDARD, THE REQUIREMENTS OF THE FLORIDA DEPARTMENT OF ENVIRONMENTAL PROTECTION SHALL HAVE PRECEDENCE.  WHERE THE REQUIREMENTS OF A STATE OR LOCAL AGENCY HAVING JURISDICTION ARE MORE STRINGENT, THOSE REQUIREMENTS SHALL PREVAIL. 5.UTILITY UTILITY LOCATIONS ARE APPROXIMATE AND BASED ON AVAILABLE INFORMATION.  CONTRACTOR IS RESPONSIBLE FOR FIELD VERIFYING & MARKING LIMITS OF CONSTRUCTION USING THE SERVICE OF A LOCATING COMPANY & CONTACTING THE UTILITY COMPANIES AS NECESSARY PRIOR TO THE START OF CONSTRUCTION. 6.CONTRACTOR CONTRACTOR SHALL PROVIDE ACCURATE AS-BUILT OF PROJECT. 7.SOIL SOIL COMPACTION SHALL BE BASED UPON THE MODIFIED PROCTOR (AASHTO T-180), GENERALLY, 95% OF MAXIMUM PROCTOR OUTSIDE PAVED AREA AND 98% OF MAXIMUM PROCTOR WITHIN PAVED AREA SHALL BE OBTAINED. 8.ALL PIPE ALL PIPE SHALL BE BACK FILLED TO MINIMUM SPECIFICATION COVER AND DEPTH BEFORE IT SHALL BE PLACED INTO SERVICE BY THE OWNER.  36” MINIMUM COVER SHALL BE REQUIRED OVER FORCE MAIN SHOULD THERE BE A CONFLICTING SPECIFICATION, THE 36” COVER COVER SHALL HAVE PRECEDENCE.  9.ALL POTABLE ALL POTABLE WATER REQUIRED FOR CONSTRUCTION PURPOSES SHALL BE METERED BY APPROVED METER WITH APPROVED BACKFLOW-PREVENTION DEVICES AND PAID FOR BY THE CONTRACTOR. 10.SEWER/FORCE SEWER/FORCE MAINS MUST BE IN SEPARATE TRENCHES FROM POTABLE WATER AND RECLAIMED WATER MAINS WITH A MINIMUM HORIZONTAL SEPARATION OF TEN (10) FEET CLEAR (EDGE TO EDGE) TO POTABLE WATER MAINS.  AT CROSSINGS A VERTICAL SEPARATION OF NOT LESS THAN 1.5 FEET (18 INCHES) FROM THE CROWN OF THE SEWER OR FORCE MAIN TO THE INVERT OF THE WATER MAIN.  IF LESS, OR IF THE WATER MAIN PASSES UNDER THE SEWER OR FORCE MAIN, THE SEWER OR FORCE MAIN SHALL BE ENCASED IN CONCRETE OR PLACED IN A PVC SLEEVE FOR 20 FEET CENTERED ON THE CROSSING. 11.CONTRACTOR CONTRACTOR SHALL OBTAIN ALL APPLICABLE PERMITS PRIOR TO COMMENCING WORK.  12.THE THE CONTRACTOR SHALL CONTACT ALL CONCERNED UTILITIES, INCLUDING THE CITY OF MARATHON, AND THE ENGINEER AT LEAST 48 HOURS IN ADVANCE OF CONSTRUCTION OPERATIONS.  ALL WORK AND TESTING SHALL BE WITNESSED BY A REPRESENTATIVE OF THE CITY OF MARATHON. TESTING NOT WITNESSED BY THE CITY OF MARATHON MAY CAUSE REJECTION OF WORK. 13.CONTRACTOR CONTRACTOR SHALL RESTORE ALL EXISTING STRUCTURES, INCLUDING BUT NOT LIMITED TO PAVERS, PAVEMENT, PLANTERS, AND CONCRETE TO PRECONSTRUCTION CONDITIONS.  14.THE THE CONTRACTOR SHALL COMPLY WITH ALL RULES AND REGULATIONS OF THE STATE, COUNTY, AND CITY AUTHORITIES REGARDING CLOSING OR RESTRICTING THE USE OF PUBLIC STREETS OR HIGHWAYS. 15.PIPE SHALL PIPE SHALL BE LAID TRUE TO LINE AND GRADE WITH BELL IN DRY TRENCH ON WELL COMPACTED BED. 16.CONTRACTOR CONTRACTOR SHALL PROVIDE MAGNETIC LOCATION TAPE (6" DEEP) OVER PVC PIPE. 17.ALL GRASS ALL GRASS AREAS DISTRIBUTED BY CONSTRUCTION SHALL BE SEEDED OR RECEIVE SOD. 18.ANY ANY DISCREPANCIES ON THE DRAWINGS SHALL BE BROUGHT TO THE ATTENTION OF THE ENGINEER BEFORE COMMENCING WORK.  19.COORDINATE COORDINATE WORK WITH OWNER TO MINIMIZE DISRUPTION TO THE SANITARY SEWER SERVICE.  