This enhances evidence about the current state of health in the population and supports future research into interventions to improve health. Efficiency and sustainability: Streamlining and automating manual processes, removing duplication of effort and reducing reliance on paper can deliver direct financial savings. This will enable clinicians, nursing and administrative staff to work more efficiently, helping to reduce unnecessary overtime costs. Efficiency gains can then be reallocated to provide additional capacity to deliver more episodes of care, which in turn contributes to improved patient experience. CHQ Patient Online Portal (POP) Parents and caregivers will be able to securely: manage specialist appointments message clinical and non-clinical teams to simplify appointments and schedule follow- up care view health records, prescribed medi- cations and test results Smart Referrals - eRefer Enables CHQ to electronically manage the triage and clinical categorisation of a referral, and upload the referral into the patient’s electronic medical record. (This forms part of the Smart Referrals iRMS state-wide rollout project, which is also being delivered by ICDP.) Smart Referrals - Reporting and Analytics Capability (R&AC) Integrates and captures near real-time data from key QH systems that support end-to-end specialist outpatient services referrals; and provides visibility of the end-to-end referral data set to enable ad-hoc reporting to identify peaks and troughs in referral activity. CHQ’s Integrated Care Strategy A primary objective of the ICDP is to provide the digital enablers to support CHQ’s overarching Integrated Care Strategy. The key components of that strategy are illustrated in the ‘Foundations of Integrated Care’ model. The model has been developed by the CHQ Integrated Care Clinical Advisory Group, chaired by Dr Dana Newcomb (Medical Director Integrated Care). Additional ieMR modules are being progressively released as part of transitioning from paper-based health records. During 2018, CHQ will have a range of digital enablement initiatives underway as part of realising its vision of becoming a fully digital healthcare service.
