General Pre-Application Information

Can we schedule a meeting or call with someone to discuss our contributions application ideas? Due to the volume of applications received each year, we are unable to set up in-person or phone meetings with organizations to discuss program ideas, strategic focuses, or funding requests prior to the submission of their charitable contributions application.

What are some of the documents we should collect prior to starting the cash contributions application?• Copy of organization's IRS determination letter and W-9• Most recent IRS Form 990 as well as the most recent audited financial statements• Organization's written statement of non-discrimination• List of organization's board members• Brochure, pamphlet, or document describing the specific project, program, or initiative listed in the application• A detailed budget breakdown outlining exactly how any potential funds awarded for a specific project,

program, or initiative would be used.

Application Information and Troubleshooting

Should I save the CyberGrants LogOn ID and password created when completing an online application?Yes, the LogOn ID and password created when setting up an account are person-specific, and will be required every time the system is accessed (e.g., review submitted applications, respond to action items, submit future applications).• To maintain account security, J&J Vision employees are unable to provide you with this data if lost or forgotten.

Our nonprofit has 501(c)(3) status but received this message when entering our tax ID: “The tax ID entered was not found in the IRS database. We cannot accept applications from organizations not certified as a 501(c)(3) by the IRS.” May we submit our IRS status letter to Johnson & Johnson Vision as proof instead?As our system verifies 501(c)(3) status using the IRS database, we are unable to accept other forms of documentation as a substitute. Please contact the IRS directly if you have concerns about your nonprofit's status listing.

Can we start an application and finish it later?You can absolutely start an application and finish later, but remember it won't automatically save. If you close a partially completed application without clicking save and finish later, any data entered since the last save will be lost!• Tip: To reduce the risk of data loss, consider typing responses into a word document and then copy/pasting theminto the application. Then, if you forget to save or your browser accidentally closes, you won't lose your existing work.

How do we obtain a copy of our organization’s application for our records?• Prior to submission: Click the printer friendly version link located on any page of the application.• After submission: Log into CyberGrants, select the application to print, then click the printer friendly version link.

What do we do if we encounter an issue while completing the application?If you have an application error issue or need CyberGrants website assistance, click the need support? link located at the bottom of the Account Login screen.

What types of project, program, or initiative-specific budget details will we have to provide in the application?• Required information includes a budget breakdown for the specific project, program, or initiative that charitablefunds are being requested for.

• A form is available within the application, but organizations may use their own version as long as it provides thesame level of information requested in the sample template.

• Instead, we encourage applicants to upload as much supporting material about the project, program, or initiativerelevant to the contributions request (e.g., annual reports, brochures, online articles, metrics, etc.), and a member ofour Corporate Social Responsibility team will reach out for additional information if needed.
