Description Students complete their homework or read a novel during this enrichment period where there is a teacher or staff member who may provide assistance for homework for Math and Reading. This enrichment period is consisted of a 40-min Math session, 40-min Reading/LA session, and 30-min computer lab session. Participating students may ask their Math and Reading/LA related questions to the teachers and complete their assignments that require use of computers. Location : CMIT Elementary Library Sample schedule: 4:00 - 4:40 Math 4:40 - 4:45 break 4:45 - 5:15 computer lab 5:15 - 5:20 break 5:20 - 6:00 Reading/Language Arts 6:00 dismissal

CHESAPEAKE MATH AND IT ELEMENTARY ENRICHMENT PROGRAM CONTRACT

The Enrichment Programs will not take place if the school is closed for any reason including intersessions,

federal and/or state holidays, teacher work-days, and/or weather- related events. The undersigned agrees with

the following terms and conditions and will pay the required tuition fees and charges for enrollment as specified

below. Please read and initial each of the following terms and conditions of enrollment in CMIT

ELEMENTARY’s Enrichment Program:

I understand installment payments, as provided on the attached payment schedule, are due and payable

on or before the specified dates. Accounts not paid in full by the due date will be assessed a $25 late

fee. If payments are not received within 10 calendars days after the due date, my child’s place may be

offered to a family on the waiting list. If any of my payments are returned by the bank, a $25 returned

check fee will be due immediately, in addition to the $25 late fee.

I understand that my child’s place in the Enrichment Program(s) I have selected will be reserved on

behalf of my child only after Chesapeake Math and IT ELEMENTARY has notified me that my

child has been accepted into the program. I further understand that a non-refundable deposit for each

Enrichment Program I have selected in the amount specified on the payment schedule on page two of

this contract is required by the date specified in order to complete the enrollment of my child into the

program

I understand that if my child withdraws from the Enrichment Program(s) during the academic school

year, a 30 day written notice must be sent to Chesapeake Math and IT ELEMENTARY. In the event

of any such withdrawal, I understand that I will be obligated to pay to the end of the 30 day notice

period even if my child is not in attendance.

I understand that if a child must be excluded by administrative decision from the Enrichment Program,

no refund will be provided for current month’s installment; however, no further financial obligation

will exist.

I understand that if my child is not picked up by 4:00 pm for Stars and 6:00 pm for Scholars programs,

there is a $1 per minute late charge which is to be added to the following payment period.

I understand that all payments must be current in order to register for the Enrichment Programs in the

following school year.

Installment Payments are due even when the school is closed for part of the month for school

intersessions, federal and/or state holidays, teacher work-days, and/or weather- related events.

I understand that my child may be removed from the Enrichment program if my child does not follow

rules and procedures set for Enrichment programs.
