■ You receive your Cigna ID card after you enroll.

■ Keep your Cigna ID card with you at all times. Show it whenever you visit a health careprofessional or facility.

WORKING WITH YOUR DOCTOR

When you choose your Primary Care Physician (PCP) to be your personal physician, youestablish and develop a relationship that remains a reassuring part of your plan. Eachcovered member of your family can choose his or her own PCP.

■ The PCP serves as a “home base” for basic care – a source for advice and direction.

■ Your PCP also coordinates your care - from preventive checkups and routine medicalcare to specialized care and hospitalizations.

■ If required as a part of your plan, your PCP will also provide you with referrals tospecialists when necessary. See your plan materials to learn whether your plan requiresyou to have a referral before seeing a specialist.

■ If your PCP is part of a medical group, you may be required to see a specialist whopractices in that group for services to be covered. Check with your PCP or call CignaCustomer Service to learn whether this requirement applies to you.

■ Referral-free access to OB/GYNs – As part of your plan, you can visit a participatingOB/GYN at any time, without a referral, even if your plan requires referrals to othertypes of specialists. Please note: If your PCP is part of a medical group you may berequired to see an OB/GYN who practices in that group for services to be covered. Checkwith your PCP or call Cigna Customer Service to learn whether this requirement appliesto you.

■ You can change your PCP at any time, for any reason.

■ Vermont allows naturopathic doctors to be PCPs, depending on your plan you may beable to choose a naturopathic doctor who is licensed to practice in VT as your PCP. Thedoctor must be considered an in-network provider who is contracted with Cigna in VT tobe covered as a PCP.

■ When you see a naturopathic doctor as your PCP you’ll pay only the PCP copay if yourplan has copayments, or the visit will be subject to the (in-network) deductible and/orcoinsurance of the medical plan in which you are enrolled. The copay is the amount youpay toward an office visit. The coinsurance is the amount you pay after your plan beginsto pay.

As a Cigna customer, you have access to the Cigna LifeSOURCE TransplantNetwork®, a network of participating organ and tissue transplant centers. Thetransplant network includes over 600 transplant programs at more than 150independent transplant facilities that are nationally recognized for their clinicaloutcomes.

Each transplant facility is evaluated for minimum annual volumes, one-year graftand patient outcomes, time on the wait list, support services and “patient friendly”environments before it is included in the Cigna LifeSOURCE Transplant Network.Programs that meet or exceed Cigna’s national Guidelines for Network Inclusion areincluded as a Program of Excellence. Transplant programs that do not meet Cigna’snational Guidelines for Network Inclusion may be contracted as a SupplementalProgram. You will need to understand your benefits to know if you have coveragefor a transplant at a Supplemental Program. Cigna LifeSOURCE participants are managed by our in-depth transplant casemanagement unit. Pre-certification

Our goal is to help make sure that you have access to the appropriate care, in theappropriate setting. We have established a wide network of physicians and wecontinuously contract with new physicians to help make sure that you have accessto care from credentialed doctors.

Your plan may require that you choose a Primary Care Physician (PCP) foryourself and your covered dependents. Your PCP is your personal doctor who cancoordinate your medical care and keep your medical history. If your plan does notrequire you to choose a PCP, you can still choose a PCP or a personal physicianfor yourself and your covered dependents to help coordinate your care. Your firststop should be your PCP or personal physician. He or she can help determine ifyou need specialty care or hospitalization.

WHAT IS PRE-CERTIFICATION?

Pre-certification is a review process where Cigna nurses, physicians and/orpharmacists work with your doctor to determine:

■ Whether a procedure, treatment or service is a covered benefit.

■ What your benefits will be for a procedure, treatment or service if you use ahealth care professional who is not in the Cigna network.

HOW DOES THE PROCESS WORK?

Your plan may require pre-certification for hospital admissions and selectedoutpatient services. If your doctor is part of the Cigna network, then he or she isresponsible for contacting Cigna to get pre-certification. If you use a doctor whois not part of the Cigna network, then you are responsible for contacting Cigna torequest pre-certification. It is important for you to review your benefit plan orcontact Cigna at the number on your Cigna ID card to understand which servicesrequire pre-certification. When pre-certification is requested, a Cigna nurseevaluates the request using nationally recognized guidelines. These guidelines areconsistent with sound clinical principles and processes and have been developedwith involvement from actively practicing health care professionals. Cigna nursesdetermine what services are covered based on your plan benefits and using thenationally recognized guidelines. When guidelines do not exist, clinical resourcetools based on clinical evidence are used.

Anytime a Cigna nurse is unable to approve coverage for clinical reasons, thecase is referred to a Cigna physician who considers each case on an individualbasis. The Cigna physician may speak with your doctor to obtain additionalinformation. You and your doctor will be notified in writing if a request for pre-certification cannot be approved based on the information we received and yourplan benefits.

10 Pre-certification

WHEN DOES THE REVIEW OCCUR?