SANITARY SEWER SHALL REMAIN IN SERVICE THROUGHOUT THE CONSTRUCTION PROJECT. 20.MAXIMUM MAXIMUM INFILTRATION AND EXFILTRATION FOR PVC GRAVITY SEWERS SHALL BE AS FOLLOWS:  150 GALLONS/INCH DIAMETER/MILE/DAY.  TEST SHALL BE ACCEPTABLE PRIOR TO CERTIFICATION OF COMPLETION. 21.NO SEWER NO SEWER SHALL BE CLOSER THAN 5' TO AN EXISTING FKEC POWER POLE WITHOUT AUTHORIZATION FROM THE FLORIDA KEYS ELECTRIC COOPERATIVE ASSOCIATION, INC. 22.ALL ALL WASTEWATER WITHIN PROPERTY LIMITS SHALL BE DIRECTED TO THE PROPOSED SANITARY SEWER COLLECTION SYSTEM.  ALL EXISTING TREATMENT SYSTEMS, SEPTIC TANKS, AND CESSPITS SHALL BE PUMPED BY A LICENSED SLUDGE HAULER, BACKFILLED, AND FOREVER ABANDONED. 23.CONTRACTOR CONTRACTOR SHALL COORDINATE TESTING WITH ENGINEER & THE CITY OF MARATHON FOR TESTING OF SYSTEM.  NO SEWAGE SHALL ENTER THE PROPOSED SYSTEM UNTIL CLEARED FOR USE BY THE DEPARTMENT OF ENVIRONMENTAL PROTECTION. 24.CONTRACTOR CONTRACTOR SHALL COORDINATE ANY SHUT DOWNS WITH THE CITY OF MARATHON. 25.NO NO DEWATERING IS ANTICIPATED FOR THE PROPOSED CONSTRUCTION. IN THE EVENT DEWATERING IS REQUIRED THE CONTRACTOR SHALL APPLY FOR ALL APPLICABLE WATER USE PERMITS AT THAT TIME. 26.CONTRACTOR CONTRACTOR SHALL BE RESPONSIBLE FOR PREPARING A MAINTENANCE OF TRAFFIC CONTROL PLAN (TCP). MOT PLAN SHALL BE SUBMITTED TO THE CITY OF MARATHON FOR APPROVAL. MOT PLAN MUST BE SIGNED & SEALED BY FLORIDA REGISTERED PROFESSIONAL ENGINEER. THE TCP SHALL BE CONSISTENT WITH FDOT STANDARD INDEX 600 SERIES. 27.RECORD RECORD DRAWINGS WILL BE THE CONTRACTOR'S OBLIGATION AND SHALL BE SURVEYED IN THE FIELD BY A LICENSED SURVEYOR, REGISTERED IN THE STATE OF FLORIDA, IN COMPLIANCE WITH THE CITY OF MARATHON STANDARDS. THE RECORD DRAWINGS SHALL ALSO INCLUDE AS-BUILT CONSTRUCTION INFORMATION FOR ALL DRAINAGE SYSTEM CONTROL STRUCTURES, DETENTION/RETENTION SYSTEM BOTTOM AREAS. SURVEY DATA WILL BE REQUIRED EVERY 100' FOR PERIMETER BERM ELEVATIONS, TOE ELEVATIONS AND SIDE SLOPES. IN ADDITION TO THE SIGNED AND SEALED RECORD SURVEY, THE CONTRACTOR SHALL SUBMIT AN AUTOCAD DRAWING VERSION 2010 OR NEWER TO THE ENGINEER SHOWING ALL FIELD CHANGES IDENTIFIED ON THE RECORD SURVEY INCORPORATED INTO THE ENGINEER'S ORIGINAL AUTOCAD DATA BASE. IN THE INTEREST OF HELPING TO MAINTAIN DATA INTEGRITY, DDAI CAN PROVIDE THE CONTRACTOR PROFESSIONAL SERVICES TO IMPORT THE SURVEYOR'S RECORD SURVEY INFORMATION IN THE DDAI ORIGINAL AUTOCAD DATA BASE AND PRODUCE THE FINAL RECORD DRAWINGS. DDAI WILL PROVIDE A PROPOSAL FOR SAID WORK UPON THE CONTRACTOR'S REQUEST. 28.ALL SIGNAGE ALL SIGNAGE AND PAVEMENT MARKINGS SHALL BE IN ACCORDANCE WITH FDOT STANDARDS FOR ROAD AND BRIDGE CONSTRUCTION LATEST EDITION, FDOT ROADWAY AND FDOT DESIGN STANDARDS LATEST EDITION, AND THE MANUAL OF UNIFORM TRAFFIC CONTROL TRANSPORTATION ENGINEERING DESIGN STANDARDS. ALL ONSITE PAVEMENT MARKINGS SHALL BE PERMANENT REFLECTIVE TRAFFIC PAINT MEETING FDOT REQUIREMENTS. ALL HANDICAP REQUIREMENTS, BOTH SIGNAGE AND STRIPING WILL BE PER THE LATEST ADA CRITERIA USING THE APPROPRIATE COLORED TRAFFIC PAINT. 