An additional short Supplementary Application is needed for this Board. Someone will contactyou about this soon. If you have any questions, please call (503) 846-3081.

How did you hear about the Board/s you are applying for? *

County website

Why are you interested in serving on the Board/s you are applying for?

I am a recently retired service (U.S. Army) member with a desire to get involved in local government.

Are you interested in other volunteer opportunities?

Yes No

If you are interested in other opportunities, may we contact you?

Yes No

What education, work experience, volunteer experience or life experience do you bring?

I served in the military for 25 years (4yrs USMC, 5yrs Oregon National Guard, 16yrs U.S. Army) and Ihave a degree in Social Science with a concentration in foreign affairs. I have served as leader on smallteams assigned to U.S. embassies responsible for creating relationships with local authorities and groupsin order to achieve U.S. goals in foreign countries.

Describe your understanding of the concerns and issues facing the Board/s you wish toserve on.

It looks as if the board is primarily concerned with providing basic services for the elderly and veterans.coordinating with other agencies and groups in order to assist these members of the community inreceiving the benefits they deserve as well improving quality of life for them and their families. This likelyentails partnerships with state and federal agencies as well as organizations such as the VFW or DAV,and Pioneer Potlatch or meals on wheels. There is also a very good chance that local employers such asNike and Intel either could donate to the board or provide volunteers from their community engagementefforts.

List any potential conflict of interest between your public and private service that mightresult from your appointment to a Board/s you are applying for.

There should be no conflict of interest as Intel encourages their employees to volunteer within thecommunities that we are apart of. Potentially my shift could change requiring me to make arrangementsfor meeting attendance.

Why are you interested in serving on the Board/s you are applying for?

I have spent a large portion of my adult life working in government and enjoy public policy work. Currently,I have taken time away from paid work to help care for my mother, but she is returning to health and I amlooking for ways to continue to serve my community. I am interested in the boards listed because I havepersonal connection to each one of them. I am a spouse of a veteran, I support and use our park and reccenters, I live in unincorporated Washington County and have served on a road expansion cmte, andfinally I served as chief of staff to a state representative on health care and have become intimately awareof challenges facing our community by working with constituents.

Are you interested in other volunteer opportunities?

Yes No

If you are interested in other opportunities, may we contact you?

What education, work experience, volunteer experience or life experience do you bring?

I have a bachelor of arts in cultural and historical studies. I have previously worked as a policy advisor to astate representative as well as working as a chief of staff. These jobs allowed me to staff legislators onseveral committees including, health care, education, higher education and workforce development, audits and IT, general government ways and means, and human services. I have also served as a mentorin our local high schools, I have served on non profit boards. Finally, I have lived in Washington County forthe past 16 years and have raised three children here. Describe your understanding of the concerns and issues facing the Board/s you wish toserve on.

I believe each of the boards I listed have unique concerns and would be interesting in different ways.URMDAC - Road maintenance in Washington County's urban roads is a continual issue for many peopleliving on the outskirts of our cities. Ensuring we are pursuing the most cost effective solutions probablyranks rather high with community members. Working towards both long- and short-term solutions thatmeet the needs of citizenry is important. Public Health Advisory Council - The work of the public healthadvisory council is important specifically as we grow our community and are seeking equity in health caredelivery. The council is most likely dealing with issues of suicide, mental health, maternal wellness andaccess to care, as well as looking to reach youth, and impoverished communities. Aging and VeteranServices - working to find and preserve funding sources to address the needs of seniors, veterans, anddisabled members of our community. Providing access to resources and a platform for members of thesecommunities to be heard is vital to valuing the contributions of these community members. Parks andRecreation Advisory Board - Ensuring equity in access, growing both membership and communityspaces. Operation and maintenance of community spaces, funding concerns.

List any potential conflict of interest between your public and private service that mightresult from your appointment to a Board/s you are applying for.

Why are you interested in serving on the Board/s you are applying for?

I am a veteran, also I have a mother in law who has been in assisted living, and is now in a nursing home.I want to help improve the conditions of end of life, and long term care in Oregon.

Are you interested in other volunteer opportunities?

Yes No

If you are interested in other opportunities, may we contact you?

Yes No

What education, work experience, volunteer experience or life experience do you bring?

I have been on a political party board as chair, and vice chair, I am as I mentioned before a veteran, and Ihave seen first hand the conditions in veterans medical facilities, as well as nursing homes in Oregon.

Describe your understanding of the concerns and issues facing the Board/s you wish toserve on.

Oregon has embraced a large public involvement in medical care, this is very expensive, and makemanaging end of life care critical when considering that the vast majority of medical care, and expense forany person is spent at the end of life. What I am afraid that this might mean for public officials is that endof life care may look like the low hanging fruit when bean counters show up to cut expenses. I do not wantthis to happen, and hope I can encourage the state to continue to treat elderly infirmed people asindividuals entitled to dignity and respect.

List any potential conflict of interest between your public and private service that mightresult from your appointment to a Board/s you are applying for.

As I mentioned I do have a family member currently bed ridden in a nursing home in Oregon, I am also adisabled veteran, so This may seam like a conflict of interest, but I believe it will make me a morethoughtful board member.

- As a disabled military vet. I am familiar with the plight of veterans in the current system.

- I have seen first-hand the extended care situation in today’s nursing homes and hospitals. I am personally invested in making sure our seniors and vets get the best care possible as I have a family member in extended care right now.

- I try to come to the table with solutions rather than problems, I want to build the systems, and processes that will ensure our future success.

Relevant experience:

- Worked as chair for the Libertarian Party of Oregon. - I am the current vice chair of the Libertarian Party of Oregon, - Worked to build the Oregon Tenth Amendment Center.

Relevant training

- Nuclear power school

Communications:

- I have spent a lot of time working on communications. As Chair, and currently Co-chair of the Libertarian Party of Oregon, I have been able to build bridges, and talk to people who do not necessarily agree with me, and come to satisfactory agreements.

• Device for Measuring the Losses of the Electrical Power or Energy in a Three-

Phase Circuits (with Fast Transient Current).

• Electromagnetic Speed Reduction Synchronous Motor

• Device for Measuring the Root-Mean-Square Value of the Fast Transient Voltage

Accomplishments:

2002-2003 Developed and implemented the original verification method for ESD immunity

equipment. A2LA and NVLAP audits have approved the method.

2007 Developed and implemented the calibration method of the magnetic field generators for the immunity test. The method uses the serial device as a sensor of the magnetic field strength.

1989-1993

1988-1989

Architected and headed the “Automatic Control of Sewage Processing Station” project.

Created, designed and assembled the equipment, which connected the sensors and

electrical drives of the station to the controlling computer. Supervised a team of 10-15

people. The project allowed to reduce the staff of the station by more than 25%.

Architected and headed the “Regulated Output Electrical Pumps for Chemical Plants”

project. Saved over 20 times the project cost by rebuilding and re-engineering the pump

station of the Synthetic Rubber Plant. Supervised a team of 8-10 people.