The review process can occur at three different times:

■ Prospective review is when Cigna receives a request before you receive care.Decisions are made within two business days of receiving all necessary information.You and your doctor will be notified verbally or electronically and by mail.

■ Concurrent review is when Cigna receives a request while you are receiving care or ina hospital, skilled nursing facility or rehabilitation facility. Determinations are madewithin one business day of receiving all necessary information. You and your doctorwill be notified verbally or electronically and by mail.

■ Retrospective review is when Cigna receives a request after you have received care.Determinations related to these services are made within thirty days after receiving allnecessary information. You and your doctor will be notified verbally or electronicallyand by mail.

If your situation requires that a determination be made right away, then Cigna willperform an urgent review. This determination will be completed within one business day.

Physicians determine coverage denials when clinical reasons are the rationale for thedenial. Denial letters will explain the reason for the decision and details on how to submitadditional information and/or proceed through the formal Appeals Process, should youdisagree with the coverage decision.

WHAT IS CASE MANAGEMENT?

Case Management is when a Cigna nurse provides you with assistance in coordinatingservices between your doctor and across different care settings, such as a hospital,rehabilitation facility and your home. The nurse will also assist you with identifyingavailable community resources for services that may not be covered by your benefit planor by providing you with health care information. 11Health Care Professional Compensation

Cigna is committed to keeping youinformed about matters related to yourhealth care plan. For that reason, weoffer the following description of theway that Cigna compensates health careprofessionals (physicians, hospitals andother health care practitioners andfacilities) that participate in ournetworks.

Cigna compensates health careprofessionals in ways that are intended toemphasize preventive care, promotequality care and ensure the appropriateand cost-effective use of coveredmedical services and supplies. Cignareinforces this philosophy throughutilization management decisions madeby its medical directors and medicalmanagement staff. Cigna employees areencouraged to promote appropriateutilization of covered health careservices and to discourageunderutilization.

The methods by which participatinghealth care professionals agree to becompensated are described generallyhere. The amount and type ofcompensation a health care professionalagrees to accept may vary dependingupon the type of plan. For example, ahospital may agree to accept less forservices provided to patients enrolled inan HMO plan than to patients enrolled inother types of plans. In addition, Cignamay attempt in various ways to promotethe use of participating health careprofessionals based upon quality andcost-effectiveness measures whileassuring quality and access to coveredservices and supplies.

DISCOUNTED FEE FORSERVICE:

Payment for services is based on anagreed-upon discounted amount from thehealth care professional’s bill.

CAPITATION:By mutual agreement, networkphysicians, physician/hospitalorganizations (PHOs) or other healthcare professional groups are paid a fixedamount (capitation) at regular intervalsfor each individual assigned to thephysician, PHO or other health careprofessional groups, whether or notservices are provided. This paymentcovers physician and/or, whereapplicable, hospital or other servicescovered under the benefits plan. Healthcare professional groups and PHOs mayin turn compensate health careprofessionals using a variety of methods.

Capitation can offer health careprofessionals a predictable income,encourage physicians to keep peoplewell through preventive care, eliminatethe financial incentive to provideservices that will not benefit the patientand reduce paperwork.

Cigna may also work with third partiesthat provide network managementservices. Under these arrangements,Cigna pays the third party a fixedmonthly amount per individual for theseservices. Health care professionals arecompensated by the third party forservices provided to Cigna planparticipants from the fixed amount.Compensation arrangements are agreedupon by the third parties and theircontracted health care professionals, andmay include discounted fee for serviceand capitation.

Some health care professionals and thirdparties that provide networkmanagement services may participate ina risk-sharing arrangement with Cigna;they agree on a target amount for thecost of certain services and share all orsome of the amount by which costs areover or under the target. Services aremonitored using criteria that mayinclude accessibility, quality of care,customer satisfaction and appropriateand cost-effective use of medicalservices and supplies.

SALARY:Physicians and other health careprofessionals who are employed to workin Cigna's Arizona staff-model medicalfacility are paid a salary. The salary isdecided in advance each year and isguaranteed regardless of the servicesprovided. Physicians are eligible for abonus at the end of the year based onperformance, which is evaluated usingcriteria that may include quality of care,quality of service and appropriate andcost-effective use of medical servicesand supplies.

BONUSES AND INCENTIVES:Some health care professionals mayreceive additional payments based ontheir performance, which is measuredusing criteria that may include quality ofcare, quality of service and appropriateand cost-effective use of medicalservices and supplies. Health careprofessionals may also receive financialand/or nonfinancial incentives thatpromote utilization of cost-effectiveparticipating health care professionals(such as hospitals, labs, specialists andvendors) and covered drugs and supplies.

PER DIEM:A specific amount is paid to a hospitalper day for all health care received. Theper diem payment may vary by type ofservice and length of stay and thepayment may in some cases be greaterthan the hospital’s normal billed charges.

CASE RATE:A specific amount is paid for all healthcare received in the hospital for a givenhospital stay (such as for a normalmaternity delivery).