29.24" STOP 24" STOP BARS SHALL BE CONSTRUCTED PER FDOT INDEX 17346 AND BE LOCATED 4' FROM THRU EDGE OF PAVEMENT UNLESS OTHERWISE SHOWN. 30.ALL ALL CLEAN-OUTS AND VALVES WILL HAVE A CONCRETE PAD AND COVER FLUSH WITH PROPOSED GRADE. CLEARING AND EROSION CONTROL NOTES 1.EROSION CONTROL IS THE RESPONSIBILITY OF THE CONTRACTOR. THE ENGINEER ASSUMES NO RESPONSIBILITY FOR ACCURACY. PRIOR TO THE START OF ANY CONSTRUCTION ACTIVITY, IT SHALL BE THE CONTRACTOR'S RESPONSIBILITY TO NOTIFY THE VARIOUS UTILITIES AND TO MAKE THE NECESSARY ARRANGEMENTS FOR ANY RELOCATIONS OF THESE UTILITIES WITH THE OWNER OF THE UTILITY. THE CONTRACTOR SHALL EXERCISE CAUTION WHEN CROSSING ANY UNDERGROUND UTILITY, WHETHER SHOWN ON THE PLANS OR LOCATED BY THE UTILITY COMPANY. ALL UTILITIES WHICH INTERFACE WITH THE PROPOSED CONSTRUCTION SHALL BE RELOCATED BY THE RESPECTIVE UTILITY COMPANIES, AND THE CONTRACTOR SHALL COOPERATE WITH THE UTILITY COMPANIES DURING RELOCATION OPERATIONS. ANY DELAY OR INCONVENIENCE CAUSED TO THE CONTRACTOR BY THE VARIOUS UTILITIES SHALL BE INCIDENTAL TO THE CONTRACT AND NO EXTRA COMPENSATION WILL BE ALLOWED. 7.THE CONTRACTOR WILL BE RESPONSIBLE FOR MAKING A VISUAL INSPECTION OF THE SITE AND WILL BE RESPONSIBLE FOR THE DEMOLITION AND REMOVAL THE CONTRACTOR WILL BE RESPONSIBLE FOR MAKING A VISUAL INSPECTION OF THE SITE AND WILL BE RESPONSIBLE FOR THE DEMOLITION AND REMOVAL OF ALL UNDERGROUND AND ABOVE GROUND. STRUCTURES THAT WILL NOT BE INCORPORATED WITH THE NEW FACILITIES. SHOULD ANY DISCREPANCIES EXIST WITH THE PLANS, THE CONTRACTOR SHALL BE RESPONSIBLE FOR CALLING THE OWNER AND REQUESTING A CLARIFICATION OF THE PLANS PRIOR TO DEMOLITION. 8.DURING CONSTRUCTION, ALL STORM SEWER INLETS IN THE VICINITY OF THE PROJECT SHALL BE PROTECTED BY SEDIMENT TRAPS SUCH AS SECURED DURING CONSTRUCTION, ALL STORM SEWER INLETS IN THE VICINITY OF THE PROJECT SHALL BE PROTECTED BY SEDIMENT TRAPS SUCH AS SECURED SYNTHETIC BALES, SOD, STONE, ETC., AS IDENTIFIED IN THE EROSION CONTROL PLAN, WHICH SHALL BE MAINTAINED AND MODIFIED AS REQUIRED BY CONSTRUCTION PROGRESS. 9.ALL EROSION AND SILTATION CONTROL METHODS SHALL BE IMPLEMENTED PRIOR TO THE START OF CONSTRUCTION AND MAINTAINED UNTIL CONSTRUCTION IS ALL EROSION AND SILTATION CONTROL METHODS SHALL BE IMPLEMENTED PRIOR TO THE START OF CONSTRUCTION AND MAINTAINED UNTIL CONSTRUCTION IS COMPLETE. 10.WHEN CONSTRUCTION IS COMPLETED, THE RETENTION/DETENTION AREAS WILL BE RESHAPED, CLEANED OF SILT, MUD AND DEBRIS, AND RE-SODDED IN WHEN CONSTRUCTION IS COMPLETED, THE RETENTION/DETENTION AREAS WILL BE RESHAPED, CLEANED OF SILT, MUD AND DEBRIS, AND RE-SODDED IN ACCORDANCE TO THE PLANS IF NECESSARY. 11.CONTRACTOR IS TO PROVIDE EROSION CONTROL/SEDIMENTATION BARRIER (SYNTHETIC BALES OR SILTATION CURTAIN) TO PREVENT SILTATION OF ADJACENT CONTRACTOR IS TO PROVIDE EROSION CONTROL/SEDIMENTATION BARRIER (SYNTHETIC BALES OR SILTATION CURTAIN) TO PREVENT SILTATION OF ADJACENT PROPERTY, STREETS, STORM SEWERS, WATERWAYS, AND EXISTING WETLANDS.
